Probabilities of Isolated and Co-Occurring Vestibular Disorder Symptom Clusters Identified Using the Dizziness Symptom Profile.

OBJECTIVES: Dizziness is among the most common reasons people seek medical care. There are data indicating patients with dizziness, unsteadiness, or vertigo may have multiple underlying vestibular disorders simultaneously contributing to the overall symptoms. Greater awareness of the probability that a patient will present with symptoms of co-occurring vestibular disorders has the potential to improve assessment and management, which could reduce healthcare costs and improve patient quality of life. The purpose of the current investigation was to determine the probabilities that a patient pres

Differential vulnerability of cochlear nuclei to Lmx1 deficiency: abnormal patterning and implications for auditory circuitry.

Precise regulation of progenitor identity is essential for the formation of functional auditory circuits. Cochlear nuclei arise from two major progenitor populations: Atoh1-expressing progenitors generate excitatory glutamatergic neurons, while Ptf1a-expressing progenitors generate inhibitory GABAergic and glycinergic neurons. Lmx1a and Lmx1b, expressed in the hindbrain roof plate, maintain Atoh1 expression in lower rhombic lip progenitors that give rise to excitatory neurons of the cochlear nuclei. Detailed analysis using dye tracing of cochlear and vestibular central projections revealed an

Schwann cells in the inner ear: development, disease, and regeneration.

Schwann cells are classically known as the constituent supporting cells of the peripheral nervous system. Beyond the scope of merely myelinating axons of the more saliently known neurons, Schwann cells comprise the majority of peripheral nervous system tissue. Through the lens of the inner ear, additional properties of Schwann cells are becoming elucidated. Therein, the process of myelin formation in development is more aptly understood as a homeostatic oscillation of differentiation status. Perpetual interaction between neural and non-neural cells of the inner ear maintains an intricate balan

Neuroglial tissue in human eighth nerve specimens.

One hundred eighty nerve biopsy specimens from patients with eighth nerve tumors or other vertiginous diseases requiring vestibular neurectomy were studied. In many specimens light microscopy revealed pale areas among the myelinated nerve fibers in the neurolemmal portion of the eighth nerve. Electron microscopy showed that these were ectopic areas of glial tissue consisting of bundles of numerous cytoplasmic processes of fibrous astrocytes. The abundance of filaments within each process and the occurrence of microtubuli suggest that these processes are reactive astrocytes and accordingly are

TMC Proteins Modulate Egg Laying and Membrane Excitability through a Background Leak Conductance in C. elegans.

Membrane excitability is a fundamentally important feature for all excitable cells including both neurons and muscle cells. However, the background depolarizing conductances in excitable cells, especially in muscle cells, are not well characterized. Although mutations in transmembrane channel-like (TMC) proteins TMC1 and TMC2 cause deafness and vestibular defects in mammals, their precise action modes are elusive. Here, we discover that both TMC-1 and TMC-2 are required for normal egg laying in C. elegans. Mutations in these TMC proteins cause membrane hyperpolarization and disrupt the rhythmi

Cochlear implantation in superficial siderosis.

Superficial siderosis (SS) of the central nervous system has been thought to be a rare condition that generates progressive hearing loss, ataxia, pyramidal signs, and dementia. The main cause of hearing loss by SS is thought to be neuronal. Because there is no histopathologic report of the human temporal bone in SS, there is a debate about the possibility of cochlear involvement. We present a 25-year-old man who was investigated for bilateral progressive sensorineural hearing loss and vestibular failure after head trauma. On brain MRI, SS of the central nervous system was detected. Distortion

Vestibular and cochlear nerve enhancement on MRI and its correlation with vestibulocochlear functional deficits in patients with Ramsay Hunt syndrome.

OBJECTIVE: The correlation between enhancement of the vestibulocochlear nerves on gadolinium-enhanced magnetic resonance imaging (MRI) and vestibulocochlear functional deficits was examined in patients with Ramsay Hunt syndrome (RHS). METHODS: Nineteen patients with RHS who showed herpes zoster oticus, peripheral facial palsy, and vertigo were enrolled. Canal paresis (CP) in the caloric test, abnormal response to ocular and cervical vestibular myogenic potentials (oVEMP and cVEMP), and refractory sensorineural hearing loss were evaluated. MRI images perpendicular to the internal auditory cana

The lateral vestibular nucleus is a site for the depressant action of benzodiazepines on the crossed extensor reflex.

The effects of diazepam microinjected into the subcerebellar nuclei, the lateral vestibular nucleus (LVN), the red nucleus and the substantia nigra were investigated using the crossed extensor reflex (CER) model in chloralose-anesthetized rats. Microinjections of diazepam at doses of 1 and 25 ng/animal into the left LVN, ipsilateral to the testing muscle, produced a dose-related CER depression, while injections of diazepam at a dose of up to 100 ng/animal into the right LVN and into other regions tested produced little effect. The CER depression produced by diazepam could be clearly reversed b

Eye-centered visual receptive fields in the ventral intraparietal area.

The ventral intraparietal area (VIP) processes multisensory visual, vestibular, tactile, and auditory signals in diverse reference frames. We recently reported that visual heading signals in VIP are represented in an approximately eye-centered reference frame when measured using large-field optic flow stimuli. No VIP neuron was found to have head-centered visual heading tuning, and only a small proportion of cells had reference frames that were intermediate between eye- and head-centered. In contrast, previous studies using moving bar stimuli have reported that visual receptive fields (RFs) in

Calcitonin-gene-related-peptide-immunoreactive innervation of the rat head with emphasis on specialized sensory structures.

The distribution of calcitonin-gene-related peptide-like immunoreactivity (CGRP-IR) was studied in sections of decalcified rat head and selected whole-mount preparations in order to address the complex peptidergic innervation patterns in peripheral cephalic specialized zones and to examine neuronal ganglia in situ. Labeled neuron somata in trigeminal, glossopharyngeal, and vagal ganglia comprised a large proportion of small to medium size type B ganglion cells. Parasympathetic ganglia (ciliary, otic, sphenopalatine, submandibular) revealed a small population of labeled somata and numerous peri

Clinical Efficacy of Vestibular Rehabilitation Training Combined with Medical Wisdom Platform on Vertigo Caused by Vestibular Neuritis.

OBJECTIVE: The clinical manifestation of vertigo caused by vestibular neuritis is acute and persistent vertigo, accompanied by nausea, vomiting, and dizziness. Low-dose glucocorticoid therapy is recommended in the acute phase, while drug therapy is not recommended in the recovery phase. Therefore, it is of certain clinical value to explore other treatment options. This study hopes to better fill the current research gap in non-drug treatment of vertigo caused by vestibular neuritis. METHODS: The medical data of 96 patients with vertigo caused by vestibular neuritis in our hospital from May 20

Skull vibration-induced nystagmus in vestibular neuritis.

BACKGROUND: As described by Dumas, an 80-100 Hz vibration applied to the mastoid produces a horizontal nystagmus, with quick phases beating away from the affected side in patients with unilateral vestibular loss (UVL). AIM/OBJECTIVES: To elucidate the characteristics of skull vibration-induced nystagmus (SVIN) in patients suffering from vestibular neuritis and how these characteristics are related to information provided by the video head impulse test (vHIT). MATERIAL AND METHODS: Sixty-three patients were enrolled to perform a vHIT to measure the gain in both ears. SVIN was induced with sti

Dynamic visual acuity during passive and self-generated transient head rotation in normal and unilaterally vestibulopathic humans.

To determine whether dynamic visual acuity (DVA) during head rotations on the stationary body can lateralize unilateral vestibular deafferentation and detect non-labyrinthine compensation mechanisms, 15 normal and 11 subjects with unilateral vestibular deafferentation underwent manually imposed and self-generated transient yaw head rotations during measurement of binocular DVA. DVA was measured by a four-alternative, forced choice, staircase procedure with optotype presentation only when head velocity exceeded thresholds of 50 degree or 75 degree/s. Eye and head movements were recorded using s

Sport-related concussion adopt a more conservative approach to straight path walking and turning during tandem gait.

BACKGROUND: It is currently unknown what specific neuronal deficits influence postural instability following SRC; however, the modulation of postural control relies heavily on the appropriate integration of sensory information from the visual, vestibular, and somatosensory system. It is possible symptom provocation of vestibular or ocular function is related to unsteady gait patterns during tandem gait. AIM: The purpose of this study was to evaluate the differences in temporal and center of pressure (CoP) metrics during discrete events of instrumented tandem gait (iTG) among those with sport-

Change of gait after unilateral vestibular neuritis: a prospective longitudinal observation study.

Although symptoms of unilateral vestibular neuritis (uVN) resolve spontaneously within several weeks, recovery of gait function has unclearness in gait parameter changes and mediolateral stability improvements. In addition, prospective longitudinal studies on gait parameters after uVN are lacking. This study was conducted to reveal longitudinal change of gait function after acute uVN and to help the precise rehabilitation planning. Twenty three participants with uVN and 20 controls were included. 3D gait analyses were conducted three times after uVN onset at monthly intervals. From the gait an
