Carbonic anhydrase inhibitors and high altitude illnesses.

Carbonic anhydrase (CA) inhibitors, particularly acetazolamide, have been used at high altitude for decades to prevent or reduce acute mountain sickness (AMS), a syndrome of symptomatic intolerance to altitude characterized by headache, nausea, fatigue, anorexia and poor sleep. Principally CA inhibitors act to further augment ventilation over and above that stimulated by the hypoxia of high altitude by virtue of renal and endothelial cell CA inhibition which oppose the hypocapnic alkalosis resulting from the hypoxic ventilatory response (HVR), which acts to limit the full expression of the HVR

Sickle Cell Trait Is a Risk Factor for Another Form of Altitude Illness.

Millet, Grégoire P, and Philippe Connes. Sickle cell trait is a risk factor for another form of altitude illness. High Alt Med Biol. 27:57-59, 2026.-Acute mountain sickness, high-altitude pulmonary edema, and high-altitude cerebral edema have been widely investigated. In this letter, we claim that beyond these three altitude illnesses, one cannot rule out an additional serious concern related to altitude exposure in a particular population; the individuals with sickle cell trait.

Altitude hypoxia and hypoxemia: pathogenesis and management.

At high altitudes, which typically exceed 2500 m, approximately 80 million people reside permanently, with over a million visitors annually. The primary effect of high altitude is hypobaric hypoxia, which leads to decreased oxygen availability and a cascade of physiological responses. However, inadequate or excessive responses can lead to malacclimatization, resulting in hypoxemia and various high-altitude illnesses, including acute mountain sickness (AMS), high-altitude cerebral edema (HACE), high-altitude pulmonary edema (HAPE), chronic mountain sickness (CMS), and high-altitude pulmonary hy

Recompression therapy of mountain sickness.

This paper describes the treatment of a severe case of acute mountain sickness with a portable hyperbaric chamber. A 37-year old climber was treated for acute high altitude pulmonary oedema, which developed on the North Col of Mount Everest, at an altitude of 7,060 m. The treatment in the portable Gamow bag hyperbaric chamber lasted two hours, with a bag pressure of 103 mm Hg (0.136 kg/cm2 or 2 psig) using ambient air, without the addition of oxygen. With this pressure increase, the hyperbaric chamber lowered the patient's effective ambient altitude from 6,050 to 4,400 m. The treatment was suc

Alpine travel. Mountain sickness, the unwelcome companion.

A personal observation and two cases from the literature are described to draw attention to the dangers of mountain sickness and to the rapid progression from the milder form, termed acute mountain sickness, to the more serious conditions of high-altitude pulmonary or cerebral oedema and their possible fatal outcome. The pathogenesis and treatment of the disorders are briefly reviewed. It is stressed that there are no substitutes for acclimatization through gradual ascent and that the best treatment is to descend to a lower altitude.

High-altitude illness: Management approach.

In high altitudes, usually above 2500 m, travelers are faced with decreased partial pressure of oxygen along with decreased barometric pressure. High-altitude illness, a syndrome of acute mountain sickness, high-altitude cerebral edema and high-altitude pulmonary edema, occurs due to the hypobaric hypoxia when there is inadequate acclimatization. This review provides detailed information about pathophysiology, clinical features, prevention and treatment strategies for high-altitude illness according to the current literature.

High-altitude exposure remodels the gut microbiota: health and disease.

With the increasing number of individuals travelling to or residing in high-altitude regions, understanding the physiological and pathological consequences of such environments has become increasingly important. High-altitude exposure poses significant challenges to human health, primarily due to hypobaric hypoxia, which triggers a cascade of responses, including energy deficiency, oxidative stress, and inflammation. One of the critical consequences is the disruption of the gut barrier, which facilitates the translocation of the gut microbiota and further exacerbates local and systemic inflamm

Altitude illnesses.

Millions of people visit high-altitude regions annually and more than 80 million live permanently above 2,500 m. Acute high-altitude exposure can trigger high-altitude illnesses (HAIs), including acute mountain sickness (AMS), high-altitude cerebral oedema (HACE) and high-altitude pulmonary oedema (HAPE). Chronic mountain sickness (CMS) can affect high-altitude resident populations worldwide. The prevalence of acute HAIs varies according to acclimatization status, rate of ascent and individual susceptibility. AMS, characterized by headache, nausea, dizziness and fatigue, is usually benign and

High-altitude pulmonary hypertension: a pathophysiological entity to different diseases.

Pulmonary hypertension is a hallmark of high-altitude pulmonary oedema (HAPE) and of congestive right heart failure in subacute mountain sickness (SMS) and chronic mountain sickness (CMS) in the Himalayas and in the end-stage of CMS (Monge's disease) in the Andes. There are studies to suggest that transmission of excessively elevated pulmonary artery pressure and/or flow to the pulmonary capillaries leading to alveolar haemorrhage is the pathophysiological mechanism of HAPE. In the Himalayas, HAPE was successfully prevented by extending the acclimatisation period from a few days to 5 weeks, ho

Effect of furosemide on altitude tolerance in experimental animals.

Furosemide which is claimed to prevent acute mountain sickness and pulmonary oedema of high altitude was tried in experimental animals to evaluate its usefulness or otherwise as a measure of prophylaxis. 90 mice, 128 rats and 44 guinea pigs--all males--were used for the studies. Half the number in each species were kept as controls. The remaining half were further divided into two batches. The first batch was given furosemide 10 mg/kg body weight. Two h later they were exposed to simulated altitude of 30,000 ft for 6 h. The second batch was given two injections of furosemide at intervals of 24

High-altitude medicine.

Medical problems occur at high altitude because of the low inspired Po(2), which is caused by the reduced barometric pressure. The classical physiological responses to high altitude include hyperventilation, polycythemia, hypoxic pulmonary vasoconstriction-increased intracellular oxidative enzymes, and increased capillary density in muscle. However, with the discovery of hypoxia-inducible factors (HIFs), it is apparent that there is a multitude of responses to cellular hypoxia. HIFs constitute a master switch determining the general response of the body to oxygen deprivation. The recent discov

[High altitude retinal hemorrhages in the expeditions to 8,000 meter peaks. A study of 10 cases].

BACKGROUND: Retinal haemorrhages are common at high altitude. Their pathogenesis is unknown. It has been suggested that they are less frequent in sherpas, and that possible predisposing factors might be the abscence of previous high-altitude experience, the extent of the high-altitude hypoxic exposure, polycythemia (because of hyperviscosity), history of cough and Valsalva manoeuvres during the expedition, existence of severe forms of mountain sickness (high-altitude pulmonary oedema and high-altitude cerebral oedema) and use of antiinflammatory drugs. The aim of this study is to know the inci

Serum neurofilament level increases after ascent to 4559 m but is not related to acute mountain sickness.

BACKGROUND AND PURPOSE: At high altitude the brain is exposed to hypoxic stress, which may result in neurological conditions, with acute mountain sickness (AMS) being the most common. We aimed to test the hypothesis that rapid ascent to high altitude alters neuro-axonal integrity, which can be detected by increased concentration of serum neurofilament light (sNfL) in the blood and may even be exaggerated in people with AMS. METHODS: Serum neurofilament light was measured using a single-molecule array (Simoa, Quanterix, Lexington, MA, USA) assay at low altitude (423 m) in 47 healthy study part

Ear, nose, and throat effects of high altitude.

High altitude changes human physiology and can result in illnesses such as acute mountain sickness, high-altitude cerebral edema, and high-altitude pulmonary edema. The physiological impacts of high-altitude illnesses occur secondary to extravasation of fluid from the intravascular space into the extravascular space during a rapid ascent. Headache, hearing disturbances, vestibular disturbances, epistaxis, sleep apnea, coughing, respiratory tract infections, and nasal obstruction are main ear, nose, and throat complaints of individuals travelling to high altitude. These complaints can cause del

Acute high-altitude illness: a clinically orientated review.

Acute high-altitude illness is an encompassing term for the range of pathology that the unacclimatised individual can develop at increased altitude. This includes acute mountain sickness, high-altitude cerebral oedema and high-altitude pulmonary oedema. These conditions represent an increasing clinical problem as more individuals are exposed to the hypobaric hypoxic environment of high altitude for both work and leisure. In this review of acute high-altitude illness, the epidemiology, risk factors and pathophysiology are explored, before their prevention and treatment are discussed. Appropriat

Medicine at high altitude.

Medicine at high altitude provides important insights into the acute and chronic effects of hypoxia. Acute mountain sickness (AMS) is a common syndrome occurring after acute ascent to over 2,500 m and is caused by increased capillary permeability. A number of factors have been identified that increase the risk of AMS, in particular exercise. Avoiding rapid ascent, undue exercise and the use of acetazolamide are useful preventative measures but severe symptoms may require oxygen, dexamethasone and descent. Acute mountain sickness is usually self-limiting but may progress into the serious syndro

Mountain neurology.

Mountain climbers may develop specific illnesses that largely depend on the altitude reached and the rate of ascent. The popularity of travel to high altitude destinations, extreme tourist activities and mountain climbing means that neurologists in low-altitude countries are increasingly likely to encounter neurological problems and disorders in people exposed to high altitude. Additionally, they may have to advise patients with pre-existing neurological conditions on the risks of ascent to altitude. This article focuses on neurological-related high-altitude illnesses: acute mountain sickness
