Benign Hydronephrosis and Elevated of Serum Levels of Carbohydrate Antigen CA 19-9: A Case Report.

BACKGROUND: Carbohydrate tumor-associated antigen (CA 19-9) has been shown to be upregulated in other malignant tumors including gastric, ov … LUSIONS: There is no doubt that CA 19-9 level is occasionally elevated in patients with obstructive urolithiasis as it was in our case. In the routine medical praxis, urolithiasis should not be neglected in the differential diagnosis of elevated concentrations of CA 19-9 marker.

Medical management of male and female cats with nonobstructive lower urinary tract disease.

This case is typical of recurrent urolithiasis managed by repeated surgery. Retrospective assessment of the disorder indicates the need for quantitative analyses of uroliths removed by cystotomy. Compliance of the owners with recommendations to minimize recurrent urolithiasis might have been beneficial. Results of medical therapy designed to induce dissolution of uroliths in this case are representative of preliminary findings of medical dissolution of naturally occurring struvite uroliths in ten other cats. It is of interest that the uroliths dissolved even though no effort was made to induce…

Asparagus P(R) cannot compete with first-line diuretics in lowering the blood pressure in treatment-requiring antihypertensives.

A six week clinical surveillance of 163 patients receiving maximally tolerable doses of Asparagus P((R)), a proprietary mixture pulversied dried asparagus root and parsley leaf, has not indicated any clinically useful antihypertensive effect. The profile of adverse events (with 7 patients leaving the surveillance early because of renally related complaints) contraindicates its use in hypertension or to promote flushing of the efferent tract in rental inflammatory conditions or urolithiasis.

Diagnostic Imaging of the Reptile Urinary System.

Reptiles can develop various diseases of the urinary tract, including, but not exclusively, urolithiasis, gout, acute and chronic kidney injury, and secondary renal hyperparathyroidism. Diagnostic imaging is instrumental in differentiating and diagnosing these ailments. This article describes the current diagnostic imaging approaches used in reptile medicine for evaluation of urinary tract disease. The use of radiographs, ultrasonography, computed tomography, MRI, and endoscopy is discussed and compared for the evaluation of urinary tract disease in reptiles.

Ureterolithiasis and papilloma formation in the ureter of a guinea pig.

A 5-year-old guinea pig with suspected urolithiasis was presented for radiology and ultrasound examinations of the abdomen. Radiographically, an irregular-shaped mineral opacity was detected in the area of the urinary bladder. Ultrasonographically, pyelectasia of the right kidney, hydroureter with an ureterolith cranial to a thickened ureter wall close to the ureterovesical junction, and a thickened urinary bladder wall were detected. Histopathologically, the thickened ureter wall was found to be a papilloma. The ureter calculus consisted of 100% calcite.

Renal graft failure due to type 1 primary hyperoxaluria.

Primary hyperoxaluria type 1 (PH1) usually presents with recurrent urolithiasis, nephrocalcinosis and progressive renal failure at a relatively young age. This report describes a patient who, due to the late onset of end-stage renal disease, had been diagnosed with PH1 only after failure of his second kidney graft. Retrospectively, his vascular problems, skeletal abnormalities and cardiac arrhythmias fit the picture of severe systemic oxalosis. Possible therapeutic options are discussed.

Laparoscopic ureterolithotomy in a child with horseshoe kidney.

A horseshoe kidney (HSK) is the most frequent genitourinary fusion anomaly in lower pole of kidney fused during fetal development. This syndrome frequently results in morphological defects such as ectopia and malrotation. These structural alterations can lead to ureteropelvic junction obstruction, kidney stones and urinary tract infections. Urolithiasis is a common consequence of the HSK. This case report describes the surgical treatment of ureteric calculus in a boy with an HSK using laparoscopic techniques.

[Changes in the nervous system in kidney diseases].

A total of 150 patients with renal pathology (chronic glomerulonephritis, pyelonephritis, urolithiasis, etc.) accompanied by the development of psychoneurological disturbances were examined. In the initial stage of the renal insufficiency the authors observed neurasthenic, radicular, polyneuritic, renovisceral syndromes, in a more remote stage--encephalopathies and disturbances of the brain circulation. The important part in the above-mentioned disturbances is played by azotemia, metabolic acidosis, disturbances of fluid-electrolyte and albumine balance, as well as arterial hypertension.

What do we miss without contrast in patients with flank pain?

Flank pain is a common complaint in the emergency department, and the most frequent cause of flank pain is urolithiasis. Spontaneous renal artery dissection and renal artery thrombosis are rare causes of abdominal pain which can result in renal parenchymal injury. They are mostly difficult to diagnose and treat in the emergency setting. The present report describes 2 patients admitted to the emergency department because of acute flank pain who were diagnosed with renal infarction.

Increased risk of chronic kidney disease in uric acid stone formers with high neutrophil-to-lymphocyte ratio.

Urolithiasis is associated with an increased risk of chronic kidney disease (CKD), irrespective of stone compositions. Chronic inflammation is an important factor for CKD progression. Neutrophil-to-lymphocyte ratio (NLR) has been recognized as a reliable biomarker of inflammation, yet its use in predicting renal deterioration in patients with urolithiasis remains limited. We aimed to explore whether the combination of stone composition and NLR could be useful as a predictor for CKD risk. A total of 336 stone formers with at least one stone submission for analysis were enrolled in the retrospec…

Radiation From Kidney-Ureter-Bladder Radiographs Is Not Trivial.

OBJECTIVE: To estimate effective dose of kidney-ureter-bladder (KUB) radiographs in a contemporary population of patients with urolithiasis. METHODS: A retrospective review was performed to identify patients visiting a urology clinic for urolithiasis where a KUB was obtained and whom had a recent computed tomography (CT). Effective dose for KUBs was estimated using a Monte Carlo based simulation program and for CT utilizing the reported dose-length-product. Age, gender, body mass index, and abdominal diameter were analyzed for association with effective dose. KUBs performed at outside facilit…

Antiurolithiasis Activity of Bioactivity Guided Fraction of Bergenia ligulata against Ethylene Glycol Induced Renal Calculi in Rat.

Dried rhizome of Bergenia ligulata (pashanbhed) is commonly used as a traditional herbal medicine with a wide range of therapeutic applications including urolithiasis. Aqueous extract of B. ligulata was prepared through maceration followed by decoction (mother extract, 35.9% w/w). Further, polarity based fractions were prepared successively from mother extract which yielded 3.4, 2.9, 5.4, 7.5, and 11.3% w/w of hexane, toluene, dichloromethane (DCM), n-butanol, and water fractions, respectively. The in vitro, ex vivo, and real-time antiurolithiasis activity of mother extract and fractions were …

Idiopathic hypercalcemia in cats.

Unexplained hypercalcemia has been increasingly recognized in cats since 1990. In some instances, hypercalcemia has been associated with calcium oxalate urolithiasis, and some affected cats have been fed acidifying diets. We studied the laboratory findings, clinical course, and treatment of 20 cats with idiopathic hypercalcemia. Eight (40%) of the cats were longhaired and all 14 cats for which adequate dietary history was available had been fed acidifying diets. Clinical signs included vomiting (6 cats), weight loss (4 cats), dysuria (4 cats), anorexia (3 cats), and inappropriate urinations (3…

Ureteric calculus with migration into psoas muscle - Case report and literature review.

Urolithiasis is a common condition managed by urologists. We present a rare CASE of a ureteric calculus migrating into the patient's psoas muscle in context of chronic infection and obstruction and summarize the literature surrounding this phenomenon. To our knowledge, only four cases have previously been presented in the literature of obstructing calculi migrating into the retroperitoneum. Our case is the first in which there was no associated psoas abscess and the migrated calculus has remained in situ without any further sequelae.

A Case Report of an Obstructing Ureteral Nephrogenic Adenoma in a Child Managed With Open Ileal Ureter.

Nephrogenic adenoma is a rare, benign lesion that can be encountered anywhere along the urinary tract. It is associated with genitourinary trauma, chronic inflammation, genitourinary surgery, renal transplant, urolithiasis, and radiation. In children, these lesions are almost exclusively found in the bladder. However, we report an unusual case of a 15-year-old boy with no prior urologic history who presented with an obstructing right ureteral nephrogenic adenoma that required an ileal ureter interposition and right ureterectomy.

Xanthogranulomatous pyelonephritis (XGPN) mimicking a "renal cell carcinoma with renal vein thrombus and paracaval lymphadenopathy".

We present a case of Xanthogranulomatous pyelonephritis mimicking as a renal cell carcinoma. This was an elderly lady who presented with pyonephrosis due to urolithiasis. On evaluation she was found to have a space occupying mass in the right kidney. Further investigations revealed an enhancing tumor with renal vein thrombus and paracaval lymphadenopathy. Subsequent histopathology showed evidence of XGPN with no malignancy. This case report highlights the fact there are a number of imaging and clinical overlaps in the diagnosis, assessment and management of this entity.

[Marked primary hyperparathyroidism not diagnosed before death].

A case of primary hyperparathyroidism (PHPT) in a patient 47 years of age diagnosed only at autopsy is reported. PHPT deposits of calcium into the muscles and heart vessels provoked irreversible cardiovascular disorders which caused death. Renal lesions were intravitally diagnosed as isolated urolithiasis but not as manifestations of PHPT. High blood levels of ionised calcium in this patient were thought a laboratory mistake though the above clinical symptoms should have excluded such opinion.
