Clinical Characteristics of Dizzy Patients Showing Discordant Results Between Bithermal Caloric Test and Video Head Impulse Test.

Background/Objectives: To evaluate the clinical characteristics and diagnostic significance of dissociation between bithermal caloric test and video head impulse test (vHIT) in patients presenting with dizziness. Methods: We retrospectively reviewed 644 patients who underwent bithermal caloric testing and vHIT at a university-affiliated general hospital. Patients were classified into concordant and discordant groups based on test results. The discordant group was further subdivided into those with abnormal caloric test and normal vHIT, and those with normal caloric test and abnormal vHIT. Demo

Hearing impairment and vestibular function in patients with a pathogenic splice variant in the LHX3 gene.

BACKGROUND: LHX3 is a gene encoding a LIM-homeodomain transcription factor important for the fetal development of several organs, such as the pituitary gland, spinal motor neurons and the inner ear. Pathogenic and likely pathogenic variants in the LHX3 gene are infrequent and result in a rare syndrome known as combined pituitary hormone deficiency-3, CPHD3. METHODS: We have studied hearing and vestibular functions in a group of eight individuals, aged 8-36 years, all of whom were homozygous for a specific variant in the LHX3 gene at chromosome 9q34. We reexamined the results of consecutive he

Motorized head impulse rotator in patients with vestibular schwannoma.

CONCLUSION: Motorized head impulse rotator is an effective technique to assess peripheral vestibular function. Approximately a quarter of patients with vestibular schwannoma (VS) had preserved preoperative responses. Vestibular disability could not be predicted based on vestibulo-ocular reflex (VOR) performance during motion stimuli, or in the caloric test. OBJECTIVES: To explore motorized head impulse rotator for evaluation of angular horizontal VOR in patients with VS, and to compare these responses to those of the caloric test and the symptoms. PATIENTS AND METHODS: We prospectively recor

Idiopathic bilateral vestibulopathy: an autoimmune disease?

Bilateral vestibulopathy (BV) is the loss of function of both peripheral labyrinths or of the eighth nerves. Its etiology remains obscure in approximately 20% to 50% of cases (so-called idiopathic bilateral vestibulopathy, IBV). Alternatively, the cause could be viral or vascular; to date, causative gene mutations have not been identified. Other potential disease mechanisms include autoimmune disorders. Antibodies have been detected against inner ear tissue (primarily against vestibular membranous labyrinth). The data suggest that the bulk of anti-labyrinthine autoantibodies may be an epipheno

Vestibular migraine as a mimic of benign paroxysmal positioning vertigo and Meniere's disease.

BackgroundVestibular migraine (VM) is a common cause of recurrent spontaneous and positional dizziness of varying durations. Short episodes of VM lasting seconds to minutes, triggered by changes in head position can resemble those of benign paroxysmal positioning vertigo (BPPV), while episodes lasting minutes to hours can mimic those of Meniere's disease (MD).ObjectiveWe aimed to compare symptoms and vestibular test results in patients with VM, BPPV and MD, where VM was categorized by episode duration.MethodsWe compared vestibular, aural and migraine symptoms, as well as cervical and ocular ve

Posturographic changes associated with music listening.

OBJECTIVES: The aim of this study was to ascertain whether listening to music might cause changes in human stability and be useful in fall prevention and rehabilitation. The aim was also to find what percentage of subjects without neurologic signs or symptoms associated with falling had less than ideal stability. DESIGN: Computer dynamic posturography (CDP) provided stability scores in 266 subjects without a history of falls or vertigo. Subjects were randomized into several different music listening groups and one control group. The music listening groups were given a daily specific music lis

Clinical Interpretation of Positional Nystagmus Provoked by both Dix-Hallpike and Supine Head-Roll Tests.

BACKGROUND: Both the Dix-Hallpike test and the supine head-roll test can provoke positional nystagmus in a group of benign paroxysmal positional vertigo patients, including but not limited to those with multiple canal involvement. This study aimed to determine the incidence and interpret the clinical significance of positional nystagmus provoked by both the Dix-Hallpike and the supine head-roll tests. METHODS: The results of video-nystagmography sessions recorded in the computer database that included both the Dix-Hallpike and the supine head-roll tests were examined. RESULTS: The records be

Less talked variants of benign paroxysmal positional vertigo.

The diagnostic criteria were established for benign paroxysmal positional vertigo (BPPV), the most common vestibular disorder worldwide, by the Barany Society in 2015. This marked an important milestone in the diagnosis and treatment of BPPV. However, there still remain uncertainties and ambiguities regarding the clinical features and pathophysiology of BPPV, and its clinical variants. In this manuscript, we will discuss 1) the emerging and controversial syndromes of BPPV (i.e., canalolithiasis of the anterior canal, cupulolthiasis of the posterior canal, and lithiasis of multiple canals) with

Primary position upbeat nystagmus.

Primary position upbeat nystagmus is a rare clinical finding. We report a patient with clinically isolated syndrome suggestive of multiple sclerosis who presented with primary position upbeat nystagmus. MRI revealed a demyelinating lesion in the lower medulla, which affected the nucleus intercalatus; this type of lesion inhibits the flocculovestibular inhibitory pathway, thereby causing upbeat nystagmus. Nystagmus still persisted after pulsed corticosteroid therapy. This could be due to a loss of central adaptation of the vestibulo-ocular system in our patient, because of more diffuse brainste

Vestibular migraine.

Vestibular migraine (VM) designates recurrent attacks of vertigo that are caused by migraine. VM presents with attacks of spontaneous or positional vertigo, lasting seconds to days, accompanied by migrainous symptoms. Because headache is often absent during acute attacks, other migrainous features have to be identified by thorough history taking. In contrast, vestibular testing serves mainly for the exclusion of other diagnoses. Treatment is targeted at the underlying migraine.

Multivariate vestibular testing: thresholds for bilateral Ménière's disease and aminoglycoside ototoxicity.

Although patient symptoms and an audiogram can typically identify the affected ear or ears in Ménière's disease, there are some cases where this differentiation is problematic. This paper concentrates on the sole use of vestibular test data to discriminate between unilateral and bilateral Ménière's disease. Patients that were known to have peripheral unilateral vestibular hypofunction (n = 104) were used as learning groups to define a region in multidimensional measurement space consisting of four vestibular test scores which summarized data from electronystagmography, sinusoidal harmonic acce

Neurological manifestations of hypothyroidism: a retrospective study of 29 dogs.

Neuromuscular signs in association with hypothyroidism are described in 29 dogs. Eleven dogs had lower motor neuron signs, 9 had peripheral vestibular deficits, 4 had megaesophagus, and 5 had laryngeal paralysis. Primarily older (mean = 9.5 years), large-breed dogs were affected, and there was no sex or breed predisposition. Duration of clinical signs before presentation ranged from 2 to 8 weeks (mean = 5 weeks). The diagnosis was based on (1) results of neurological examination (29 dogs); (2) electromyographic abnormalities (18 dogs), including fibrillation potentials (n = 18), positive sharp

[The status of galvanic assessment of equilibrium].

The galvanic test is the only method of localising the site of the disease in the differential diagnosis between a vestibular endorgan or neuronal lesion. The galvanic nystagmus persists even after complete destruction of the inner ear but is impaired as soon as the peripheral vestibular neurone is damaged. We investigated 30 normal subjects, 10 patients with Menière's disease and 20 patients with an acoustic neuroma. The patients had a reduced caloric response on one side. All subjects were stimulated monaural-unipolar. The photoelectronystagmograms were analysed by determining slow phase vel

Results of sinusoidal harmonic acceleration test in one thousand patients: preliminary report.

One thousand patients with dizziness were tested by sinusoidal harmonic acceleration with frequencies of .01, .02, .04, .08, and .16 Hz. Two hundred fifty (25% of data base) were randomly selected for this study. One hundred forty-eight diagnoses were confirmed: 66 (45%) patients had a clinical diagnosis of peripheral vestibular dysfunction, 28 (19%) had central vestibular dysfunction, and 53 (36%) had dizziness of undetermined cause. This preliminary study investigated the response patterns of phase (latency), asymmetry (slow phase preponderance), and gain (output/input) as they correlated wi
