Twenty-three patients with trypanosomiasis in the meningo-encephalitic period have been examined neurologically. Main neurological symptoms are evocative of the affection and consist in frontal syndrome, often shown by a cheiro-chin reflex, the most frequent objective neurological sign, extrapyramidal injury, neuro-endocrine dysfunction, vestibular, cerebellar, pyramidal and meningitis syndromes. Cerebro-spinal alterations are not steadfast.

We describe one case of reactive encephalopathy as a complication during treatment of human trypanosomiasis (T.b. gambiense) with melarsoprol (Arsobal, Mel B.). A 37 year white man who had typical T. gambiense sleeping sickness and secondary convulsions, coma and hemiplegia after three days of treatment by melarsoprol. Within 48 hours of intensive care the symptoms and signs released. A survey carried out on 101 patients has shown that the treatment of human sleeping sickness, theoretically free, is relatively costly. The average total cost amount to 58,000 F C.F.A., ranging from free treatment up to much as 110,000 F C.F.A. This high cost in part explains the cases where treatment is refused, something which has frequently been observed in the Congo. It would therefore be desirable for patients, who are significant epidemiological reservoirs of T. b. gambiense, to receive economic aid.

The results are described of a study of 60 patients with sleeping sickness from north-east Zambia together with 60 hospital controls and 27 nearest-neighbour controls. Eight symptoms were significantly commoner among sleeping-sickness patients than among either set of controls, and some of these symptoms were used to devise a scoring system for use by rural medical personnel. The equipment of a laboratory making indirect antibody-fluorescent tests on dried blood blots in the heart of the trypanosomiasis focus of Bouaflé (Ivory Coast) gave good results. Changes in the distribution of T, B and null cells in the spleen and bone marrow have been studied in inbred mice infected with Trypanosoma brucei. Immunofluorescent staining combined with a 3H-thymidine pulse and autoradiography showed activation of all three cell types. A transient increase in splenic T cells was followed by dramatic increases in B cells (2·5-fold) and in null cells (35-fold). By day 12, after the first peak of parasitaemia, nearly half the spleen cells were dividing. In the bone marrow, very large blast cells appeared within a week of infection.
