Testing for osteogenesis imperfecta in cases of suspected non-accidental injury.

To evaluate if laboratory testing for osteogenesis imperfecta (OI) identifies children unrecognised by clinical examination in instances where non-accidental injury (NAI) is suspected as the likely cause of fracture, we carried out a retrospective review of available medical records and biochemical test results from 262 patients. Cultured fibroblasts were received for biochemical testing for OI from children in whom the diagnosis of NAI was suspected. Eleven of the samples had alterations in the amount or structure of type I collagen synthesised, consistent with the diagnosis of OI, and in 11

Osteogenesis Imperfecta and non-accidental injury: problems in diagnosis and management.

It has been noted in the literature that Osteogenesis Imperfecta is frequently mistaken for non-accidental injury. This article serves to illustrate the difficulty in differentiating between the two conditions and that they can occur concomitantly in one patient.

Accidental splenic injury during surgical treatment of the colon and rectum.

Although operative injury to the spleen continues to account for a large number of splenectomies, the incidence of injury during operations upon the colon and rectum has not previously been reported. Of 993 consecutive colonic and rectal excisions, the spleen was injured in eight and splenectomy resulted in three. Two hundred and sixty patients had the splenic flexure of the colon mobilized. All splenic injuries occurred in this group (3.1 per cent) for an incidence of splenectomy during splenic flexure mobilization of 1.15 per cent. In all patients, splenic injury consisted of a capsular tear

Non-accidental childhood injury presenting at a hospital casualty department.

One hundred children who attended a Sydney Paediatric Casualty Department with injuries, burns or ingestions of harmful substances were divided into two groups. One group (30 children) consisted of cases which were considered to be those of child abuse or neglect (CAN). In the other group (70 children) there was no suspicion of child abuse or neglect. This was regarded as the "simple accident" (SA) group. The 100 families were visited at home and interviewed about their child's health, child rearing practices, parental expectations for the child, and socioeconomic status. Factors which had a s

Severity and impact of accidental bowel leakage two decades after no, one, or two sphincter injuries.

BACKGROUND: Information about the long-term severity and subjective impact of anal incontinence in women after 1 or 2 consecutive obstetrical anal sphincter injuries is still scarce and contradictory. OBJECTIVE: This study aimed to describe the severity and impact of anal incontinence among women with 2 previous deliveries 2 decades after birth and to analyze the relative effect of 1 vs 2 obstetrical anal sphincter injuries in comparison with no obstetrical anal sphincter injuries and the possible influence of obstetrical anal sphincter injury on other pelvic floor disorders. Risk factors for recurrent injuries in victims of suspected non-accidental trauma: a retrospective cohort study.

BACKGROUND: Many children who are victims of non-accidental trauma (NAT) may be repeatedly evaluated for injuries related to maltreatment. The purpose of this study was to identify risk factors for repeated injuries in children with suspected NAT. METHODS: We conducted a retrospective cohort study using claims data from a pediatric Medicaid accountable care organization. Children with birth claims and at least one non-birth related claim indicating a diagnosis of NAT or skeletal survey in 2007-2011 were included. Recurrent events were defined as independent episodes of care involving an urgen

Youth versus adult "weightlifting" injuries presenting to United States emergency rooms: accidental versus nonaccidental injury mechanisms.

Resistance training has previously been purported to be unsafe and ineffective in children. The purpose of this investigation was to evaluate resistance training-related injuries presenting to U.S. emergency rooms by age, type, and mechanism of injury. We hypothesized that older athletes would sustain greater percentages of joint sprains and muscle strains, whereas younger athletes would sustain a greater percentage of accidental injuries that would result in an increased percentage of fractures in youths. The U.S. Consumer Product Safety Commission (CPSC) National Electronic Injury Surveillan…

Accidental injuries associated with nonhuman primate exposure at two regional primate research centers (USA): 1988-1993.

Although occupationally acquired zoonoses of nonhuman primates have been well documented, the epidemiology of work-related injuries associated with occupational exposure to nonhuman primates has not been studied. To investigate such injuries, we retrospectively reviewed injury records at one regional primate research center and distributed a self-administered, anonymous questionnaire to at-risk personnel at two centers. Records of bite, animal-inflicted scratch, needlestick, cut, and mucous membrane exposure injuries were reviewed at one center for the 5-year period 1988 to 1993 to determine i

[Incomplete clinical inquiry and documentation of findings as the cause of legal problems after accidental injuries].

The starting point of every expert opinion is the clinical inquiry and the description of clinical findings based on a sorrow and complete examination. Without this it can be difficult or even impossible for the expert to answer with sufficient safety the relevant question concerning the sequelae of an accident, or concerning the causal connection between trauma and damage or, as well, concerning possible medical malpractice. Based on 238 expert opinions given by the author within two years, it becomes obvious, that in 18% (41 cases) the opinion ended in an unsatisfactory manner, exclusively f

Non-accidental Trauma Injury Patterns and Outcomes: A Single Institutional Experience.

Non-accidental trauma (NAT) victims account for a significant percentage of our pediatric trauma population. We sought to better understand the injury patterns and outcomes of NAT victims who were treated at our level I pediatric trauma center. Trauma registry data were used to identify NAT victims between January 2008 and December 2012. Demographic data, injury severity, hospital course, and outcomes were evaluated. One hundred and eighty-eight cases of suspected NAT were identified. Children were mostly male and white. The median age was 1.1 years; the median Injury Severity Score was 9. Tra

Accidental penetrating brain injury through anterior fontanelle: a rare phenomenon.

Penetrating injuries of the anterior fontanelle are exceedingly uncommon and are most often associated with inflictedinjury. This report of a 7-month-old male infant, whose father was working with a knife which accidentally slipped from his hand and penetrated the infants anterior fontanelle and frontal lobe, demonstrates that accidental injuries may also occur rarely.
