Gaps in the anion gap.

OBJECTIVE: To review limitations of the use of serum anion gap in clinical practice. DATA SOURCES: Original reports and reviews. STUDY SELECTION: Sources containing the most recent pertinent information. DATA SYNTHESIS: Theoretical and practical limitations beset the use of serum anion gap. Awareness of these limitations reduces but does not eliminate wrong diagnoses based on the anion gaps. CONCLUSIONS: Serum anion gap has a limited value in the differential diagnosis of acid-base disorders and can be misleading.

Implementation of rules for the estimation of compensatory responses in acid-base analysis: A review and proposal for laboratory practice.

Acid-base homeostasis is essential for cellular and systemic function. Its maintenance requires tightly regulated compensatory mechanisms capable of correcting disturbances that threaten physiological balance, as dysregulation can be life-threatening. When a primary metabolic disturbance occurs and alters bicarbonate concentration (HCO₃), the main compensatory mechanism is a respiratory response that adjusts the partial pressure of carbon dioxide (PCO₂). Conversely, a primary respiratory disturbance, whether acute or chronic, leads to changes in PCO₂, and compensatory response occurs through a

Breathing and balance: Clinical insights and management strategies of respiratory acid-base disorders.

Respiratory acidosis and alkalosis are two of the four main acid-base processes encountered in clinical practice. These two processes are tightly balanced by breathing. Identification and management of respiratory acid-base disorders are important for clinicians, particularly when caring for patients with underlying pulmonary and neurological conditions. Basic knowledge of respiratory function, alveolar gas exchange, and innate compensation concepts can provide clinicians with the tools to design safe and effective nutrition care plans. These concepts will also aid in monitoring response to me

Regulation of Acid-Base Balance in Patients With Chronic Kidney Disease.

Normallly the kidneys handle the daily acid load arising from net endogenous acid production from the metabolism of ingested animal protein (acid) and vegetables (base). With chronic kidney disease, reduced acid excretion by the kidneys is primarily due to reduced ammonium excretion such that when acid excertion falls below acid porduction, acid accumulation occurs. With even mild reductions in glomerular filtration rate (60 to 90 ml/min), net acid excretion may fall below net acid production resulting in acid retention which may be initially sequestered in interstitial compartments in the kid

[Design and development of analysis software for acid-base balance disorder based on Visual Basic.NET].

OBJECTIVE: To develop a diagnostic analysis software for determining the type of acid-base balance disorder. METHODS: Mathematical models w … pH, arterial partial pressure of carbon dioxide (PaCO2), HCO3- and anion gap (AG)] and the types of acid-base imbalance (literature results) were recorded. All cases were reanalyzed by software and the type of acid-base balance disorder was determined (software diagnostic type). Kappa-test and McNemar-test were performed for the two diagnostic results. RESULTS: The …

[Interpretation of acid-base disorders].

Keeping blood pH levels stable within narrow limits is prerequisite for maintaining normal body function. Disruptions of this equilibrium can occur for a variety of reasons and may result in life-threatening conditions. Therefore, the identification and the interpretation of acid-base disorders are of great importance in emergency medicine. The same applies to intensive care as well as to almost all areas of internal medicine. In this article, we provide a practical overview of all relevant acid-base disorders that are due to both metabolic and respiratory disturbances. In addition, it shows h

Electrolytes disturbances in cancer patients.

PURPOSE OF REVIEW: Hypernatremia, hyperphosphatemia, hypocalcaemia, hyperkalaemia and hypermagnesemia are electrolytes disturbances that can arise in cancer patients in relation to unique causes that are related to the cancer itself or its treatment and can lead to delay or interruption of cancer therapy. This article summarizes these main causes, the proposed pathophysiology and the recommended management for these disturbances. RECENT FINDINGS: There have been many cancer drugs approved in the field of oncology over the past several years and a subset of these drugs have been associated wit

The binge-purge syndrome: a common albeit unappreciated cause of acid-base and fluid-electrolyte disturbances.

The binge-purge syndrome (bulimarexia) is one of the four major eating disorders. It is a very important entity with which all clinicians should have some familiarity. This is true not only because of its frequency and the potential severity of several of its numerous complications, but because it has become one of the common masqueraders of modern medicine. Indeed, nephrologists may become interested in and involved with patients having this disorder because of their frequent fluid-electrolyte and acid-base disturbances, particularly hypokalemia and metabolic alkalosis, which initially often

[Bedside interpretation of complex metabolic acid-base disturbances using standard base excess partitioning].

Metabolic acid-base disturbances often result from several independent processes. These processes are difficult to tease out using traditional bicarbonate-centred approaches. According to standard base excess partitioning, standard base excess is the sum of the acid-base effects of changes in the concentration of sodium, chloride, lactate, albumin and an unspecified entity termed »Other Ions«. Since the first five of these entities are routinely analysed, »Other Ions« can be calculated at the bedside. In this article, five short case vignettes are used to illustrate how standard base excess pa

Low noncarbonic buffer power amplifies acute respiratory acid-base disorders in patients with sepsis: an in vitro study.

Patients with sepsis have typically reduced concentrations of hemoglobin and albumin, the major components of noncarbonic buffer power (β). This could expose patients to high pH variations during acid-base disorders. The objective of this study is to compare, in vitro, noncarbonic β of patients with sepsis with that of healthy volunteers, and evaluate its distinct components. Whole blood and isolated plasma of 18 patients with sepsis and 18 controls were equilibrated with different CO2 mixtures. Blood gases, pH, and electrolytes were measured. Noncarbonic β and noncarbonic β due to variations

Respiratory Acid-Base Disorders.

Respiratory acid-base disorders are often not thought of as frequently as their metabolic cousins, which occur more frequently in the emergency department. Although most respiratory and acid-base disturbances are driven by lung pathology, central nervous system and other organ systems can and do play a role as well. Although managing the airway and appropriate mechanical ventilation may be necessary, it is akin to placing a band-aid on a large wound. It is crucial for the emergency clinician to discover the etiology of the disturbance as management depends on treating the underlying etiology t

[Drug-induced acid-base disorders].

Drug-induced acid-base disorders may be classified into four categories with respect to the mechanism. 1. Metabolic acidosis is induced by a large acid loads incurred from exogenous sources (e.g. NH4Cl, or toxin ingestion) or endogenous acid production (e.g. generation of ketoacids or lactic acids by alcohol or phenformin) or base loss (e.g. abuse of laxatives). 2. Metabolic alkalosis results from exogenous bicarbonate loads (e.g. milk-alkali syndrome) or effective extracellular fluid contraction, potassium depletion plus hyperaldosteronism (e.g. vomiting, diuretics, or licorice). 3. Renal tub
