Computational single-neuron mechanisms of visual object coding in the human temporal lobe.

Understanding how the human brain encodes visual objects involves deciphering the neural computations and circuits in the temporal lobe. Here, we recorded intracranial EEG from the human ventral temporal cortex (VTC) and medial temporal lobe (MTL), as well as single-neuron activity in the MTL, to investigate the computational mechanisms of neural object coding. The VTC exhibited axis-based feature coding, and a neural feature space could be constructed using VTC neural axes, within which visual objects clustered according to high-level categorical relationships. Importantly, MTL neurons encode…

Facial emotion recognition in temporal lobe epilepsy: Behavioral performance, reaction time and alexithymia.

OBJECTIVE: Facial emotion recognition (FER) deficits are well documented in temporal lobe epilepsy (TLE), but most studies have focused on recognition accuracy, with limited attention to reaction time (RT) and its relationship with alexithymia. This study aimed to compare FER accuracy, RT, and alexithymia levels between patients with TLE and healthy controls (HCs) and to examine associations between FER performance and alexithymia. METHODS: Twenty-two patients with unilateral TLE and 30 age-, sex-, and education-matched HCs were included. FER accuracy and RTs were assessed using a standardize…

Scalp Electroencephalography Markers of Hidden Interictal Epileptiform Discharges in the Mesial Temporal Lobe.

Interictal epileptiform discharges (IEDs) are crucial for epilepsy diagnosis but are often undetectable on scalp EEG (scEEG). This study aims to develop a Support Vector Machine classifier to detect mesial temporal lobe (MTL) IEDs invisible on scEEG using simultaneous intracranial EEG (iEEG) and scEEG recordings and to identify contributing scEEG features. Data from 17 patients with epilepsy were analyzed. IED epochs were extracted where IEDs were present on iEEG but absent on scEEG. Control epochs were selected from periods without IEDs on both Electroencephalographies (EEGs). Feature selecti…

Abnormal functional connectivity patterns in temporal lobe epilepsy-An international ENIGMA-epilepsy study.

OBJECTIVES: Temporal lobe epilepsy (TLE) impacts multiple brain networks. Aberrant functional connectivity has been demonstrated in resting-state networks (RSNs) that mediate higher brain functions in TLE. This study aimed to identify the reproducible patterns of altered functional connectivity in TLE in a large, international cohort through ENIGMA-Epilepsy. METHODS: Resting-state functional MRI datasets from nine centers across North America, South America, Europe and South Africa, including 442 people with TLE and 387 healthy adults, were analyzed. We examined group differences in whole-bra…

Bridging Continuous and Discrete Models of the Anterior Temporal Lobe via Cortical Gradients.

The anterior temporal lobe (ATL) is critical for semantic cognition, yet its organisation remains debated. Some evidence supports a patchwork of discrete, domain-specific parcels, while the graded hub hypothesis proposes smooth, ordered transitions. We examined regions along ATL's curvature, testing whether connectivity profiles varied systematically or discontinuously along whole-brain dimensions of functional connectivity. The structural and functional connectivity of ATL varied largely systematically on the heteromodal-unimodal dimension, with intermediate ventral parcels showing stronger h…

Language network reorganization in temporal lobe epilepsy: insights from task-based functional MRI.

OBJECTIVE: In temporal lobe epilepsy (TLE), recurrent seizures can cause structural and functional disruptions within the temporal lobe and nearby language regions, resulting in neural reorganization. In this study, we leverage task-based functional magnetic resonance imaging (tb-fMRI) to understand how this language reorganization process varies based on the seizure location, the timing of seizure onset, and the chronicity of epileptic activity. METHOD: 84 drug-resistant TLE patients who completed a tb-fMRI sentence completion task were included in this study. Analysis included group compari…

Small sharp spikes precede the development of epileptiform discharges in temporal lobe epilepsy.

Small sharp spikes (SSS), traditionally regarded as benign EEG variants, have increasingly been linked to temporal lobe epileptogenicity. However, their potential role as early EEG biomarkers of temporal lobe epilepsy (TLE) remains unclear. We analyzed 11 patients with or without a prior epilepsy diagnosis whose initial EEGs demonstrated isolated SSS in the absence of interictal epileptiform discharges (IEDs). The cohort included 8 females and 3 males with a median age of 35 years (IQR 26-43). Four patients had a diagnosis of epilepsy at presentation, whereas seven did not. Over a median follo…

Association between disconnected networks and surgical outcome in drug-resistant temporal lobe epilepsy.

PURPOSE: To characterize alterations in structural network connection strength (SNCS), functional network connection strength (FNCS), and structural-functional correlation (SFC) of preoperative disconnected networks in temporal lobe epilepsy (TLE) and the association of these measures with postoperative seizure outcomes. METHODS: This retrospective cohort study enrolled 39 TLE patients with 29 healthy controls (HCs). Patients were divided into seizure-free (SF) (Engel Ia) and non-seizure-free (NSF) (Engel Ib-IV) groups. Each network was partitioned into predefined resection-anchored tiers, in…

Cognitive outcomes following surgical resection in tumoral temporal lobe epilepsy: a scoping review.

BACKGROUND AND AIMS: Surgical strategy in tumoral temporal lobe epilepsy (TLE), remains controversial in the literature given the complex balance between seizure cessation and cognitive function post-operatively. Strategies range from lesionectomy to more extensive resections. This review investigates the effects of temporal lobe tumor resection strategies on cognitive function outcomes. METHODS: A comprehensive literature review was conducted using PubMed, Embase, Cochrane, and Web of Science. Search terms encompassed seizure-related conditions (e.g., epilepsy), surgical approaches (e.g., ag…

Memory performance, hippocampal volumes and memory fMRI in frontal versus temporal lobe epilepsy.

PURPOSE: Patients with temporal lobe epilepsy (TLE) and patients with frontal lobe epilepsy (FLE) show an increased risk of memory impairment relative to healthy controls, alongside structural and functional encoding-related cerebral alterations. To better specify syndrome-related differences, we directly compared TLE and FLE patients. METHODS: We evaluated 56 TLE (25 right-sided) and 24 FLE patients with respect to fMRI activation during encoding of scenes, faces, and words, post-scan recognition accuracy, neuropsychological verbal and figural memory tests, and hippocampal volumes. RESULTS:…

Neural correlates of emotion processing and regulation dissociate frontal and temporal lobe epilepsy.

When confronted with negative events, regulating emotions is essential for self-management and mental health. Temporal lobe epilepsy (TLE) patients, particularly right TLE (rTLE) patients, show altered responses towards emotional stimuli. However, neural correlates of emotion regulation and its interplay with stimulus-based emotion processing have not been studied in TLE so far. Therefore, we compared functional MRI activity during up- and downregulation of emotions towards negative scenes among 21 left TLE and 18 rTLE patients, 18 frontal lobe epilepsy (FLE) patients and 17 healthy controls. …

Causal dependencies between frontal and temporal lobe regions underlying word search and retrieval.

Verbal fluency is a behavioral task that requires the generation of words from a semantic category (category fluency) or words beginning with a specific letter (letter fluency). Although word production engages a frontal-temporal-parietal network, no studies have tested how lesions to temporal and parietal lobe areas that represent semantic and phonological knowledge dampen neural responses in the left pars triangularis and the left pars opercularis, two adjacent regions in the left inferior frontal gyrus implicated in word search and retrieval. Here, 52 patients with temporal lobe lesions und…

Temporal lobe brain abscess: Etiology, microbiology, surgical management, and outcomes - A systematic review.

BACKGROUND: Temporal lobe brain abscess is an uncommon but clinically significant intracranial infection most frequently associated with otogenic disease. Although brain abscess has been widely studied, location-specific analyses focusing on the temporal lobe remain limited. This systematic review aimed to evaluate the etiology, microbiology, surgical management, and outcomes of temporal lobe brain abscess reported in the literature. METHODS: A systematic literature search was performed in PubMed, Scopus, Web of Science, and Google Scholar to identify studies reporting temporal lobe brain abs…

Network changes associated with right anterior temporal lobe atrophy: insight into unique symptoms.

Semantic behavioral variant (also referred to as right temporal) frontotemporal dementia is a newly described syndrome associated with focal right anterior temporal lobe atrophy and a distinctive combination of behavioral and semantic changes. It is considered the right-sided counterpart of semantic variant primary progressive aphasia, with which it has overlapping neuropathological and cognitive mechanisms. Although more is known about how brain network alterations relate to both losses (e.g., word comprehension deficits) and gains (e.g., hyper-fluency) in cognitive and behavioral processes i…

On-scalp magnetoencephalography based on optically pumped magnetometers to investigate temporal lobe epilepsy.

Cryogenic magnetoencephalography (MEG) has a lower yield in temporal lobe epilepsy (TLE) than in extratemporal lobe epilepsy. The advent of optically pumped magnetometers (OPMs) might change this situation thanks to on-scalp MEG. This study aims to investigate the detection/localization accuracy of interictal epileptiform discharges (IEDs) using on-scalp MEG in TLE and the added value of face-OPMs. Eleven patients underwent a 1-h MEG recording with OPMs placed both on the scalp (flexible cap, scalp-OPMs) and on the face (glasseslike structure, face-OPMs). Nine patients underwent cryogenic MEG.…

Self-compassion, mental health, and emotion regulation in adults with temporal lobe epilepsy.

BACKGROUND: People with temporal lobe epilepsy (TLE) are at increased risk of mental health difficulties, most commonly anxiety and depression. Self-compassion, comprised of self-kindness, common humanity, and mindfulness, has been associated with better mental health in wider epilepsy populations. Previous research also suggests that self-compassion may be protective by fostering better emotion regulation. However, this has not been studied in TLE. AIMS: To (1) compare adults with TLE, other types of epilepsy (non-TLE), and without epilepsy (non-epilepsy) on measures of self-compassion, anxi…
