Distal Triceps Tendon Ruptures: A Case Series Highlighting Diagnostic Challenges, Surgical Management, and Functional Outcomes.

Triceps tendon ruptures are uncommon but significant injuries, often presenting diagnostic challenges due to nonspecific clinical features. Delayed or missed diagnoses can result in prolonged functional impairment. We present a retrospective case series of five patients who sustained distal triceps tendon ruptures through various mechanisms, including direct trauma and heavy lifting. Clinical presentations ranged from elbow swelling and pain to palpable tendon gaps, with radiographic "flake signs" consistently observed. Ultrasonography effectively identified tendon ruptures and retraction in f

[Tendon transposition to restore muscle function in the hand].

Nerve injuries in the upper extremity can result in severe disability. In the last three decades, progress in microsurgical techniques has improved the outcome for nerve injuries and if the prognosis is reasonably good, nerve repair should usually be performed prior to tendon transfer procedures. However, above all proximal lesions of peripheral nerves such as high radial nerve palsy still often yield unsatisfactory results, despite a technically well-executed nerve repair. Prognosis further depends on the time interval since the injury and also on the age of the patient, as the regenerative p

Rectus Femoris Quadriceps Muscle Rupture Reconstruction With Semitendinosus Autograft and Achilles Allograft.

Rectus femoris musculotendinous junction injuries occur at the interface between the muscle fibers and the tendon of the rectus femoris muscle. These types of injuries can result from acute trauma or repetitive microtrauma, leading to muscle retraction, fibrosis, muscle fiber degeneration, and chronic inflammation. Management of these injuries typically involves conservative treatment, including rest, ice, compression, and elevation, followed by a structured rehabilitation program. Surgical intervention is required when conservative measures fail and in cases of complete tendon avulsion or chr

[Late outcome of severed extensor tendons in the area of the hand and fingers].

In the care and further treatment of patients with hand injuries, a necessary requirement of the hand surgeon is that he be able to cope with the interlocking factors that are no longer influential, for example, the type and extent of the trauma or the age of the patient; the influential factors are a thought-out care plan, atraumatic action with regard to optimal restoration of the hand. This also showed a retrospective analysis of results and post-examination from flexor tendon injuries in the hand of 298 patients, who were surgically cared for from 1984 to 1994 at the surgical clinic of the…

Complete dislocation of the knee: spectrum of associated soft-tissue injuries depicted by MR imaging.

OBJECTIVE: Complete knee dislocation is a rare injury. The purpose of this study was to evaluate the spectrum of injuries that are depicted by MR imaging in patients with a dislocation of the knee and to determine if there is any predictive factor that might indicate those patients who may be at risk for popliteal nerve injury. MATERIALS AND METHODS: A retrospective search for patients sustaining traumatic knee dislocations who had radiographs and an MR imaging examination of the knee as part of their initial evaluation was done at three level I trauma centers for the period between 1989 and

Multi-modal imaging of the subscapularis muscle.

UNLABELLED: The subscapularis (SSC) muscle is the most powerful of the rotator cuff muscles, and plays an important role in shoulder motion and stabilization. SSC tendon tear is quite uncommon, compared to the supraspinatus (SSP) tendon, and, most of the time, part of a large rupture of the rotator cuff. Various complementary imaging techniques can be used to obtain an accurate diagnosis of SSC tendon lesions, as well as their extension and muscular impact. Pre-operative diagnosis by imaging is a key issue, since a lesion of the SSC tendon impacts on treatment, surgical approach, and post-oper

Treatment of Peripheral Nerve Injuries in Syria's War Victims: Experience from a Northern Israeli Hospital.

BACKGROUND: The collapse of the Syrian healthcare system during the civil war led numerous citizens to cross the Syrian-Israeli border to seek medical care. OBJECTIVES: To describe the epidemiology of peripheral nerve injuries (PNIs) sustained in war, their management, and short-term outcomes. METHODS: A retrospective case series study was conducted on 45 consecutive patients aged 25.7 ± 9.3 years. These patients were referred to the hand surgery unit of the department of orthopedic surgery and traumatology at Galilee Medical Center between December 2014 and June 2018. Median time between in

[Secondary reconstruction of extensor pollicis longus tendon].

BACKGROUND: Secondary reconstruction of the extensor pollicis longus tendon (EPL) can be performed using an intercalated tendon graft (ITG) or a tendon transposition, preferably the extensor indicis transposition (EIT), since a secondary suture of the EPL tendon is not possible in most cases due to degenerative changes or defect injuries. A direct comparison of ITG and EIT in one single patient collective based on a standardized follow-up protocol has not been performed yet. PATIENTS AND METHOD: For the first time, these two operative procedures were directly compared in a retrospective analy

A loop suture for dynamic tendon transfer in the repair of chronic or acute extensor pollicis longus ruptures.

Ruptures of extensor pollicis longus (EPL) tendon may need a tendon transfer for repair, especially in chronic ruptures. We describe a novel transfer technique using a loop-tendon suture for dynamic transfer of extensor pollicis brevis to EPL. We performed an end-to-side transfixing-loop self-locking suture preserving the extensor pollicis brevis, which acts as a motor unit for the distal end of the repaired tendon. Contraindications would mainly be simultaneous rupture of EPL and brevis tendons. No complications were observed, except for mild metacarpal-phalangeal stiffness in chronic rupture

Salvage options for peroneal tendon ruptures.

Reports have demonstrated that peroneal tendon tears can be successfully treated by direct repair or debridement and tubularization, based on the extent and nature of the pathology. Irreparable peroneal tendon tears or completely unsalvageable tendons after failure of previously attempted repairs are rare, and as a result there is a lack of high-level evidence to guide the management of these complex injuries. When irreparable tears are encountered the salvage options include tenodesis, bridging of the defect using allografts or autografts, and tendon transfers. The choice of treatment strateg

Ultrasound of the upper limb: when to use it in athletes.

Ultrasound is a powerful screening examination for athletes with upper limb injuries. It is very effective in demonstrating injuries to muscle, tendon, ligament, and nerves. The technique is particularly useful in excluding joint effusion and in detecting soft tissue foreign bodies. Dynamic ultrasound examination is especially useful in detecting subluxation of tendon and nerves; it may be the only means of demonstrating the cause of snapping. Detailed anatomical knowledge is mandatory, and the examination should be structured and systematic. Ultrasound may overlook disorders of the deep struc

Bilateral Tibial Tubercle Avulsion Fractures With an Associated Patellar Tendon Avulsion in an Adolescent Patient.

Tibial tubercle fractures in pediatric patients are increasing in frequency as more children participate in sports. These injuries are often seen in boys engaging in jumping activities before closure of their proximal tibial physis. Bilateral tibial tubercle fractures have been reported in the literature, but less frequent are associated patellar tendon ruptures with fracture of the tubercle. In this case report, we present an 11-year-old girl who sustained bilateral tibial tubercle fractures, including an associated patellar tendon rupture from the tubercle on the right lower extremity. We de

Coracoid impingement syndrome due to intensive rock climbing training.

Overuse and acute injuries to the upper body are common in rock climbing. Such injuries primarily affect the fingers; but shoulder problems are increasingly common, especially among more experienced and older climbers who climb at a high ability level. Such shoulder problems are often due to subacromial impingement, shoulder dislocations with bankart lesions, hyperlaxity, SLAP lesions or irritations of the long biceps tendon. In contrast to these known conditions, we describe a case of an ambitious female rock climber who trained intensively and developed a coracoid impingement caused by hyper

Epidemiology and outcomes of Achilles tendon ruptures in the National Football League.

The purpose of this study is to document the epidemiology of Achilles tendon ruptures in the National Football League (NFL) and to quantify the impact of these injuries on player performance. A retrospective review of several online NFL player registries identified 31 Achilles tendon ruptures in NFL players between 1997 and 2002. Nineteen percent of injuries occurred during preseason play, while another 18% occurred during the first month of the official season. There was a postinjury reduction of 88%, 83%, and 78% in power ratings for wide receivers, running backs, and tight ends, respectivel

Late reconstruction of sagittal band laceration.

Injuries to the sagittal band with subsequent instability of the extensor digitorum communis tendon are uncommon. In an unusual presentation, a laceration to the radial sagittal band of the little finger was manifest as an abduction deformity of the digit. This injury was treated by repair of the sagittal band and extensor digiti minimi tendon transfer.

Accuracy of High-Resolution Ultrasonography in the Detection of Extensor Tendon Lacerations.

BACKGROUND: Lacerations to the extensor mechanism are usually diagnosed clinically. Ultrasound (US) has been a growing diagnostic tool for tendon injuries since the 1990s. To date, there has been no publication establishing the accuracy and reliability of US in the evaluation of extensor mechanism lacerations in the hand. The purpose of this study is to determine the accuracy of US to detect extensor tendon injuries in the hand. METHODS: Sixteen fingers and 4 thumbs in 4 fresh-frozen and thawed cadaveric hands were used. Sixty-eight 0.5-cm transverse skin lacerations were created. Twenty-seve…

Early passive mobilization after digital nerve repair and grafting in a fresh cadaver.

In the clinical management of combined tendon and nerve injuries, there are competing treatment strategies. Isolated tendon injuries should be rapidly mobilized after repair to prevent adhesion formation, whereas isolated nerve repairs are usually immobilized to prevent disruption and to allow axon regrowth. Recommendations in the published literature for the management of combined tendon and nerve injuries are vague and advise up to 3 weeks of immobilization. The goals of this study were to determine which length of nerve gap resulted in rupture of a repair following postoperative mobilizatio…

Zone-II flexor tendon repair: a randomized prospective trial of active place-and-hold therapy compared with passive motion therapy.

BACKGROUND: In order to improve digit motion after zone-II flexor tendon repair, rehabilitation programs have promoted either passive motion or active motion therapy. To our knowledge, no prospective randomized trial has compared the two techniques. Our objective was to compare the results of patients treated with an active therapy program and those treated with a passive motion protocol following zone-II flexor tendon repair. METHODS: Between January 1996 and December 2002, 103 patients (119 digits) with zone-II flexor tendon repairs were randomized to either early active motion with place a
