Intermittent catheters.

Intermittent catheterisation can improve patients' quality of life and promote independence. Nurses need to be aware of the products available, to ensure patients receive appropriate equipment. Nurse prescribing may soon confirm the nurse's key role in selecting these products.

Characterization of antimicrobial resistance among Proteus mirabilis isolates from catheter-associated urinary tract infections and non-catheter-associated urinary tract infections in Egypt.

BACKGROUND: Recent worldwide reports of increased numbers of multidrug-resistant (MDR) Proteus mirabilis (P. mirabilis) isolates, particularly those producing extended-spectrum β-lactamases (ESBLs), are alarming. P. mirabilis is a common causative agent of complicated urinary tract infections (UTIs), particularly in patients with long-term urinary catheterization. This study aimed to assess the prevalence, antibiotic resistance patterns, and determinants of P. mirabilis among catheter-associated UTIs (CAUTI) and non-catheter-associated UTIs (non-CAUTI). METHODS: One hundred and three Proteus …

Pediatric Primary Alveolar Soft Part Sarcoma of the Bladder.

A 9-year-old girl was diagnosed with primary alveolar soft part sarcoma of the bladder after imaging examinations and transurethral resection (TUR) of the bladder tumor. As a positive surgical margin of the TUR indicated residual tumor cells, we performed a cystourethrectomy to remove the tumor. A continent urinary reservoir for self-catheterization was constructed using the Mainz pouch technique, and an abdominal (umbilical) continent catheterizable stoma using the appendix was performed. For 2.5 years postoperatively, the patient remained free of local recurrence and distant metastasis. The

Continuous urinary incontinence presenting as the initial symptoms demonstrating acontractile detrusor and intrinsic sphincter deficiency in multiple system atrophy.

Severe stress incontinence due to intrinsic sphincter deficiency may be common in aged women. However stress urinary incontinence could be the initial symptom of severe neurogenic bladder even if no neurological signs have previously been recognized. Urodynamic study is necessary in such a case. Tension-free vaginal tape surgery and intermittent self-catheterization may be one option, but self catheterization might become difficult when the neurological disorders develop. In this case report, we highlight the need for caution in identifying the presence of an occult neurogenic bladder when int

Urethrospongiosal fistulas: clinical and therapeutic considerations.

From 1986 to 1990, 8 cases of urethrospongiosal fistulas were observed. All the patients had a history of bleeding after a difficult catheterization. The symptoms were not pathognomonic but the fistulas were visible only by X-ray examination. The authors suggest that urethrospongiosal fistulas are more common than one expects, especially in cases of bleeding after complicated catheterization. This is more frequent when coexistent urethral strictures or prostatic hypertrophy make the maneuver difficult. The authors also suggest that the urinary extravasation in the corpus spongiosum could expla

Community-acquired Xanthomonas maltophilia pyelonephritis.

We have described the case of a patient with a community-acquired urinary tract infection with what is more typically a nosocomial pathogen. Our patient had not been hospitalized recently, and his only established risk factor for X maltophilia infection was chronic bladder catheterization. Although paraplegic and thus disabled, he was not debilitated. He had had no recent antibiotic therapy prior to the onset of symptoms, but he did have a history of multiple previous bladder catheter-related infections. We believe that chronic outpatient bladder catheterization, with its risk of recurrent inf

Antimicrobial resistance and associated risk factors in urinary tract infections diagnosed in the emergency department: a retrospective observational study.

BACKGROUND: Urinary tract infections (UTIs) are among the most common causes of emergency department admissions and represent a major source … ected in 16.9% and 17.9% of Gram-negative isolates, respectively. Recent antibiotic use, indwelling urinary catheterization, and malignancy were identified as independent risk factors for ESBL positivity. Male sex, malignancy, and indwelling urinary catheterization were independently associated with carbapenem resistance. Among Enterococcus spp. isolates, vancomycin resi …

Botulinum toxin in spinal cord injury patients with neurogenic detrusor overactivity.

Evidence for the efficacy and safety of intravesical onabotulinum toxin A (onabotA) injections has led to them being licensed in many countries, including Korea, for the treatment of patients with urinary incontinence due to neurogenic detrusor overactivity (NDO) resulting from spinal cord injury or multiple sclerosis who are refractory or intolerant to anticholinergic medications. OnabotA injections have an inhibitory effect on acetylcholine release for up to 10 months, with a recommended dose of 200 U. OnabotA treatment has a beneficial effect not only on urinary symptoms, but also on qualit

Retained intravesical catheters in a patient with spinal cord injury performing clean intermittent self-catheterization: A case report.

CONTEXT: Clean intermittent self-catheterization (CISC) is a first-line bladder management strategy for patients with spinal cord injury (SCI). However, complications such as urinary tract infections and stone formation are relatively common. The retention of the catheter as an intravesical foreign body is an exceedingly rare event and can present with symptoms similar to infection or radiolucent calculi. FINDINGS: A 36-year-old male with a history of chronic spinal cord injury (T12, AIS A) utilizing clean intermittent self-catheterization (CISC) presented with urinary frequency and the prese

Age and Urinary Retention in Women After Botox Treatment for Overactive Bladder.

IMPORTANCE: Urinary retention can occur after intradetrusor onabotulinumtoxinA administration. There are inconsistent data regarding whether older adults are at an increased risk of urinary retention following treatment. OBJECTIVE: The aim of this study was to investigate the relationship between age and the development of urinary retention following intradetrusor onabotulinumtoxinA administration. STUDY DESIGN: This was a retrospective cohort study of women initiating overactive bladder treatment with intradetrusor onabotulinumtoxinA from 2017 to 2022. The exposure of primary interest was a

The influence of antibiotic prophylaxis on bacterial resistance in urinary tract infections in children with spina bifida.

BACKGROUND: Bacterial resistance to antibiotics is an increasingly threatening consequence of antimicrobial exposure for many decades now. In urinary tract infections (UTIs), antibiotic prophylaxis (AP) increases bacterial resistance. We studied the resistance patterns of positive urinary cultures in spina bifida children on clean intermittent catheterization, both continuing and stopping AP. METHODS: In a cohort of 176 spina bifida patients, 88 continued and 88 stopped using AP. During 18 months, a fortnightly catheterized urine sample for bacterial pathogens was cultured. UTIs and significa
