Assessment of Pulp Chamber Morphology of Primary Maxillary and Mandibular Molars Using Spiral Computed Tomography: An Analytical Study.

BACKGROUND: The basic anatomy of primary teeth gives an idea of an ideal access cavity with minimal loss of tooth structure. Central groove, cemento enamel junction (CEJ), roof and floor of the pulp chamber, and furcation area are significant landmarks in tooth morphology. In primary teeth, the pulp occupies a higher volume of the crown than permanent teeth do, and with progressing caries, the comparatively thin layer of hard mineralized tissue between the inner and outer surfaces causes the tooth pulp to become involved very quickly. The thickness of mineralized tissue surrounding the pulp va

Endodontic Retreatment Using Rotary System and Aesthetic Rehabilitation of a Deciduous Central Maxillary Incisor in a 5-Year-Old Child: A Case Report.

INTRODUCTION: Early childhood caries (ECC) affects the primary dentition of preschool children and can result in the need for pulp treatment. Endodontic retreatment in deciduous teeth is indicated when initial treatment fails, representing a conservative alternative to extraction. OBJECTIVE: This case describes pulp retreatment using a rotary system and complete aesthetic rehabilitation of the anterosuperior sector in a pediatric patient. CASE REPORT: A 5-year-old systemically healthy male patient attended the clinic with ECC affecting the maxillary central and lateral incisors. Clinical exa

Is there bacterial infection in the intact crowns of teeth with pulp necrosis of sickle cell anaemia patients? A case series study nested in a cohort.

AIM: To evaluate the presence of bacteria in permanent teeth with intact crowns (without caries, periodontal disease or dental trauma) in patients with sickle cell anaemia (HbSS genotype) by analysing their clinical, imaging and microbiological parameters. METHODOLOGY: This is a case series study nested in a cohort. In the first follow-up of this cohort study (Journal of Endodontics, 2013, 39, 177), 10 HbSS patients with at least one tooth with an intact crown and clinically diagnosed with pulp necrosis by pulse oximetry adapted for dentistry and a cold pulp sensitivity test (n = 27 teeth) we

Enhanced conservation of vital pulp and apical tissues by the application of crown rotation surgery for inversely impacted central incisors: A follow-up analysis of two patients over 4 years.

AIM: To describe a novel surgical method (crown rotation surgery) to manage inversely impacted central incisors with immature roots. METHODOLOGY: Two young patients each presented with an inversely impacted maxillary central incisor. To protect the apical tissues, the two impacted incisors were rotated downwards to a relatively normal position without extraction from their bony sockets. RESULTS: After crown rotation surgery, spontaneous eruption, continuous root development, and periodontal healing of the rotated incisors were observed. The pulp retained vitality and blood flow was normal. M

A systematic review of the complications of high-risk third molar removal and coronectomy: development of a decision tree model and preliminary health economic analysis to assist in treatment planning.

Coronectomy is an alternative surgical technique for the management of high-risk third molars. It involves the removal of the crown of a tooth and the deliberate retention of the roots, thereby avoiding injury to the inferior alveolar nerve (IAN). Previous studies have suggested that it reduces the risk of nerve injury when compared with surgical extraction. The purpose of this study was to systematically review the incidence of complications following coronectomy such as IAN injury, pain, dry socket, infection, root migration, and need for re-operation. A comparative cost analysis of coronect

Somatosensory changes in Chinese patients after coronectomy vs. total extraction of mandibular third molar: a prospective study.

OBJECTIVES: This study aimed to quantitatively compare the somatosensory function changes of inferior alveolar nerve (IAN) after mandibular third molar extraction with a surgery protocol of coronectomy, as opposed to the conventional method. MATERIALS AND METHODS: Patients with a lower third molar directly contacting IAN were recruited and assigned either to a test group (coronectomy group) or a control group (conventional extraction). A standardized quantitative sensory testing (QST) battery was performed for four times: one week before surgery and the second, seventh, and 28th days after su

The effect of fiber post location on fracture resistance of endodontically treated maxillary premolars.

BACKGROUND: There is no sufficient literature on the effect of post location on endodontically treated premolar teeth with 2 roots. OBJECTIVES: The aim of the study was to evaluate the effect of fiber post location on fracture resistance and failure mode of endodontically treated premolars with 2 roots. MATERIAL AND METHODS: Fifty extracted maxillary first premolars with 2 roots were divided randomly into 5 groups. Group 1 was comprised of sound teeth, which received only metal crowns (control). Teeth from groups 2, 3, 4, and 5 were decoronated 2 mm above the cementoenamel junction (CEJ) and

Can crown-root ratio predict premolar eruption?

INTRODUCTION: The purpose of this study was to establish a method to prospectively predict the eruption of mandibular second premolars with panoramic radiographs. This technique could be used clinically to optimize the timing of comprehensive treatment, detect abnormalities of development and eruption, or effectively estimate eruption timing and determine the need for early intervention because of caries or defects of the deciduous dentition. METHODS: Three hundred one white children from an orthodontic practice in Alaska between the ages of 6 and 16 years were examined and monitored with ser

Forced eruption in the treatment of transverse root fractures.

A relatively new method for treating transverse root fractures near the gingival crevice is reviewed. Two cases are described in which the fractured teeth are orthodontically erupted to facilitate restoration. A periodontal pocket was reduced in the first case, and the technique was modified to accommodate a patient wearing a partial denture in the second case. The technique appears to be a favorable alternative to extraction or periodontal surgery.

Invasive cervical resorption: etiology, diagnosis, classification and treatment.

BACKGROUND: Invasive cervical resorption (ICR) is not well understood by the professional, being misdiagnosed, leading to inappropriate treatment and unnecessary loss of tooth. INTRODUCTION: ICR is defined as a localized process of resorption, which begins in the cervical area of the tooth, just below the epithelial junction and above the ridge crest in the area of the connective tissue insertion. Possible predisposing factors include external trauma, orthodontic movement, surgical procedures, periodontal disease and its treatments, endogenous bleaching, pressure generated by wind instruments

The internal fit and fracture resistance of stainless-steel crowns and porcelain fused to metal crowns in intact and severely damaged permanent molars: an in vitro study.

AIM: Severe caries in mixed dentition frequently affects mandibular first permanent molars, often necessitating full-coverage restorations. Stainless steel crowns (SSCs) and porcelain-fused-to-metal (PFM) crowns are among the most commonly used options. This study aimed to compare the internal fit and fracture resistance of SSCs and PFMs in both intact and restored mandibular first molars. MATERIALS AND METHODS: Forty extracted mandibular first molars were randomly divided into four groups: (1) intact teeth-SSCs, (2) intact teeth-PFMs, (3) restored teeth-SSCs, and (4) restored teeth-PFMs. In

Dentigerous cyst associated with ectopic canine and a supernumerary tooth: a rare occurrence.

Amongst the cysts of the jaw dentigerous cyst (DC) is one of the most prevalent types of odontogenic cysts, which is associated with the crown of an unerupted or developing tooth. DC is more commonly seen with mandibular third molar and maxillary canine and rarely other teeth are involved. These cysts seldom associate with supernumerary teeth. The purpose of this article is to describe a case of large dentigerous cyst associated with supernumerary teeth and an ectopic canine, which is a rare presentation along with its management.

Histological and developmental insights into the herbivorous dentition of tapinocephalid therapsids.

Tapinocephalids were one of the earliest therapsid clades to evolve herbivory. In acquiring derived tooth-to-tooth occlusion by means of an exaggerated heel and talon crown morphology, members of this family have long been considered herbivorous, yet little work has been done to describe their dentition. Given the early occurrence of this clade and their acquisition of a dentition with several derived features, tapinocephalids serve as an important clade in understanding adaptations to herbivory as well as macroevolutionary patterns of dental trait acquisition. Here we describe the histology o

[Incisors with a mind of their own].

A 43-year-old man, known to have hypodontia, was bothered by his over-erupted central incisors. After the placement of implants and the accompanying crown- and bridgework, the central incisors appear to be increasingly over-erupted. The history shows that this phenomenon has already been seen in the past. Three mechanisms can be responsible for the over-eruption, specifically: continuous facial growth, over-eruption of both central incisors in the maxilla as a consequence of agenesis of opposing dentition and relapse of the orthodontic treatment as a result of the fact that orthodontic retenti
