Common abdominal emergencies in children.

Because young children often present to EDs with abdominal complaints, emergency physicians must have a high index of suspicion for the common abdominal emergencies that have serious sequelae. At the same time, they must realize that less serious causes of abdominal symptoms (e.g., constipation or gastroenteritis) are also seen. A gentle yet thorough and complete history and physical examination are the most important diagnostic tools for the emergency physician. Repeated examinations and observation are useful tools. Physicians should listen carefully to parents and their children, respect th

Prognostic value of anion gap calculation in cattle with abomasal volvulus: 58 cases (1980-1985).

Values for anion gap, serum chloride, and base excess were tabulated for 58 dairy cows with abomasal volvulus. Test values for survivors (n = 40) and nonsurvivors (n = 18) were compared. Surviving cattle were released for production or salvage. Nonsurviving cows died or were euthanatized. Accuracy of preoperative anion gap, serum chloride, and base excess values in predicting outcome was evaluated by calculating the sensitivity, specificity, predictive value, and negative predictive value of these at assigned cutoff values. Anion gaps greater than or equal to 30 mEq/L were indicative of a poor

Repair of symptomatic diaphragmatic hernia during pregnancy.

Congenital diaphragmatic hernia complicating pregnancy is a rarity, accounting for only six out of 17 cases of diaphragmatic hernia reported in the English literature. This case report describes the first successful repair of an acutely symptomatic foramen of Bochdalek hernia during pregnancy, with maternal and fetal survival. In the asymptomatic patient, surgery should be performed promptly on an elective basis in the first and second trimesters. During the third trimester, an asymptomatic defect should be repaired at the time of elective cesarean section. Active labor should be avoided. If s

[Which hiatal hernias require surgery?].

Several clinical situations may justify surgery for hiatal hernia. The approach varies depending on whether the problem is a mechanical complication or a gastro-oesophageal reflux responsible for oesophagitis, peptic stenosis, chest pain, respiratory disorders or Barrett's oesophagus. Recurrence of reflux after surgery raises even more complex problems. Evaluation by modern exploratory techniques helps in establishing precise indications for the operation which must, be reserved to selected cases.

GERD surgery in non-neurologic patients: Modified Laparoscopic Hill-Snow Repair is a valid alternative to Nissen fundoplication. Results of a 20 years of follow-up.

Nowadays laparoscopic Nissen fundoplication represents the gold standard in surgical treatment of complicated Gastro-Esophageal-Reflux Disease (GERD), above all in cerebral palsy patients. In non-neurological patients without gastrostomy Nissen fundoplication can create some problems (gas bloat syndrome, dysphagia). Laparoscopic Hill-Snow repair is an established surgical alternative, but it is reported only in adult population. We describe our modification of Hill-Snow technique and our experience in a large series of non-neurological children in order to report its effectiveness and applicab

Acute obstruction of a chronic organoaxial gastric volvulus detected on blood pool imaging of a bone scan.

Acute obstruction of a chronic gastric volvulus was incidentally detected on blood pool imaging of a total body bone scan performed for an elevated prostate-specific antigen level. The patient had been diagnosed with an organoaxial gastric volvulus within a paraesophageal hernia 4.5 years previously, with no evidence for obstruction. The patient remained asymptomatic until becoming acutely obstructed on the day of his bone scan. This case gives further evidence for the additive utility of routine total body blood pool imaging for detecting and characterizing both osseous and nonosseous patholo

Total parenteral nutrition-associated cholestasis in surgical neonates may be reversed by intravenous cholecystokinin: a preliminary report.

Neonatal infants who require total parenteral nutrition (TPN) after major operations are susceptible to total parenteral nutrition-associated cholestasis (TPNAC). A therapeutic dilemma ensues if cholestasis does not resolve after the institution of full enteral nutrition. The authors report the reversal of TPN-associated cholestasis by intravenous cholecystokinin in eight infants who had undergone major surgery during the neonatal period. The indications for surgery were necrotizing enterocolitis in three patients, midgut volvulus in one, gastroschisis in one, diaphragmatic hernia in one, necr

Outcomes of non-obstructed gastric volvulus: Repair during the index hospitalization vs early elective setting.

INTRODUCTION: There is a current gap in literature regarding the appropriate timing of non-emergent repair for a resolved obstructing gastric volvulus. This study aims to evaluate outcomes related to hiatal hernia repair (HHR) during the index hospitalization versus early outpatient repair, with the assumption that outpatient repair will result in better outcomes. METHODS: This study included HHR data from 2017 to 2022. The primary outcome assessed overall morbidity using the Clavien-Dindo classification among various secondary outcomes. RESULTS: A total of 25 patients were included. There w

Cutaneous presentation of plasmablastic post-transplant lymphoproliferative disorder in a 14-month-old.

We present a 14-month-old female child who developed multiple erythematous nodules on her abdomen 5 months after liver and small bowel transplantation. Skin biopsy revealed a dense infiltrate of large cells in the dermal and subcutaneous layers with frequent mitotic figures. The cells were noted to have abundant cytoplasm, prominent nucleoli, and open chromatin. Immunohistochemical stains were positive for CD138, CD56, Ki67 (>90%), and lambda chain restriction. Rare mature B cells (CD20) and rare T cells (CD3) were noted. She was diagnosed with high-grade post-transplant lymphoproliferative di

Management of acute paraesophageal hernia.

BACKGROUND: Acute paraesophageal hernia is a surgical emergency presenting with sudden chest or abdominal pain, dysphagia, vomiting, retching or significant anemia. Severe cases can present with respiratory failure or systemic sepsis. This can be due to gastric volvulus, incarceration, strangulation, severe bleeding or perforation. Traditionally this has been treated with an open surgery. The purpose of this study is to develop a management algorithm and evaluate the role of a laparoscopic approach for these cases. METHODS: A retrospective chart review was performed for patients operated on f

[Two rare complications of percutaneous endoscopic gastrostomy: obstruction of the pylorus and gastrocolic fistula occurring in one patient].

UNLABELLED: We report an 10 month old male infant, who developed an obstruction of the pylorus by dislocation of the retention disk 5 months after percutaneous endoscopic gastrostomy (PEG). This could be corrected under endoscopic control. Two years after PEG insertion diarrhoe occurred immediately after feeding caused by a gastrocolic fistula with dislocation of the retention disk in the colon transversum. An excision of the fistula and a resection of the colon segment were performed successfully. CONCLUSION: In patients with PEG and unclear abdominal symptoms a tube dislocation has to be ke

A retrospective study of factors influencing survival following surgery for gastric dilatation-volvulus syndrome in 306 dogs.

BACKGROUND: Identification of specimens that contain ethylenediaminetetraacetic acid (EDTA) is frequently necessary when investigating potentially mislabeled or improperly collected specimens. OBJECTIVE: To evaluate the performance of rapid EDTA detection test strips in clinical specimens. METHODS: We applied specimens to test strips designed to detect EDTA (QUANTOFIX EDTA) using a pipet (drop mode). Reactions were scored visually on a scale from red (no EDTA) to orange (low/indeterminate EDTA) to yellow (contains EDTA). RESULTS: Test strips reliably identified specimens from EDTA-containin
