This article presents some of the less frequently described nerve pathologies associated with severe plantarflexion-inversion ankle sprains. It outlines the likely mechanisms of nerve injury, the typical presentations, and treatment possibilities. An anatomic review of the lower extremity, with emphasis on the neurologic structures, is also included to assist in understanding the mechanism and location of the nerve injury.

Both ankle and hip movements are used to maintain balance in strategies defined as bottom up and top down models. This suggests that pathological impairment of either of these articulations could modify the parameters of balance control. In order to appreciate the bearing of ankle damage on the proper control of equilibrium, posturographic recordings were compared, in a static and two dynamic tests, between 15 professional national basketball players, with histories of 10 to 15 ankle sprains, and 50 controls. In statokinesigrams obtained with eyes open or closed, center of foot pressure displa

Lateral ankle instability can present as either acute or chronic. Proper identification of the etiology is necessary to determine the proper treatment plan. The Brostrom-Gould procedure, when properly used, can provide adequate correction of the unstable ankle. A review of the procedure, its indications, and the results of 44 patients who underwent the procedure will be discussed.

In our highly active society, individuals and groups push their physical limits. Consequently, the incidence of whiplash injury is increasing. Patients may not recover rapidly or fully from a whiplash injury, thus leading to chronic neck and spinal symptoms and litigation. Physicians must recognize whiplash injury as a manifestation of total-body trauma and treat accordingly, with particular emphasis on alleviating abnormal tension of the fascia. Precise description of the accident, followed by healing methods tailored to well-defined bodily injury, aids in effective management. Whiplash injur

A random clinical trial was performed to determine the immediate effects of early ankle mobilization on active duty military members with moderate and severe lateral ankle sprains. Sixty-four subjects were randomly allocated to one of three treatments: a plaster cast, an air-stirrup (Aircast Inc., Summit, New Jersey), or an elastic wrap. Each intervention lasted 2 weeks followed by 3 weeks of rehabilitation. Subjects who were allowed early mobilization returned to work and running significantly sooner than those placed in plaster casts (p = 0.029). Swelling (p = 0.009) and pain (p < 0.001) dec

An understanding of the anatomy and biomechanics of the lateral ankle ligaments is essential to make a proper diagnosis in patients with ankle sprains. Specific radiographic stress tests can aid in differentiating single-versus double-ligament injuries and in determining their severity. Inadequately treated ankle sprains can result in chronic pain, instability, and early osteoarthritis. Thus, the proper diagnosis and initial treatment are important. This paper reviews the essential features of the anatomy, biomechanics, diagnosis, and natural history of lateral ankle sprains.

This paper reviews the treatment alternatives for acute and chronic lateral ankle ligament sprains. Inadequately treated ankle sprains can result in chronic lateral ankle instability, disabling pain, and the early onset of osteoarthritis. There are a multitude of reconstructive techniques used for chronic lateral ankle instability. Morbidity associated with present techniques includes loss of proprioception, stiffness following cast immobilization, loss of subtalar motion, loss of internal rotation of the talus during ankle plantar flexion, and recurrent instability. In addition, this paper re

Failure in muscle strain injuries has been reported to occur within the muscle belly, at the myotendinous junction, or within muscle near the myotendinous junction. The goal of this investigation was to determine by electron-microscopic examination the site of lesion in whole muscle strained to failure. In addition, site and conditions for failure of stimulated and unstimulated muscle were compared. Frog semitendinosus myotendinous units with intact tendon-bone junctions were strained at physiological strain rates to failure. All failures occurred at or near the proximal myotendinous junction

A poorly diagnosed and treated wrist injury could become a career-ending injury. Optimal treatment begins on the playing field, not the operative field. A careful, thorough field evaluation is mandatory to ensure prompt recognition of a potentially debilitating injury. Suspected fractures or ligamentous disruption require immediate referral to a physician for definitive early treatment. When the trainer and physician work as a team, early diagnosis, treatment, and rehabilitation of wrist injuries can be instituted to reach the goal of maintaining functional healthy wrists for future athletic e

We studied the long-term sequelae of hand injuries as a result of playing volleyball. In a retrospective study, 226 patients with injuries of the hand who were seen over a 5-year period at our Trauma Department, were investigated. Females accounted for 66% of all injuries. The mean age was 26 years, with a peak in the age group of 15 to 29 years. Sprains and strains were observed most frequently (39%), followed by fractures (25%) and contusions (16%). The fingers were involved in 44% of the cases. Most injuries of the hand occurred in recreational players. Recreational players had more left-si…

By means of a questionnaire, an inventory was made of the residual complaints and possible handicaps 9 months post-injury in 1012 patients who had been treated for distortion or lateral ankle ligament injury. The response rate was 81% and responses were equally distributed among the various grades of distortion. Residual complaints were reported by 30% of the patients. The injuries were classified into three grades. For all grades, the nature and frequency of the residual complaints were the same. Explanations for the study results or guidelines for treatment cannot be derived from this study.

As has been previously discussed, the incidence and resultant associated disabilities of ankle sprains have been well documented in the literature. The staggering statistics on long-term disability show that there is no such thing as a simple ankle sprain. The degree of disability is related to the extent of the initial injury as well as the follow-up medical care provided. It is this fact that requires a complete understanding of the injury as well as a proper treatment and rehabilitation program. One of the reasons cited for the long-term disability or lack of consistently good results in tr

A rare injury in young children at the ankle joint is described. In two cases a chronic fibular ligament insufficiency was found. Both children were treated operatively by extraepiphyseal plate ligament plasty, which gave excellent stabilization and no permanent disability.

Twenty patients with chronic ankle instability were examined radiographically for anterior talar displacement and talar tilting. The examinations were carried out with the ankles untaped and taped, and the taped ankles were examined again after 20 min running. The measurable instability was significantly improved after taping, but after exercise this was seen only for talar tilting. Generally, the best stabilizing effect of taping was obtained in the ankles with the greatest degree of instability. After exercise, all but one of the adhesive taping bandages were loose and were mostly acting as

We report the first series of proximal interphalangeal joint sprains and reviewed 50 sprains in 48 patients. The majority of patients were males injured at work. The radial collateral ligament was injured about twice as often as the ulnar ligaments. The little finger was injured significantly less than the others. Proximal interphalangeal joint sprains can be diagnosed by the following: pain localized to the injured ligament, swelling greatest at the site of the injured ligament, tenderness greatest at the site of the injured ligament, pain at the site of the injured ligament during lateral st

Anterior tibiotalar impingement is caused by extension at the ankle. Repeated impingement causes two symptom-producing and potentially disabling syndromes. The most frequent is impingement exostoses which form on strong feet that offer resistance to impingement. This condition may be compatible with dancing for many years, but if symptoms warrant, surgery is a consideration. The other syndrome is found in young dancers with long, slender, flexible feet. These feet are weak and allow midfoot sag and plantar fascial stretch causing chronic foot strain. Such feet do not tolerate dancing without e
