Congenital nephrogenic diabetes insipidus treated with acetazolamide.

Congenital nephrogenic diabetes insipidus (CNDI) is a rare disorder. The condition is characterised by an inability of distal nephron segments to respond to normal or raised concentrations of serum antidiuretic hormone. In this report, we describe the case of a 13-year-old male known with CNDI who experienced a pedestrian vehicle accident leading to coma following a head injury. Intra-operatively, severe hypernatraemia and polyuria were observed. Following an inadequate response to conventional therapy, acetazolamide was prescribed resulting in an immediate response to therapy. To the best of …

Acetazolamide: future perspective in topical glaucoma therapeutics.

Through this review it is contemplated that acetazolamide (ACZ), an age-old treatment for glaucoma with a myriad of side effects and inadequate topical effectiveness, may be formulated into a topically effective agent by utilizing various newer formulation approaches of ocular drug delivery. Even though it has a poor solubility and penetration power, various studies mentioned in the review indicate that it is possible to successfully formulate topically effective ACZ by using: (i) high concentration of the drug, (ii) surfactant gel preparations of ACZ, (iii) ACZ loaded into liposomes, (iv) cyc…

Acetazolamide treatment for infantile central sleep apnea.

Central sleep apnea is a common respiratory pattern in healthy neonates. Nevertheless, frequent central sleep apnea associated with drops in oxygen saturation may contribute to infantile morbidity. Recently, low-dose acetazolamide was shown to reduce symptomatic central sleep apnea in adults. We treated 12 infants, median conceptional age 42 weeks (range, 40-44 weeks), with central sleep apnea. In all cases, the central apnea index was >40/h total sleeping time (apnea > or = 3 sec). The cumulative duration of drops in oxygen saturation below 90% was more than 3 min/h total sleeping time. All i…

Zinc therapy of acetazolamide-induced side-effects.

A double-blind cross-over trial extending over 4 weeks with administration of 0.2 g zinc sulphate 3 times daily and a placebo was carried out on 12 patients having grave acetazolamide-induced side-effects manifesting themselves as gustatory disorder, anorexia, and paraesthesia. All the patients had S-zinc levels within the normal range. These rose during zinc therapy periods, to fall again within placebo periods. Recording of the degrees of subjective side-effects based on interviews showed the side-effects to abate towards the conclusion of the trial period, independently of the randomizing p…

Central sleep apnea. Improvement with acetazolamide therapy.

Respiratory rhythm during sleep may be dependent on blood pH with apneas being associated with alkalosis. Acidification may therefore have therapeutic value in some forms of sleep apnea. We administered acetazolamide to six patients with symptomatic central sleep apnea, a disorder of respiratory rhythm with little or no upper airway obstruction. Sleep studies were carried out before and after one week of drug therapy, during which time the mean arterial pH decreased from 7.42 to 7.34. All six patients had significant improvement, demonstrating a 69% reduction in total apneas. Five of the six p…

Non-Cardiogenic Pulmonary Oedema Provoked by Acetazolamide.

A 61-year-old male was brought to the Emergency Department with severe shortness of breath, a throbbing headache, sweating, nausea, vomiting and diarrhoea after the administration of an acetazolamide tablet (250 mg) at a private ophthalmology clinic. On presentation, a chest X-ray was performed, showing diffuse alveolar opacities bilaterally, indicating pulmonary oedema, as seen in CT chest also. However, his echocardiogram revealed a normal ejection fraction with no signs of ischemia. He was subsequently diagnosed with non-cardiogenic pulmonary oedema (NCPE) and was immediately started on hig…

Acetazolamide Attenuates Lithium-Induced Nephrogenic Diabetes Insipidus.

To reduce lithium-induced nephrogenic diabetes insipidus (lithium-NDI), patients with bipolar disorder are treated with thiazide and amiloride, which are thought to induce antidiuresis by a compensatory increase in prourine uptake in proximal tubules. However, thiazides induced antidiuresis and alkalinized the urine in lithium-NDI mice lacking the sodium-chloride cotransporter, suggesting that inhibition of carbonic anhydrases (CAs) confers the beneficial thiazide effect. Therefore, we tested the effect of the CA-specific blocker acetazolamide in lithium-NDI. In collecting duct (mpkCCD) cells,…

Acetazolamide impairs fear memory consolidation in rodents.

Acetazolamide (AZ) is an carbonic anhydrase inhibitor, which has been used in the treatment of seizures, mountain sickness and glaucoma. Memory impairment by AZ has been reported in patient interviews; however, the related mechanism is unclear. We applied two fear conditioning paradigms, shuttle avoidance and passive avoidance, in both rats and mice to investigate this clinical anecdote. Adult Wistar rats receiving AZ 1 h before the shuttle avoidance test showed decreased avoidance rates, especially at high dosage. Adult ICR mice receiving AZ both before and after acquisition trials showed the…

[Contact dermatitis caused by acetazolamide under occlusion].

INTRODUCTION: Acetazolamide is a carbonic anhydrase inhibitor used topically for local secondary treatment of posttraumatic or postoperative edema. CASES: Two women had contact dermatitis, secondarily extensive, after local cutaneous use of acetazolamide under a compression panty after liposuction. The eruption disappeared after acetazolamide was stopped and local treatment administered. Cutaneous tests were positive for acetazolamide. DISCUSSION: Local allergic reactions are mentioned in the monograph on topical acetazolamide. Cases of contact dermatitis from this drug have not so far been …

Reversible pontine ischemia caused by acetazolamide challenge.

We report the findings in a patient with a midbasilar artery stenosis in whom reversible ischemic neurologic deficits developed during cerebral blood flow imaging with acetazolamide challenge. The potential for ischemic complications from acetazolamide challenge is discussed.

Acetazolamide reduces peripheral afferent transmission in humans.

Carbonic anhydrase has been localized in skeletal muscle and nerve, thus, inhibition with acetazolamide (ACZ) may alter nerve and/or muscle function in healthy humans. ACZ (3 oral doses 14, 8, and 2 h prior to testing) reduced isometric force (37%) and peak to peak electromyographic (EMG) amplitude (1.38 mV to 0.83 mV), while increasing EMG latency associated with a unilateral Achilles tendon-tap. Reflex recovery profiles, following a contralateral conditioning tap, were similar in both placebo and ACZ experiments. ACZ led to significant changes in Hmax/Mmax ratio (52.19/14.42 to 45.73/15.65) …

Use of acetazolamide in volume overload: start at the beginning.

The treatment of volume overload has vexed physicians for centuries. References to herbal remedies for the treatment of fluid retention date … that spurred the interest in synthetic diuretic agents. Despite being the first agent synthesized, acetazolamide became the "forgotten diuretic" until a recent resurgence has reinvigorated interest in its utility in patients with acute decompensated heart failure. This review will describe the background, clinical trials, and proposed utility of acetazolamide …

Exploring Neurohormonal Modulation by Acetazolamide in Heart Failure.

Background Heart failure (HF) is commonly managed by addressing water and sodium (Na) balance, with arterial circulation playing a major role in influencing renal Na and water excretion. Recently, chloride (Cl) has been recognized as an important factor in HF, associated with volume regulation and its modulation of renin-angiotensin-aldosterone system (RAAS) activity through macula densa signaling, which impacts Na retention and neurohormonal activation. Acetazolamide, a carbonic anhydrase inhibitor, can enhance decongestion in HF by increasing urinary Na and Cl excretion when added to loop di…
