Sleep Disturbance in Bipolar Disorder: Neuroglia and Circadian Rhythms.

The worldwide prevalence of sleep disorders is approximately 50%, with an even higher occurrence in a psychiatric population. Bipolar disorder (BD) is a severe mental illness characterized by shifts in mood and activity. The BD syndrome also involves heterogeneous symptomatology, including cognitive dysfunctions and impairments of the autonomic nervous system. Sleep abnormalities are frequently associated with BD and are often a good predictor of a mood swing. Preservation of stable sleep-wake cycles is therefore a key to the maintenance of stability in BD, indicating the crucial role of circa

Sleep-disordered Breathing in Neuromuscular Disease.

Patients with neuromuscular disorders involving respiratory muscles (upper airway muscles, respiratory accessory muscles, diaphragm, even abdominal muscles that stabilize the chest) have more significant problems with breathing during sleep, especially during REM sleep, than during wakefulness. There are means of appropriately treating sleep disordered breathing and improving the quality of life of these patients. Treatment helps to avoid daytime symptoms and additional autonomic nervous system dysfunction. The treatment involves support of breathing during sleep. It must be adjusted to the se

Stress and sleep: Novel insights into stress-related sleep disorders from EEG features to neural circuitry.

Sleep disorders and stress-related psychiatric conditions exhibit a high degree of comorbidity. Epidemiological patterns suggest that sleep disturbances frequently precede the onset of affective symptoms. This temporal relationship indicates that stress-induced disruption of sleep physiology may act as an early driver in the pathogenesis of psychiatric disorders. Current clinical assessments relying on subjective measures or standard polysomnography staging may overlook subtle neurophysiological alterations. This review examines the impact of stress on sleep dynamics through the lens of specif

Progress of non-motor symptoms in early-onset Parkinson's disease.

Parkinson's disease (PD) is a common degenerative disease of the central nervous system that is characterized by movement disorders and non-motor symptoms (NMSs). The associated NMSs primarily include neuropsychiatric symptoms, autonomic dysfunction, sleep-wake disorders, pain, fatigue, and hyposmia. These NMSs can occur at any stage of PD, especially before the onset of motor symptoms, and may affect a patient's quality of life more than motor symptoms. Although PD is most commonly diagnosed in people over 65 years, some patients exhibit symptom onset before the age of 50, which is clinically

[Sleep and neurodevelopment: a timely subject].

Neurodevelopmental disorders (NDDs) include a heterogeneous group of conditions characterized by an alteration of neurodevelopmental processes leading to a significant impact on the individual's personal, social, and/or academic functioning. Among the many comorbidities frequently associated with NDDs, sleep disturbances play a major role, affecting between 30% and 80% of concerned people. This article explores the various links between sleep and NDDs through a review of the scientific literature highlighting the most recent research in this area, with a particular focus on two neurodevelopmen

Relationship between social rhythms and mood in patients with rapid cycling bipolar disorder.

Disruptions in the sleep-wake cycle frequently characterize affective illness and have led to a number of theories linking sleep-wake and/or circadian rhythm disturbance to affective illness. Recently, researchers have expanded these chronobiological theories to include the role of lifestyle regularity, or daily social rhythms. In this study, the Social Rhythm Metric (SRM) was used to explore the relationship between social rhythms and mood in patients with rapid cycling bipolar disorder and to compare the social rhythms of patients with those of healthy control subjects. Patients' SRM scores

Overweight and obese children with sleep disordered breathing have elevated arterial stiffness.

BACKGROUND: The prevalence of obese children with sleep disordered breathing (SDB) is increasing. Obesity and SDB are independent cardiovascular risk factors, of which arterial stiffness is an early sign. Pulse wave velocity (PWV), is a marker of arterial stiffness and central systolic blood pressure (cSBP) is a better predictor of cardiovascular outcome than peripheral blood pressure. Therefore, we aimed to determine PWV and cSBP in overweight/obese or normal weight children with sleep disordered breathing (SDB), and non-snoring normal weight controls. METHODS: Children (3-18 y) with SDB (ov

Comparative distribution of the hypothalamic neurons activated during wakefulness and paradoxical (REM) sleep using male TRAP2-red mice: contribution of orexin, MCH, Lhx6, and a new marker Meis2.

STUDY OBJECTIVES: Paradoxical sleep (PS) is a state involving numerous hypothalamic neuronal subpopulations, many remaining neurochemically uncharacterized. Our goal was to compare hypothalamic neurons active during wakefulness or PS rebound (PSR) and explore their potential overlap, with a focus on melanin-concentrating hormone (MCH), Orexin (Orx), Lhx6, and a new contingent of Meis2-expressing neurons. METHODS: In the same male TRAP2-red mouse, neurons activated during wakefulness (4 h) and PSR (2 h) express TdTomato and c-Fos, respectively. Double-labeling and triple immunofluorescence wit

Sound asleep, yet still listening: auditory processing during human sleep.

Sleepers can wake to their name, but ignore louder irrelevant sounds, a paradox that suggests more than simple sensory gating. Growing evidence reveals flexible auditory coupling during human sleep, ranging from complete disconnection to high-level cognitive processing and even to physical responses. Here, we synthesize the multiple cognitive levels at which auditory stimuli are processed (or not) during human sleep and outline factors modulating this processing. Moments of sensory isolation may promote restoration and memory consolidation, while moments of environmental coupling enable threat

The role of the hypothalamic neuropeptides hypocretin/orexin in the sleep-wake cycle.

The novel neuropeptides hypocretin/orexin have recently been located on the lateral hypothalamus cells. This system has been linked to the regulation of both feeding and sleep, and recent studies have found an association between a defect in these neuropeptides and narcolepsy. We conducted a MEDLINE review of all the articles published since the discovery of hypocretin/orexin peptides, narrowing the field to the relationship between these neuropeptides and sleep. The finding of a deletion in the transcription of the hypocretin receptor 2 gene in narcoleptic Doberman pinschers and the developme

Bidirectional interactions between the baroreceptor reflex and arousal: an update.

Studies involving genetic engineering on animal models and mathematical analysis of cardiovascular signals on humans are shedding new light on the interactions between the arterial baroreceptor reflex (baroreflex) and arousal. Baroreceptor stimulation, if very mild or performed under anaesthesia, may inhibit cortical arousal. However, substantial increases or decreases in baroreflex activation cause arousal in animal models and human subjects in physiological conditions. On the other hand, cardiovascular changes during autonomic arousals and between the states of wakefulness and sleep involve

Sleep alterations following exposure to stress predict fear-associated memory impairments in a rodent model of PTSD.

Sleep abnormalities, such as insomnia, nightmares, hyper-arousal, and difficulty initiating or maintaining sleep, are diagnostic criteria of posttraumatic stress disorder (PTSD). The vivid dream state, rapid eye movement (REM) sleep, has been implicated in processing emotional memories. We have hypothesized that REM sleep is maladaptive in those suffering from PTSD. However, the precise neurobiological mechanisms regulating sleep disturbances following trauma exposure are poorly understood. Using single prolonged stress (SPS), a well-validated rodent model of PTSD, we measured sleep alteration

Sleep problems in children with autism.

Autism is a developmental disability characterized by severe deficits in social interaction and communication, and the presence of repetitive-ritualistic behaviors. Sleep problems are frequently reported by parents of children with autism with prevalence estimates of 44-83% for sleep disorders in this population. To better understand sleep in autism, we surveyed sleep problems in 210 children with autism using a Likert-based questionnaire for parent report. The most frequently reported sleep problems included difficulty in falling asleep, restless sleep, not falling asleep in own bed, and freq

Sleep-related motor and behavioral disorders: Recent advances and new entities.

Patients with sleep-related motor and behavioral disorders present to a variety of subspecialty clinics (neurology, sleep medicine, respiratory medicine, psychiatry). Diagnosing these disorders can be difficult, and sometimes they have a significant impact on quality of life. Alongside a number of common and well-recognized conditions, several new disease entities have been described in recent years that present with abnormal nocturnal motor phenomena (such as ADCY5-associated disease and anti-IgLON5 disease). Our understanding of the neural basis and prognostic significance of other sleep-rel

[Nighttime eating disorders--clinical symptoms and treatment].

Nighttime eating is categorized as either night eating syndrome (NES) or the sleep-related eating disorder (SRED). Both diseases are often connected with an increase of the body mass, obesity, and with psychiatric disturbances. NES is characterized by evening hyperphagia, abnormally increased food intake after the evening meal, nocturnal awakings with ingestions, morning anorexia, and insomnia. Patients suffering from NES are aware of their nocturnal ingestions. It is suggested that NES is an abnormality in the circadian rhythm of meal timing that occurs in people with normal circadian rhythm

The role of leptin in rodent and human sleep: A transdiagnostic approach with a particular focus on anorexia nervosa.

This narrative review addressed to both clinicians and researchers aims to assess the role of hypoleptinemia in disordered sleep with a particular focus on patients with anorexia nervosa (AN). After introducing circadian rhythms and the regulation of circulating leptin, we summarize the literature on disordered sleep in patients with AN and in fasting subjects in general. We highlight novel single-case reports of substantially improved sleep within days after initiation of off-label metreleptin treatment. These beneficial effects are set in relationship to current knowledge of disordered sleep
