Treatment of unstable distal clavicle fractures (Neer type IIb): a modified system using a miniature locking plate with a single button.

Distal clavicle fractures are common in patients with shoulder injuries. We retrospectively evaluated the clinical outcomes of a novel fixation technique using a miniature locking plate with a single button in patients with distal clavicle fractures associated with coracoclavicular ligament disruption. The study involved seven patients with distal clavicle fractures with a follow-up period of 12 months. All patients were diagnosed with type IIb fractures according to the Neer classification. The distal clavicle fracture was fixed with a miniature locking plate, and the coracoclavicular ligamen

The use of Tightrope device as the sole method of fixation in treating lateral end clavicle fractures.

BACKGROUND: Lateral end clavicle fractures can be challenging due to the small and often comminuted lateral fragment, problems with union and stability and implant morbidity. We retrospectively reviewed outcomes of Tightrope device in isolation to treat lateral end clavicle fractures. METHODS: Subjective and objective measures were assessed for 29 patients. The subjective comprised of functional clinical scores: Oxford shoulder score and EuroQoL5D. The objective measures were maintenance of fracture reduction, bone healing and complications. RESULTS: Median age was 36 years and 72% of cases

Prospective study of incidence and predisposing factors for clavicular fracture in the newborn.

OBJECTIVE: To determine the incidence of clavicular fracture, associated fetal and maternal risk factors and its connection with quality care control. SUBJECTS AND METHODS: A total of 3030 newborns delivered vaginally were evaluated for clavicular fractures by three separate physicians. The study group included all newborns with fractured clavicle. A control group consisted of 52 newborns delivered vaginally with no history of fractures. Maternal records were evaluated for possible predisposing factors. RESULTS: Forty-six (l.5%) newborns were found to have clavicular fractures. When compared

Functional Outcome after Simultaneous Bilateral Four-Part Proximal Humerus Fracture: A Comparison of ORIF and Hemiarthroplasty in an Individual Patient.

Simultaneous bilateral four-part proximal humeral fractures are rare. Four-part fractures of the proximal humerus are difficult for the patient and technically demanding for the surgeon. Our surgical tactic is to attempt open reduction and internal fixation where possible. We report the functional outcome in a 56-year-old female who sustained simultaneous bilateral four-part proximal humeral fractures after falling down a flight of stairs. Open reduction and internal fixation using threaded pins and tension band suture was performed on one side, and shoulder replacement hemiarthroplasty was re

Fixation of mid-third clavicular fractures with knowles pins: 78 patients followed for 2-7 years.

From January 1993 to December 1997, we treated 78 (75 fresh, 1 delayed union and 2 nonunion) fractures of the mid-third clavicle with open reduction and internal fixation using 3.8 mm Knowles pins. All patients used an arm sling for 2-6 weeks after surgery, and were told to avoid vigorous exercise or work with heavy loads for the next 6 weeks. 73 fractures healed within 6 months. 3 complications occurred relating to the surgical procedures. The final rate of bone union was 77/78, although 3 patients showed a delay in bone union and 1 fracture was revised because of a loosened Knowles pin. Afte

[Classification of fractures of the proximal end of the humerus following prognostic and therapeutic aspects].

The classification of the proximal humeral fractures according to the four-segment concept of Neer has proven to be very useful. It not only considers the fracture itself but also lesions of the soft tissue. Adapting Neer's concept we propose a modified classification of the proximal humeral fractures which we believe is clearer and more practicable. This classification is illustrated by means of drawings and typical roentgenograms. The description of the different fracture-types includes the mean characteristics such as fragment-displacement, status of the rotator cuff and blood circulation t

Arthrography in evaluation of birth injuries of the shoulder.

Birth injuries of the shoulder and proximal humerus of infants can be difficult to evaluate on plain radiographs because of lack of ossification in the proximal humeral epiphysis. We here describe arthrography of the infant shoulder and advocate it as a useful diagnostic procedure in the evaluation of humeral fractures and brachial palsies. Two infants with birth injuries of the shoulder are described. The value of dynamic fluoroscopic evaluation of the injured joint in conjunction with arthrography is illustrated in evaluation of a brachial plexus palsy.

Rapidly Destructive Arthrosis of Bilateral Humeral Heads Caused by Subchondral Insufficiency Fracture.

The bilateral shoulder pain of an 81-year-old Japanese woman due to falls persisted despite celecoxib treatment, and plain X-rays later showed bilateral collapsed humeral heads. After ruling out osteoarthritis, infectious arthritis, crystal-induced arthritis, neuropathic arthropathy, and osteonecrosis, we diagnosed bilateral shoulder joint rapidly destructive arthrosis (RDA). Lumbar bone mineral density showed very low T-score (-4.1). Primary osteoporosis was observed. Histology of biopsied humeral head indicated the features of fracture healing process: callus formation and osteoclasts withou

[A case of acromioclavicular dislocation without coracoclavicular ligament rupture accompanied by coracoid process fracture].

Acromioclavicular joint dislocation with intact coracoclavicular ligaments accompanied by fracture of the coracoid process is a rare injury. The patients are treated with conservative and/or surgical methods. A 30-year-old male patient developed type 3 acromioclavicular dislocation and coracoid process fracture due to a fall in the right shoulder. Both injuries were treated surgically. Following open reduction, the acromioclavicular joint was fixed with a Knowles pin, and the coracoid process was fixed with a 4.0-mm malleolar screw. Active-assisted rehabilitation of the shoulder was initiated

Assessment of anterior shoulder instability by CT arthrography.

Computed tomography (CT) immediately after double-contrast shoulder arthrography was taken in twenty-two young male patients with anterior shoulder instability including recurrent dislocation and subluxation. This recently developed technique called CT arthrography can provide significant information about patients with glenohumeral instability which is difficult to obtain by conventional arthrography. Information about glenoid labrum pathology is useful for proper management of the shoulder with instability. Lesions identified in this study include anterior labral defects (attenuation, tear,

Cubitus varus deformity following supracondylar fracture of the humerus. A method for measuring rotational deformity.

Cubitus varus following supracondylar fracture of the humerus in children consists of varus, hyperextension, and internal rotation deformities of the distal bone fragment of the humerus. There has been no simple clinical method of measuring internal rotation deformity. The authors report here on a method of accurately measuring the internal rotation of the shoulder in a position with the elbow at 90 degrees flexion on the back and the shoulder held at the maximum extension. When a patient with developing cubitus varus attempts to rotate his shoulder internally, there is an apparent increase in

Arthroscopy of the shoulder.

Between December 1979 and June 1982, 74 arthroscopic examinations of the shoulder were done at our institution. Arthroscopy was done in 28 shoulders to evaluate glenohumeral instability, in 23 to assess the joint and tendons before acromioplasty for chronic supraspinatus tendinitis, in 8 to diagnose or stage arthritic diseases, in 7 to define rotator cuff tears, in 3 to assess long-standing periarthritis, in 2 to visualize intra-articular fractures, in 2 to treat infections, and in 1 to evaluate the extent of pigmented villonodular synovitis. Examination of the shoulder with the patient under

Scapular dyskinesis after treatment of proximal humerus fracture, a 3-dimensional motion analysis and clinical outcomes.

BACKGROUND: The alteration of scapular kinematics can predispose patients to shoulder pathologies and dysfunction. Previous literature has associated various types of shoulder injuries with scapular dyskinesis, but there are limited studies regarding the effect that proximal humeral fractures (PHFs) have on scapular dyskinesis. This study aims to determine the change in scapulohumeral rhythm following treatment of a proximal humerus fracture as well as differences in shoulder motion and functional outcomes among patients who presented with or without scapular dyskinesis. We hypothesized that d

Irreducible anterior dislocation of the shoulder due to soft tissue interposition of subscapularis tendon.

The shoulder joint is the most unstable joint in the body and is easily dislocated. Anterior shoulder dislocation is the commonest and can be associated with glenoid and humeral fractures. Anterior shoulder dislocations are not infrequently associated with cuff tendon tears; however, anterior shoulder dislocation is easily reducible. Irreducible anterior dislocation of the glenohumeral joint is uncommon, and this could be due to bony as well as soft tissue causes. Persistent anterior dislocation due to torn subscapularis interposition in the glenohumeral joint is very rare, and only a few oper

Mortality associated with bone fractures in COPD patients.

BACKGROUND AND OBJECTIVE: COPD is well known to frequently coexist with osteoporosis. Bone fractures often occur and may affect mortality in COPD patients. However, in-hospital mortality related to bone fractures in COPD patients has been poorly studied. This retrospective study investigated in-hospital mortality of COPD patients with bone fractures using a national inpatient database in Japan. METHODS: Data of COPD patients admitted with bone fractures, including hip, vertebra, shoulder, and forearm fractures to 1,165 hospitals in Japan between July 2010 and March 2013, were extracted from t

The effect of shoulder manipulation on rotator cuff integrity.

The use of shoulder manipulation in the treatment of frozen shoulder remains controversial. Humeral fractures and neurological damage are the risks associated with the procedure. A concern of causing a rotator cuff tear exists but the incidence of iatrogenic rotator cuff tears is not reported. The purpose of this study was to assess the effect of shoulder manipulation for frozen shoulder on the integrity of the rotator cuff. In a prospective study, 32 consecutive patients (33 shoulders) with the diagnosis of frozen shoulder underwent manipulation of the shoulder under anaesthesia (MUA), 18 fem

Arthroscopic reduction and hollow screw internal fixation for Eyres Type IIIA scapular coracoid fracture: a case report.

BACKGROUND: A coracoid process fracture combined with an acromioclavicular (AC) joint dislocation is an uncommon injury that typically causes significant pain and limits shoulder movement. Open reduction and internal fixation have been the traditional treatment approach. However, arthroscopic techniques are emerging as a promising alternative for managing these injuries. CASE REPRESENTATION: A 35-year-old woman presented with right shoulder pain following an accidental fall. Imaging studies revealed a coracoid process fracture along with an AC joint dislocation. The fracture was classified as

Extra-articular knowles pin fixation for unstable distal clavicle fractures.

BACKGROUND: Unstable distal clavicle fractures may benefit from surgical treatment. This report described a new method with transacromial extra-articular Knowles pin fixation to treat these unstable fractures. The hypothesis is that the results would be improved. METHODS: Twenty-five patients with unstable distal clavicle fractures were treated by transacromial extra-articular Knowles pin fixation, coracoclavicular ligament repair and augmentation. The mean clinical and radiographic follow-up was 51 months. Functional outcomes were evaluated by a modified Constant-Murley score. RESULTS: Nine

[The arterial complications of closed injuries to the shoulder girdle].

After a period of time of 5 years, we made a review of all the patients registered at our Center. Eight cases of closed traumatism at the scapulohumeral belt, with vascular injury associated (0.2% of all closed traumatisms at this level registered in our center during this time) were reported. Six patients presented also an osteoarticular injury associated. Three different vascular injuries were showed: three cases of complete arterial section, two arterial contusions and a partial arterial section. From the two cases without osteoarticular injuries, one of them showed a complete section and t
