The purpose of the present study was to investigate the factors that may contribute to the sensitization to flour in baker's apprentices, and to predict this type of sensitization with a stepwise logistic regression model. A statistical study was conducted on 3697 patch-tested patients in order to find any cross-reacting or concomitant allergens. 44 correlations were found. 3 were well-known cross-reacting substances (e.g., benzocaine and p-phenylenediamine). 7 were unlikely, probably random, findings and the remaining 27 were considered to be either new cross-reacting allergens (e.g., promethazine and azo dyes) or concomitant allergens found in the same kind of products (e.g., nickel and cobalt in alloys, ethylenediamine and neomycin in Kenacombin cream, colophony and carba-mix in adhesive bandage).

It is shown that apple allergens are probably proteins and that they can be extracted in an active form only if reactions with phenolic compounds present in apple are inhibited. This is accomplished by incorporating chelators and solid polyvinylpolypyrrolidone in the extraction medium. Phenol, commonly used as a preservative, should not be added. In 13 patients with contact allergy to sesame oil, studies were undertaken to elucidate the nature of the allergens. Sesamol, sesamin and sesamolin were identified in crude and purified (pharmaceutical) sesame oil. Patch tests showed 8 of the 13 patients to be positive to sesamol and 12 to sesamolin and sesamin. Patch tests with the pure substances on thin-layer sheets were inconclusive as to any difference between these substances. Group allergy to several substances related to sesamol could not be clearly demonstrated.

Patch testing is an important tool in the diagnosis of allergic contact dermatitis. Although this technique can be accurate, occasionally the results may be inconclusive. A previously positive result to an allergen may become negative upon repeat testing, and this may complicate the process of achieving a definitive diagnosis.
