Influence of neuromuscular transmission and Na+ channel inhibitors on strength-duration relationship in isolated slow- and fast-twitch muscles.

Strength-duration (S-D) relationship reflects muscle excitability which partially determines force production. Isolated muscle contains part … ion. ThFS significantly increased in the presence of NMT inhibitors including pancuronium (Pan) and succinylcholine in both SOL and EDL muscles. Moreover, ThFS in the presence of NMT inhibitors was higher in SOL compared to EDL. To clarify the mechanism of the different ThFS between SOL and EDL, contribution of Na+ channel was investigated. In the presence of 1 μ …

The current status of malignant hyperthermia.

Malignant hyperthermia (MH) is a rare and life-threatening pharmacogenetic disorder triggered by volatile anesthetics, the depolarizing muscle relaxant succinylcholine, and rarely by strenuous exercise or environmental heat. The exact prevalence of MH is unknown, and it varies from 1:16 000 in Denmark to 1:100 000 in New York State. The underlying mechanism of MH is excessive calcium release from the sarcoplasmic reticulum (SR), leading to uncontrolled skeletal muscle hyper-metabolism. Genetic mutations in ryanodine receptor type 1 ( RYR1) and CACNA1S have been identified in approximately 50% …

Entrapment of a laryngotracheal topical anesthesia kit during tracheobronchial foreign body removal: a case report.

BACKGROUND: In order to reduce the irritation of the airway during tracheobronchial foreign body (TFB) removal, tracheal surface anesthesia … gen saturation (SpO2) decreased to 91% and her heart rate dropped from 150 to 100 bpm. Atropine and succinylcholine were administered intravenously immediately, then the surgeon tried to free the TFB by pushing it back into the trachea, after which the LTA applicator was easily withdrawn, and TFB was removed successfully. The girl was discharged from hospital wit …

Anaesthetic management of patients with myopathies.

: The anaesthetic management of patients with myopathies is challenging. Considering the low incidence and heterogeneity of these disorders, … h mitochondrial myopathies may be problematic, considering the risk for propofol-infusion syndrome. Succinylcholine is contra-indicated in all patients with myopathies. Following an individual risk/benefit evaluation, the use of volatile anaesthetics in several non-MH-linked myopathies (e.g. myotonic syndromes, mitochondrial myopathies) is considered to be well t …

[Heart failure and anaphylactic shock. A report of two cases].

Anaphylactic shock can sometimes take the appearance of heart failure, in relation to an acute coronary syndrome, even with normal coronary arteries, that we illustrate by two observations. We firstly report the case of an anaphylactic shock caused by succinylcholine, after anesthesia induction for inguinal hernia surgery in a 50-year-old man with cardiovascular risks, who presented with ventricular fibrillation followed by a cardiac arrest. An acute and severe anterior coronary syndrome was suspected and treated with thrombolysis. Then the electrocardiogram normalized, as well as the left ven…

Malignant hyperthermia developing during esophageal resection in an 82-year-old man.

Malignant hyperthermia (MH) is a rare but fatal complication that develops under general anesthesia. Reports on MH in patients over the age of 80 years are unusual. We experienced a case of MH in an 82-year-old patient during esophageal resection. Anesthesia was induced with propofol and succinylcholine, and maintained with sevoflurane. Neither masseter spasm nor rigidity of the limbs was seen during induction. Body temperature (BT) at induction was 36.0 degrees C. Three hours after incision, the level of end-tidal CO2 was elevated to 55 mmHg. We assumed that the rise in end-tidal CO2 had occu…

Customized anesthetic preservation of ictal threshold in electroconvulsive therapy: role of adjunctive remifentanil with etomidate.

We present a case of anesthesia for electroconvulsive (ECT) therapy that was complicated by emetic sensitivity to etomidate, fragile ictal threshold, and mild pseudocholinesterase deficiency. The anesthetic was designed in this patient taking all his issues in consideration. The mild pseudocholinesterase deficiency necessitated a (50-75%) reduction in succinylcholine dosage, careful monitoring of the train of four, and postictal amnestic coverage to prevent paralysis upon waking. The significant emetic response to etomidate prompted substitution to propofol and preemptive ondansetron. Propofol…

[Are we prepared to diagnose and managed malignant hyperthermia?].

BACKGROUND AND OBJECTIVES: Malignant Hyperthermia (MH) is an uncommon but potentially lethal disease associated to halogenate agents and/or succinylcholine exposure. Resulting mortality and morbidity may be decreased by early diagnosis and specific treatment, including sodium dantrolene. Brazil has approximately 160 million inhabitants assisted by more than 6000 anesthesiologists. In the last decade, special attention was given to this disease, resulting in better informed anesthesiologists and more prepared hospitals to treat malignant hyperthermia (MH). This study aimed at measuring the leve…

A randomized multicenter study of remifentanil compared with halothane in neonates and infants undergoing pyloromyotomy. I. Emergence and recovery profiles.

UNLABELLED: Pyloric stenosis is sometimes associated with hemodynamic instability and postoperative apnea. In this multicenter study we examined the hemodynamic response and recovery profile of remifentanil and compared it with that of halothane in infants undergoing pyloromyotomy. After atropine, propofol, and succinylcholine administration and tracheal intubation, patients were randomized (2:1 ratio) to receive either remifentanil with nitrous oxide and oxygen or halothane with nitrous oxide and oxygen as the maintenance anesthetic. Pre- and postoperative pneumograms were done and evaluated …

Studies on the neuromuscular blocking action of commercial paraquat in mouse phrenic nerve-diaphragm.

The neuromuscular blocking actions of commercial and pure paraquat on the mouse phrenic nerve-diaphragm were compared. Twitch responses of mouse diaphragm to nerve stimulation were inhibited by commercial paraquat (5-500 microM), but not by pure paraquat (1 mM). At concentrations greater than 30 microM, commercial paraquat also directly inhibited the contractions induced by electrical stimulation of the muscle. The inhibitory actions of commercial paraquat on twitches were potentiated by pretreatment with either 0.7 microM d-tubocurarine or 2.2 microM succinylcholine. Moreover, commercial para…

[Late postoperative apnea in a premature newborn infant].

We report the case of a premature newborn child (36 weeks) who was operated on a teratoma of the sacrum when he was 12 days old and weighed 2,950 g. The patient presented a late postoperative apnea 17 hours after anesthesia. The anesthetic technique consisted of lumbar epidural blockade with 0.33% bupivacaine at a dose of 2.25 ml and superficial inhalation anesthesia with 0.5% isoflurane. Relaxing muscular agents used in this case were succinylcholine (3 mg) for orotracheal intubation and pancuronium bromide (0.3 mg) for maintaining the anesthetic level. The immediate postoperative phase was u…

The rapidly acting fazadinium produces fast relaxation similar to that produced by succinylcholine, while its effect is not associated with the side-effects of the former agent, i.e. hyperkalaemia, fasciculations. As a result of the latter, it is a useful aid in preventing the always threatening aspiration in the anaesthesia of caesarean sections …
