Agreement between apnea-hypopnea index and hypoxic burden for severity classification of obstructive sleep apnea.

This study aimed to evaluate the agreement between apnea-hypopnea index (AHI)- and hypoxic burden (HB)-based obstructive sleep apnea (OSA) severity classifications. A retrospective single-center study of adults aged 18-79 years undergoing full-night in-laboratory polysomnography was conducted. OSA severity by AHI was categorized as normal (<5 events/hour), mild (5 to <15), moderate (15 to <30), or severe (≥30). HB severity was categorized as mild (<3.34), moderate (3.34 to 6.66), or severe (≥6.67). Agreement among patients with OSA (AHI ≥5) was assessed using Cohen's kappa. Ninety-five patient

Effects of Soft Cervical Collar on Apnea-Hypopnea Index in Patients with Obstructive Sleep Apnea.

BACKGROUND Obstructive sleep apnea (OSA) is a sleep disorder characterized by recurrent upper airway obstruction, leading to intermittent hypoxia and sleep fragmentation. Continuous positive airway pressure (CPAP) is the criterion standard treatment, but adherence remains a significant challenge. This study aimed to evaluate whether a soft cervical collar could reduce the apnea-hypopnea index (AHI) and improve oxygen saturation by preventing excessive neck flexion and airway collapse, potentially serving as an alternative or adjunctive therapy for OSA. MATERIAL AND METHODS Ethical approval was

Apnea-Specific Pulse-Rate Response is Associated With Early Subclinical Atherosclerosis in Obstructive Sleep Apnea.

INTRODUCTION: In patients with obstructive sleep apnea (OSA), novel metrics such as hypoxic burden (HB) and sleep apnea-specific pulse-rate response (ΔHR) may better correlate with cardiovascular diseases (CVD) than the apnea-hypopnea index (AHI). This manuscript aims to assess the correlation between ΔHR and HB with subclinical atherosclerosis in patients with OSA, testing the hypothesis that elevated ΔHR and HB are associated with subclinical atherosclerosis development. METHODS: In a prospective study, individuals aged 20-65 years with suspected OSA without known comorbidities were consecu

Development and validation of a diagnostic model for obstructive sleep apnea.

STUDY OBJECTIVES: Obstructive sleep apnea (OSA) is a prevalent, underdiagnosed disorder linked to serious health risks. This study developed a diagnostic model for OSA. METHODS: Clinical data were analyzed from 3,038 adults who underwent polysomnography at the Second Affiliated Hospital of Xinxiang Medical University between 2015 and 2024. Sixteen candidate predictors were initially considered and a 70:30 split was used to divide participants into training and validation groups. A multivariate logistic regression model was developed to predict OSA risk. Model performance was assessed using the

[The role of obesity and weight reduction in obstructive sleep apnea].

Obesity plays a central role in the pathomechanism of obstructive sleep apnea and is strongly associated with the severity of sleep-disordered breathing. Weight reduction in obese patients has been shown to decrease the apnea-hypopnea index (AHI), improve sleep architecture, and reduce cardiometabolic risk. However, traditional, specialty-specific models of care do not address sleep and metabolic disorders in an integrated manner, and weight management often receives less emphasis in obstructive sleep apnea care compared with positive airway pressure (PAP) therapy. Incretin-based therapies - p

The impact of exercise training on the obstructive sleep apnea endotypes.

RATIONALE: Weight-loss interventions such as exercise are often recommended to manage obstructive sleep apnea (OSA). Interestingly, exercise can modestly improve OSA severity even without significant changes in body weight. However, the mechanisms underlying the improvements in OSA severity, separate from those observed with weight loss, are currently unknown. We aimed to assess how exercise affects the OSA endotypes (upper-airway collapsibility, loop gain, arousal threshold and upper-airway muscle compensation) and identify individuals most likely to benefit. METHODS: We retrospectively anal

The effect of pharyngeal structures on the severity of obstructive sleep apnea.

BACKGROUND/OBJECTIVE: Obstructive sleep apnea (OSA) is characterized by complete or partial cessation of breathing during sleep. The tongue is suggested as a possible anatomical site causing airway obstruction. However, the role of other pharyngeal structures in the development of OSA remains unclear. We designed a study using both the apnea-hypopnea index (AHI) and the oxygen saturation measurements to assess the severity of OSA. We aimed to identify critical anatomical structures of the upper airway that correlate with the severity of OSA and to evaluate the utility of magnetic resonance ima

Overview of the Role of Pharmacological Management of Obstructive Sleep Apnea.

Obstructive sleep apnea (OSA) remains a prominent disease state characterized by the recurrent collapse of the upper airway while sleeping. To date, current treatment may include continuous positive airway pressure (CPAP), lifestyle changes, behavioral modification, mandibular advancement devices, and surgical treatment. However, due to the desire for a more convenient mode of management, pharmacological treatment has been thoroughly investigated as a means for a potential alternative in OSA treatment. OSA can be distinguished into various endotypic or phenotypic classes, allowing pharmacologi

Pathophysiology of the comorbidity of glaucoma with obstructive sleep apnea: A postulation.

Glaucoma is a serious and progressive optic neuropathy, the exact pathophysiology of which is still poorly understood. Furthermore, glaucoma exhibits significant comorbidity with obstructive sleep apnea (OSA) that warrants an in-depth study in view of highly probable beneficial and far-reaching clinical implications. In this brief paper, the authors have studied the existing theories in an attempt to explain the comorbidity and its underlying pathophysiology. From the ensuing evidence, the role of connective tissue strength has emerged as a major factor and which appears to play a pivotal role

Dreams and Nightmares in Patients With Obstructive Sleep Apnea: A Review.

Obstructive sleep apnea (OSA) can present with or provoke various psychological symptoms. In this article, we critically review studies that have examined dreams, dream recall, and dream content in patients with OSA. Obstructive events induce recurrent sleep fragmentation and intermittent desaturations in patients with OSA, which may trigger different parasomnias, including nightmares. Contradictory results have been reported concerning dreams in patients with OSA; while some investigators have reported less dreams in OSA patients, others have described that patients with OSA have increased dr

Utility of portable monitoring in the diagnosis of obstructive sleep apnea.

Obstructive sleep apnea (OSA) is a common but underdiagnosed sleep disorder, which is associated with systemic consequences such as hypertension, stroke, metabolic syndrome, and ischemic heart disease. Nocturnal laboratory-based polysomnography (PSG) is the gold standard test for diagnosis of OSA. PSG consists of a simultaneous recording of multiple physiologic parameters related to sleep and wakefulness including electroencephalography (EEG), electrooculography (EOG), surface electromyography (EMG), airflow measurement using thermistor and nasal pressure transducer, pulse oximetry and respira

Orexin: a potential role in the process of obstructive sleep apnea.

Obstructive sleep apnea (OSA) is a complicated disease with an unrecognized mechanism. Obesity, sex, age, and smoking have been found to be independent correlates of OSA. Orexin (also named hypocretin) mainly secreted by lateral hypothalamus neurons has a wide array of biological functions like regulating sleep, energy levels and breathing. Several clinical studies found ties between orexin and OSA. Because of the close correlation between orexin and obesity, sex, age and smoking (which are the key risk factors for OSA patients), we hypothesize that orexin may play a key role in the pathogenes
