[Repair of soft-tissue defect of amputation stumps of the forearm with free flap from the traumatic amputated extremity].

OBJECTIVE: To report the clinical results of the repair of soft-tissue defect of amputation stumps of the forearm with free flap from the traumatic amputated extremity. METHODS: Five patients, 4 males and 1 female, aged 32 (22 - 43), with soft-tissue defect of the remaining stump of the traumatic amputation of the forearm, 3 cases in the right forearm and 2 cases in the left forearm, underwent repair of the defect by free flap from the traumatic amputated extremity with the size of the flaps ranging from 8 cm x 9 cm to 9 cm x 12 cm. The patients were followed up for 2.6 years (1.5 - 3.5 years…

Amputation stump management. A preliminary report on the use of shrink plastic film as a surgical dressing in amputation surgery.

This report describes the use of clear plastic bags on vinyl chloride-vinylidine chloride copolymer (also known as S Film) for the postoperative management of amputation stumps. The bags are shrunk to the stump by the application of heat and act as a primary dressing. Their use has been effective in controlling postoperative oedema and infection as well as giving the surgeon direct visual access to the stump without disturbing the wound. A case of a below-knee amputation managed by this method is described.

The role of microvascular free flaps in salvaging below-knee amputation stumps: a review of 22 cases.

Twenty-two cases of traumatic below-knee amputation stumps with inadequate soft-tissue coverage salvaged with microvascular free flaps were reviewed retrospectively. All patients would have required an above-knee amputation for prosthesis fitting had microvascular free flaps not bee utilized. A total of 24 flaps were used in 22 patients; parascapular 11 (46%), foot filet six (25%), latissimus dorsi four (17%), lateral thigh, tensor fascia lata, and groin one (4%). Free flaps were performed immediately after injury in five (21%) cases, within the first week in two (8%), between 1 and 3 months i…

[Necrosis of the plantar skin flap in partial foot amputation stumps. A note on the reconstructive technic].

The paper describes one of the complications found in foot amputation stumps: necrosis of the covering skin flap. Technical notes are reported regarding the preparation of the flap and the repair of lesions due to flap necrosis.

Amputation Stump Revision Using a Free Sural Neurocutaneous Perforator Flap.

BACKGROUND: The development of microsurgery and the concept of the surgical flap have enabled orthopedic surgeons to deal with various soft tissue defects and compositions during limb salvage procedures. However, severely damaged bony structures with accompanying soft tissue deficiencies often result in amputation. When performing amputations, surgeons must preserve the stump length to maximize functional ambulation, even during the posttraumatic phase and chronic period when a fillet flap is not an option. PATIENTS AND METHODS: Eleven patients with previous below-knee amputation were enrolle

Regional first web flap used to cover the amputated stump of the thumb.

The regional first web flap is a distally-based flap that is raised from the radiodorsal aspect of the metacarpophalangeal joint of the index finger towards the dorsal first web, and used to cover a thumb amputation. The advantage of this flap over the conventional cross-finger flap is that is gives a thicker and hairless flap with no skin graft to the donor site. Mobilisation of the joints, including the metacarpophalangeal joint of the index finger, can also be maintained during the immobilisation period between two stages.

Turn-up bone flap for lengthening the below-knee amputation stump.

When amputation just below the knee becomes necessary after extensive loss of bone from the tibia and of anterior soft tissue in the treatment of tumours, fractures or infection, the remaining proximal tibia may be too short for a below-knee prosthesis, although the knee may be normal. We have included the distal tibia or foot in a long posterior flap by turning it up thus increasing the length of a very short proximal tibial stump. The knee is thereby saved, allowing satisfactory use of a below-knee prosthesis. This technique is particularly applicable when the distal leg is normal and well v

[Kaposiform angiodermatitis (pseudo-Kaposi's disease) on an amputation stump. A new entity].

Kaposiform cutaneous lesions were observed on the amputation stump of a 65-year-old male whose leg had been amputated above the knee. Histological examination revealed pronounced stasis dermatitis. This finding, together with phlebectasias and fine arborized cutaneous varices on the stump end, suggested that the pathogenesis may involve venous congestion. This may be caused by suction in the prosthesis socket and compression of the stump at the socket edge.

[Problems of peripheral nerve surgery in amputation stump pain and phantom limbs].

Follow-up examination of 17 patients with severe and lasting stump and phantom limb pain which could not be improved by analgetics and who then underwent surgical procedures at the neuroma. The results were discouraging. Only 2 patients showed a lasting improvement. Resection of the neuroma and injection of alcohol into the neuroma seem to be successful only if the preoperative history of pain is very short or if there is an isolated injury of a peripheral nerve and a subsequent single neuroma.

Reconstruction of Chopart's Amputation Stump with the Osteomusculocutaneous Latissimus Dorsi-Rib Flap.

We present an original technique for reconstruction of the right forefoot after Chopart's amputation. The case report was of a 20-year-old man who was a nonsmoker without comorbidities. He was referred to our department because of equinus deformity, pain, and prominence of the cuboid under the skin, leading to chronic ulceration at the stump. We performed flow through free latissimus dorsi flap including the eighth and tenth ribs to reconstruct the stump. Results were assessed at 18 months after operation, showing good stability and normal functional outcome of the foot. The patient is satisfi

Amputation stump perfusion is predictive of post-operative necrotic eschar formation.

BACKGROUND: A large proportion of patients develop poor amputation stump healing. We hypothesize that Laser-Assisted Fluorescent Angiography (LAFA) can predict inadequate tissue perfusion and healing. METHODS: Over an 8-month period we reviewed all patients who underwent lower extremity amputation and LAFA. We evaluated intra-operative LAFA global and segmental stump perfusion, and post-operative modified Bates-Jensen (mBJS) wound healing scores. RESULTS: In 15 patients, amputation stumps with lower global perfusion demonstrated higher mBJS (P = 0.01). Lower suture-line perfusion also correl…

Free fillet foot flap for salvage of below-knee amputation stump.

Management of extensive bone and soft tissue defects, which occur after severe trauma of lower extremities and always lead to an unacceptable amputation in some cases, continues to challenge reconstructive surgeons. When performing lower extremity amputation, preservation of the knee joint has been put into a higher priority. The benefit of below-knee amputation over above-knee ones concentrates on a more normal gait with less energy expenditure during ambulation when a functioning knee joint is present.

[Referred pain from amputation stump trigger points into the phantom limb].

OBJECTIVES: The aim of the study was to examine whether/how myofascial stump trigger points (TPs) after lower leg amputations are able to pr … ith toe projections more distal than those with tibial projections. CONCLUSIONS: Myofascial TPs in amputation stumps are common and able to produce sensations and pain in the phantom limb. Most reported experiences were localized in the toes, as phantom pain usually is. There seems to be a "stump representation" and it seems possible that "referred TP pain" and "p …
