The salivary microflora of institutionalized mentally retarded persons with chronic rumination was investigated, especially emphasizing the occurrence of the aciduric microorganisms such as mutans streptococci, lactobacilli and yeasts. 10 of those subjects together with 10 control subjects resident in the same institution were selected at random. The control subjects were mentally retarded but without rumination. Samples of saliva were collected about 2 hours after lunch. The results obtained were as follows: 1. Spores of Absidia corymbifera were inoculated orally into sheep with ruminal acidosis produced by feeding barley. Lesions, which developed in forestomachs of all four inoculated cases, included desquamation of superficial layers of the mucosae and focal necrosis from lamina propria to muscular layers. Granulomatous lesions were in the submucosa of three sheep. Lesions in the abomasum (two sheep) included focal necrosis, diffuse hemorrhages, and infiltration of neutrophils. Succinivibrio dextrinosolvens C18 was found to possess glutamine synthetase (GS), urease, glutamate dehydrogenase, and several other nitrogen assimilation enzymes. Rumination is the chronic regurgitation, chewing, and reswallowing of previously ingested food. The study reported here, using a parametric design, examined the control of rumination by the quantity of food eaten at meals. The subjects were three profoundly retarded individuals who chronically emitted this behavior. The quantity of food by weight ingested daily was varied in 10-oz steps in both ascending and descending series (data were collected only after breakfasts and lunches). The effects of intracerebroventricular (i.c.v.) and intravenous (i.v.) administration of dopamine and its antagonists (domperidone and metoclopramide) on forestomach motility were investigated in four sheep fitted with Nichrome electrodes and strain gauges implanted on the reticulum and the caudo-dorsal sac of the rumen. A single case where persistent and pervasive ruminations were treated by intensive audiotaped exposure to the ruminations themselves is reported. Exposure at two levels produced significant effects in an interrupted time series analysis of daily frequency, and the patient was rumination free at the end of treatment and at 6 months follow-up.
