Burkitt lymphoma associated with human immunodeficiency virus infection and pulmonary tuberculosis: A case report.

INTRODUCTION: The association of human immunodeficiency virus (HIV) infection with Burkitt lymphoma is related to the presence of Epstein Barr virus infection and the impact of the HIV antigen on the expansion of B-polyclonal cells. In Southeast Europe, the association is rare, and recognizing this is important in the therapeutic decision to increase patient survival rate. The association of HIV with Burkitt lymphoma and tuberculosis is even more rarely described in the literature. PATIENT CONCERNS: We present the case of a 40-year-old patient who presented with a 3-week history of fever (max

Imaging of musculoskeletal tuberculosis.

Tuberculosis (TB) represents a major public health problem worldwide. Any tissue may be infected. Involvement of the musculoskeletal (MSK) system account for 1-3% of all tuberculous infections. MSK TB may manifest as tuberculous spondylitis, arthritis, osteomyelitis, and soft tissue infections. Although TB spondylitis may present with distinctive imaging features compared to pyogenic infections of the spine, the imaging semiology of extra-spinal TB infections is mostly nonspecific and may mimic other lesions. TB infections should therefore always be considered in the differential diagnosis, pa

Paradoxical Reaction in a Patient with Co-Occurring Tuberculous Meningitis and Pott's Disease.

BACKGROUND Paradoxical reactions to tuberculosis (TB) are clinical or radiological worsening of prior tuberculous lesions or the development of new lesions upon treatment with appropriate anti-tuberculosis therapy (ATT). This phenomenon has been described in both HIV-seropositive and HIV-seronegative patients. Although historically estimated to occur in 6-30% of HIV-seronegative patients with TB, the phenomenon is often under-recognized in the current era, particularly in countries of low TB prevalence. We describe a case of a TB paradoxical reaction affecting the CNS and spine in an HIV-seron

Surgery for Central Nervous System Tuberculosis in Children.

Tuberculosis (TB) is the second most common cause of death due to a single infectious agent worldwide after COVID-19. Central nervous system tuberculosis is widely prevalent in the world, especially in the developing countries and continues to be a socioeconomic problem. It is highly devastating form of tuberculosis leading to unacceptable levels of morbidity and mortality despite appropriate antitubercular therapy. The clinical symptoms are varied and nonspecific. They can be easily overlooked. Tuberculous meningitis is the most common presentation and its sequelae viz. vasculitis, infarction

A Rare Case of Concurrent Intramedullary and Brain Tuberculoma: Diagnostic Challenges and Management.

Intramedullary spinal tuberculomas constitute a small percentage of spinal tuberculosis. These, in combination with brain tuberculomas, are an uncommon manifestation of central nervous system (CNS) tuberculosis. This report details a unique case of a 32-year-old retroviral disease-positive male who presented with a two-month history of symmetrical quadriparesis and recent seizures. The neurological assessment revealed significant weakness, with a power of 3/5, associated with stiffness, hypoesthesia below the neck, and urinary hesitancy. He was diagnosed via magnetic resonance imaging (MRI), w

Neurological complications of human immunodeficiency virus infection in Zimbabwe-2005.

Zimbabwe is a high prevalence area for HIV infection, and provides opportunities for studying complications of AIDS in both antiretroviral naive and treated groups of patients. Figures for HIV prevalence are among the highest in the world, but the number of people receiving treatment is very small. Economic and political factors contribute to this important health crisis. Opportunistic diseases as a whole and their relative frequencies of presentation in southern Africa are discussed, emphasizing the lead role of tuberculosis. The limited data that do exist regarding neurological complications

Hodgkin disease with spinal cord compression.

Hodgkin disease is a nodal disease. Spinal cord or root compression is a rare complication and usually seen in the setting of progressive, advanced disease. We report 2 cases of Hodgkin disease in pediatric patients who presented with neurologic signs. One patient had paravertebral masses and involvement of thoracic vertebrae, which was initially misdiagnosed as spinal tuberculosis. The second patient who presented with paraplegia and bladder and bowel involvement had an epidural mass with collapse of thoracic vertebra. Lymph node biopsy revealed Hodgkin disease, mixed cellularity in both the

Multiple cystic tuberculous lesions in the skeleton (Jüngling disease): a case report.

STUDY DESIGN: A case is reported. OBJECTIVES: To report and discuss a case of multiple cystic tuberculous skeletal lesions. SUMMARY OF BACKGROUND DATA: Multiple cystic tuberculosis lesions of the skeleton, or Jüngling disease, is a rare variety of tuberculosis reported extensively 30 to 40 years ago. No such severe type of tuberculosis has been reported in recent years. The patient was a 43-year-old woman with a history of steroid treatment, whose cervical lesions caused C2 and C3 destruction and spinal cord impingement. METHODS: Medical history, laboratory data, radiographs, bone scan, and

Concurrent Multilevel Spinal Intra-medullary with Extensive Intracranial Tuberculomas: A Rare Case Report.

Disseminated tuberculomas in the brain and spinal cord are rare. To the best of our knowledge, only nine cases of spinal intra-medullary tuberculomas with cranial involvement have been reported till date. However, involvement of all levels in the spinal cord, brain stem with pan lobar involvement of the cerebrum and cerebellum has not been reported so far. We present such a case of a 12-year-old boy with history of pulmonary tuberculosis, who presented with gradual onset of quadriparesis and generalized seizures. We have discussed the unusual clinical presentation and the temporal changes in m

Pregabalin-induced first degree atrioventricular block in a young patient treated for pain from extrapulmonary tuberculosis.

Pregabalin, widely used in the treatment of several pain disorders, is usually well tolerated. Uncommonly, the drug may induce cardiac side effects, rarely prolongation of the PR interval. The latter has never been described in patients with healthy heart or normal renal function. We characterize a unique case of a young man with extrapulmonary tuberculosis and no detectable or known cardiac or kidney diseases, treated with pregabalin to control the severe pain due to the involvement of the spinal cord by the tuberculosis, showing an atrioventricular (AV) block due to pregabalin administration

[A case of miliary tuberculosis and chest wall cold tuberculous abscess with contents draining into the epidural space].

A 37-year-old man was admitted because of miliary tuberculosis. However, his fever and general condition did not improve with chemotherapy. Six months later, paraplegia occurred following sudden radicular back pain without any alteration of segmental sensation. There were no abnormal findings on X-ray films of the thoracic and lumbar vertebrae. Examination by magnetic resonance imaging revealed that the contents of a cold tuberculous abscess in the right chest wall had drained into the epidural space through the intervertebral foramen. Drainage of the chest wall abscess and laminectomy were pe

When and how to operate on spondylodiscitis: a report of 13 patients.

PURPOSE: Conflicting reports exist regarding the surgical indications, timing, approach, staged or not operation, and spinal instrumentation for patients with spondylodiscitis. Therefore, we performed this study to evaluate the outcome of a series of patients with spondylodiscitis aiming to answer when and how to operate on these patients. MATERIALS AND METHODS: We retrospectively studied the files of 153 patients with spondylodiscitis treated at our institution from 2002 to 2012. The approach included MR imaging of the infected spine, isolation of the pathogen with blood cultures and/or biop

Spinal infection--an overview and the results of treatment.

A retrospective review of thirty-eight patients (16 males and 22 females) with spinal infection between 1993 and 1998 revealed that the mean age was 39.9 years and the peak incidence was in the 5th decade of life. Infections in thirty-two patients (84.2%) were tuberculous in origin, 13.2% were pyogenic and 2.6% were fungal. Back pain was a symptom in 94.7% while 55.8% had neurological deficits, of which two-thirds were tuberculous in origin. Twenty-two patients (57.9%) had an impaired immune status secondary to pulmonary either tuberculosis, diabetes mellitus, intravenous drug abuse, prolonged

An unusual aspect of Pott's disease: small bowel obstruction from a tuberculous psoas abscess.

Pott's disease typically presents with back pain and systemic symptoms, but atypical presentations can obscure diagnosis. We describe a rare manifestation where the initial presentation was acute small bowel obstruction. A 20-year-old female presented with acute abdominal pain, distension, and vomiting, with a 3-month history of intermittent lower back pain. Computed tomography scan revealed distal ileal obstruction due to wall thickening adjacent to a left psoas abscess and a large paravertebral abscess. Aspirated pus confirmed Mycobacterium tuberculosis. The diagnosis was small bowel obstruc

Granulomatous infective spondylitis in a patient presenting with progressive difficulty in walking: the differential between tuberculosis and brucellosis.

We report a case of infectious spondylitis in a 52-year-old woman who presented with progressive difficulty in walking. The patient had a 2-month long history of neurological symptoms, which progressed rapidly to paraplegia, following her admission. Imaging studies demonstrated the presence of vertebral lesions as well as additional tissue with inflammatory elements in the spinal canal, which caused a mass effect. In combination with the presence of increased cells and protein in the cerebrospinal fluid (CSF), the differential was steered towards causes of infectious spondylitis, primarily tub
