Management of Acute Extensor Tendon Injuries.

Thin soft tissue covering extensor tendons make them prone to injury. The extensor mechanism achieves a delicate balance with the flexor system. Inappropriate management in the acute setting can lead to long-term deformity and dysfunction. Acute extensor tendon injuries are usually managed with splinting and/or primary repair of the tendon. In cases of tendon length loss, tendon graft or flap may be necessary for reconstruction. This article presents a series of cases illustrating the appropriate management of traumatic extensor tendon injuries.

Acute Calcific Tendonitis of the Longus Colli.

BACKGROUND: Acute calcific tendonitis of the longus colli (ACTLC) is an aseptic inflammatory response to deposition of calcium in the longus colli muscle tendons. Although reports in the literature are scarce, ACTLC likely represents an underreported disease process that routinely goes misdiagnosed. We report a case of ACTLC and provide a brief review of the literature. CASE REPORT: A 45-year-old otherwise healthy female presented with a 3-day history of progressive neck pain, decreased neck mobility, and odynophagia. She exhibited tenderness over the right posterolateral neck with limited ne…

Achilles Tendon Rupture in Elite Athletes.

The management of acute Achilles tendon rupture in elite athletes is a current area of clinical controversy. Recent studies have reported near-equivocal outcomes in patients who undergo either nonsurgical or surgical treatment of Achilles tendon rupture; however, similar functional outcomes may not be observed in elite athletes who are at the highest levels of athletic performance and undergo nonsurgical or surgical treatment of Achilles tendon rupture. Surgeons should understand the risks and benefits of nonsurgical and surgical management of acute Achilles tendon rupture. Surgeons also shoul

Distal Biceps Tendon Anatomic Repair.

Distal biceps injuries, which usually occur in active middle-aged men, can result in chronic pain and loss of supination and flexion strength3,4. Surgical repair of a ruptured distal biceps tendon can reliably decrease pain and improve strength compared with nonoperative management3,4. However, even following successful healing and rehabilitation of a surgically repaired biceps tendon, full supination strength is rarely restored5-7. The expected outcome following distal biceps repair using a traditional anterior approach is a measurable loss of rotational strength, especially from neutral to s

[Acute rupture of the tibialis anterior tendon].

UNLABELLED: Tibialis anterior tendon rupture is a relatively rare injury, with diverse etiologies, that can cause significant deficits in the functionality of the ankle. These injuries are sometimes diagnosed too late, being in many occasions the alteration in walking the symptom that makes the patient go to the doctor. The six weeks will mark the limit between acute and chronic injury, requiring different therapeutic approaches, along with other parameters such as age, functional demand or concomitant pathologies. OBJECTIVE: To present two cases of acute rupture, with different etiology, the

Ultrasound-Guided Percutaneous Tendon Treatments.

OBJECTIVE: The purpose of this article is to review currently available tendon treatments, especially those performed with sonographic guidance. CONCLUSION: Treatments of tendon disease have continued to develop and expand, and multiple therapeutic options have become available, all with varying levels of supportive clinical evidence of their efficacy. The use of ultrasound to direct these treatments improves accuracy and performance by facilitating visualization of the target and relevant adjacent structures.

Synaptic failure: The achilles tendon of sphingolipidoses.

The presence of life-threatening neurological symptoms in more than two-thirds of lysosomal storage diseases (LSDs) underscores how vulnerable the nervous system is to lysosomal failure. Neurological dysfunction in LSDs has historically been attributed to the disruption of neuronal and glial homeostasis resulting from the progressive jamming of the endosomal/lysosomal pathway. In neurons, a dysfunctional endosomal-lysosomal system can elicit dire consequences. Given that neurons are largely postmitotic after birth, one can clearly understand that the inability of these cells to proliferate obl

Tendon Transfers in the Rheumatoid Hand for Reconstruction.

Long-standing rheumatoid arthritis can result in spontaneous tendon rupture caused by attrition of the tendons. Ruptures of the ulnar-sided extensor tendons, flexor pollicis longus, and the flexor digitorum profundus can be seen. Primary repair of these tendon ruptures is frequently not possible because of delayed presentation and tendon damage by the disease process. Tendon transfers are the preferred method of treatment in patients with rheumatoid arthritis. At surgery, it is critical to address the underlying cause of rupture to prevent future tendon ruptures. Rates of tendon rupture may de…

Tendon Injuries of the Hand in Kirikkale, Turkey.

BACKGROUND: Tendon injuries are one of the most common injuries of the hand and challenging problems in trauma surgery. They usually require surgical repair and unlike the single tendon injuries, flexor tendon injuries have higher morbidities when accompanied with nerve injuries. In the present study, I aimed to report the clinical experiences and outcomes about tendon injuries. METHODS: A total of 180 patients (149 males, 31 females) between 17 and 56 years old were operated in the clinic due to tendon injury. Seventy isolated extensor tendon injuries, 60 isolated flexor tendon injuries, 30

Triceps Tendon Ruptures: A Systematic Review.

BACKGROUND: Triceps tendon ruptures (TTR) are an uncommon injury. The aim of this systematic review was to classify diagnostic signs, report outcomes and rerupture rates, and identify potential predisposing risk factors in all reported cases of surgical treated TTR. METHODS: A literature search collecting surgical treated cases of TTR was performed, identifying 175 articles, 40 of which met inclusion criteria, accounting for 262 patients. Data were pooled and analyzed focusing on medical comorbidities, presence of a fleck fracture on the preoperative lateral elbow x-ray film (Dunn-Kusnezov Si

Rupture of the triceps tendon - A case series.

Triceps rupture is the least common among all tendon injuries. The usual mechanism of injury is a fall on an outstretched hand, although direct contact injuries have also been reported to cause this injury. The diagnosis of acute triceps tendon rupture may be missed, which can result in prolonged disability and delayed operative management. We presented three cases of acute triceps tendon rupture each at different site showing the spectrum of injury to the muscle and mechanism of injury and management were also discussed.

Is there still a place for Achilles tendon lengthening?

Patients with diabetes and ankle equinus are at particularly high risk for forefoot ulceration because of the development of high forefoot pressures. Stiffness in the triceps surae muscles and tendons are thought to be largely responsible for equinus in patients with diabetes and underpins the surgical rationale for Achilles tendon lengthening (ATL) procedures to alleviate this deformity and reduce ulcer risk. The established/traditional surgical approach is the triple hemisection along the length of the Achilles tendon. Although the percutaneous approach has been successful in achieving incre…

Madelung Deformity and Extensor Tendon Rupture.

Extensor tendon rupture in chronic Madelung deformity, as a result of tendon attrition on the dislocated distal ulna, is a rare occurrence. It is, however, seen more often in rheumatoid arthritis. There are few case reports in the English-language literature on this issue. We report a case of multiple tendon ruptures in a previously undiagnosed Madelung deformity.

Fibroma of tendon sheath with 11q rearrangements.

Fibroma of tendon sheath is an uncommon, benign fibroblastic tumor that usually occurs in the upper extremities of young and middle-aged adults. A clonal chromosomal aberration, t(2;11)(q31-32;q12), has been described in one case. We herein present a unique cytogenetic finding of fibroma of tendon sheath arising in the first web space of the right hand of a 38-year-old woman. Physical examination showed a 3.5-cm, firm, mobile, non-tender mass. Magnetic resonance imaging showed a well-defined soft tissue mass with iso- to slightly-low signal intensity relative to skeletal muscle on both T1- and

Tendon rupture as an adverse effect of rasagiline.

A 66-year-old woman with advanced Parkinson disease (PD) was referred to our center for an adjustment of her antiparkinsonian medication. To reduce daily off-time, we introduced rasagiline 1 mg/d. Three days after starting this new treatment, she presented with intense arthralgia that symmetrically affected the shoulders, hands, and hips without myalgia. Ten days later, while walking, she experienced acute pain on the inner side of her right thigh, with the absence of any trauma. Findings of ultrasonography confirming the diagnosis of partial avulsion of the right harmstrings. Rasagiline was s
