[Respiratory monitoring during sleep].

A review of respiratory disorders of sleep with the possible related clinical alterations are described. Techniques and methods used in non-invasive respiratory monitoring during sleep are presented. Diagnostic events and therapeutic approaches are discussed related to special patient groups: the obstructive sleep apnoea syndrome, asthma, chronic obstructive pulmonary disease and neuromuscular diseases.

Mania following sleep deprivation.

A first episode of mania is described in a previously healthy man who was partially sleep deprived for four nights. The sleep deprivation preceded the psychosis. During the psychotic episode he believed that he was the Messiah. This case is discussed in the light of reports exploring the relationship between psychosis and sleep deprivation.

Sleep spindles in epilepsy.

Spindles are a ubiquitous phenomenon in sleep, but their physiology and the effects of neurologic disorder on their frequency and amplitude are incompletely understood. We compared the incidence of three commonly defined spindle types (14-15 Hz, 12-13 Hz, and 10 Hz) and the frequency and amplitude of spindles during Stage II sleep in 50 patients with complex partial, partial and secondarily generalized, and primary generalized seizures, with and without interictal behavioral symptoms. All patients had 12-13 Hz and 14-15 Hz spindles of symmetric character in C3-A1 and C4-A2 derivations during p…

Sleep and cranial dystonia.

A nocturnal polygraphic study was performed on 10 patients with cranial dystonia (blepharospasm (BS) and oromandibular dystonia (OMD)). All patients showed impaired sleep efficiency and reduced slow and REM sleep, more marked in subjects with severe dystonia. Abnormal muscular activity decreased progressively with deeper sleep and during the first hours of the night, without disappearing. A disordered hypnic++ pattern and impaired motor control even during sleep are typical features in cranial dystonia.

Classification of sleep disorders.

This review discusses recent developments in classifying the sleep disorders. Several classification systems are presented, and the Diagnostic Classification of the Sleep and Arousal Disorders (DCSAD), published by the Association of Sleep Disorders Centers in 1979, is used as the basis for reviewing their differences. The four main sections of the DCSAD are presented: the disorders of initiating and maintaining sleep, the disorders of excessive somnolence, the sleep-wake schedule disorders, and the dysfunctions associated with sleep, sleep stages, or partial arousals. Disorders discovered sin…

[Sleep in neurologic diseases].

Neurological diseases may induce alterations in sleep which are difficult to interpret owing to the multiple interferences introduced by treatments, associated somatic symptoms and time-related changes in causal lesions. The consequences of vascular or traumatic lesions observed in man confirm the data obtained from experimental lesions created in animals, thereby throwing some light on the roles played by the cerebral cortex, the thalamus and the brain stem nuclei. In degenerative diseases polygraphic recordings show subclinical signs of extension which are exceptionally specific of any parti…

Sleep patterns of shiftworkers.

Work hours affect sleep because the sleep behavior of people on nonworkdays differs from that on workdays. For most workers, workday sleep is shorter than nonworkday sleep. On nonworkdays most workers sleep at night. Work schedule interacts with the effects of work hours on workday sleep. On workdays, people on the nightshift sleep the least, people on the afternoon/evening shift sleep the most, and people on the dayshift sleep less than those on the afternoon/evening shift but more than those on the nightshift. Thus, the infradian sleep strategy elected by a shiftworker is ordinarily related …

Benzodiazepines, breathing, and sleep.

The benzodiazepines are sedative hypnotic drugs, i.e., central nervous system depressant drugs, that may adversely affect the control of ventilation during sleep. Prescription of these drugs may worsen sleep-related breathing disorders, especially in patients with chronic obstructive pulmonary disease or cardiac failure. The most frequent users of sedative hypnotics are the polymorbid elderly with a secondary complaint of insomnia. Although the benzodiazepines may reduce sleep fragmentation, their long-term use may also cause health problems, such as complete obstructive sleep apnea in heavy s…

Sleep in postpolio syndrome.

Post-polio patients may develop additional neuromuscular and respiratory symptoms decades after the acute attack, the post-polio syndrome. We hypothesize some post-polio symptoms may be due to breathing disorders occurring during sleep. We performed polysomnography on 13 post-polio patients: group 1 (five patients) were those already on ventilatory assistance (rocking beds) and group 2 (eight patients), those without any assistance. Patients requiring new treatment were then evaluated on nasal CPAP or nasal mask ventilation. Group 1 patients, on rocking beds, demonstrated consistently poor sle…

Psychosis in sleep apnoea.

A 30-year-old man presenting with intellectual impairment and recurrent psychotic episodes was subsequently found to have suffered from a chronically untreated obstructive sleep apnoea syndrome. Polysomnography revealed sleep fragmentation, slow wave sleep deprivation and abnormal arterial oxygen desaturation. Tonsillectomy led to complete resolution of sleep apnoea and remission of psychosis at 2 years' follow-up, but his apparent intellectual impairment persisted. The limited literature on psychosis associated with sleep apnoea is briefly reviewed.

Sleep apnea in children.

Sleep apnea in children develops when airway obstruction at night is severe; however, lesser degrees of obstruction may also cause problems. The most common cause of nighttime obstruction with or without apnea is hyperplasia of the tonsils and adenoids. Other conditions such as craniofacial anomalies and neuromuscular disorders may predispose children to obstruction of the airway during sleep. Although cor pulmonale, heart failure, and cardiorespiratory arrest are the most dramatic results of obstructive apnea, before these occur many other problems may develop that are detrimental to the chil…

Sleep apnea and hypothyroidism.

Thyroid deficiency states are now a well recognized cause of the sleep apnea syndrome. The spectrum of disease ranges from mild, asymptomatic hypothyroidism to severe myxedema, and the disorder is associated with both obstructive and central types of sleep apnea. A variety of factors may be involved, including upper airway obstruction with or without obesity, and alterations in ventilatory drive. The definitive therapy is thyroid hormone replacement, which has been shown to diminish or completely eliminate apneic episodes and arterial oxygen desaturation, as well as to effect many improvements…

Electroencephalographic sleep in mania.

Electroencephalographic (EEG) sleep patterns were examined in nine unmedicated patients meeting DSM-III-R criteria for a current manic episode (four men and five women) for two to four consecutive nights. Compared with age- and sex-matched normal control subjects, manic patients exhibited significantly decreased total recording period, decreased time spent asleep, increased time awake in the last two hours of recording, shortened rapid eye movement (REM) latency, increased REM activity, and increased REM density. These results suggest that mania is associated with marked disturbances of sleep …

[Sleep in Pick disease].

19 polygraphic sleep recordings from 12 patients with Pick's Disease, including four histologically proved cases, were compared to those of an age-matched control group. Symptoms had been present for a mean 8 years, the patients being aged 59 to 78 (mean 70.5 years). All sleep stages could be identified. Total sleep time was reduced and the number of awakenings was sharply increased. High proportion of stage 1 contrasted with the reduction in the other sleep stages with disappearance of stage 4 in advanced cases. REM Sleep was identified in all recordings, although reduced as a function of the…
