Cardioprotection with volatile anesthetics in cardiac surgery.

Since the proteome of osteoarthritic articular cartilage has been poorly investigated as yet, we adapted proteomic technologies to the study of the proteins secreted or released by fresh human osteoarthritic cartilage in culture. Fresh cartilage explants were obtained from three donors undergoing surgery for knee joint replacement. The explants were dissected out, minced, and incubated in serum-free culture medium. After 48 h, proteins in the medium were identified by two-dimensional or off-gel electrophoresis coupled to tandem mass spectrometry, or by using an antibody-based protein microarra

Comparison of intracarotid anesthetics for EEG silence.

The goal of this study was to compare systemic and cerebrovascular effects of three anesthetic drugs (etomidate, thiopental, and propofol) when delivered by intracarotid and intravenous routes in doses that produce electrocerebral silence (electroencephalography [EEG]). EEG activity, mean arterial pressure (MAP), and laser Doppler flow as a proxy of cerebral blood flow (CBF) of 24 anesthetized New Zealand white rabbits were continuously recorded. Data were compared at three timepoints: baseline, during EEG silence, and after recovery of EEG activity. Drugs were randomly injected via the caroti

Corneal complications following abuse of topical anesthetics.

PURPOSE: To describe corneal complications in three patients following abuse of topical anesthetics. MATERIALS AND METHODS: We describe one case with bilateral corneal perforation, another with unilateral corneal perforation and a third case of chronic keratitis following excessive use of topical anesthetics. RESULTS: Two patients continued to instill topical anesthetic drops despite all recommendations to stop. The result was a bilateral corneal perforation in the first case and a large unilateral descemetocele in the second. The third patient who had chronic toxic keratitis discontinued th…

An update on local anesthetics in dentistry.

Local anesthetics are the most commonly used drugs in dentistry. This article provides a brief update on the pharmacology, adverse effects and clinical applications of these drugs, as well as the role of vasoconstrictors.

[Anesthetics management in patients with epidermolysis bullosa].

The anesthetic management used in a 4-yr-old boy with dystrophic epidermolysis bullosa (DEB) submitted to long-lasting urological surgery is reported. Medical treatment of cutaneous bullae in various stages of healing was undertaken preoperatively. Other preoperative therapies were performed to treat sepsis and to improve poor nutritional state. Different measures were used during monitoring and inhalation induction and maintenance of anesthesia in order to prevent frictional trauma to the skin and oropharyngeal mucosa, leading to blistering. Anesthesia and surgery proceeded uneventfully, whil

Contrasting effects of anesthetics in tadpole bioassays.

The median anesthetic concentrations (AC 50 values) in the bath solution for loss of righting reflex (RR) or nociceptor response (NR) were determined in larval Rana catesbeiana. The AC50 (NR)/AC50(RR) ratios were between 1.6 and 1.9 for ethanol, ether and diazepam. Comparable ratios for hexobarbital and thiopental were 9.8 and 6.2, which indicated a marked lack of "analgesic" effect. The tadpole AC50 values were consistently higher than AC50 values reported for nonionized and unbound drug in mammalian blood water. The tadpole/mammal ratio for loss of NR during thiopental anesthesia was approxi

Pharmacological studies on new potential local anesthetics.

The results of the pharmacological screening of a series of new basic aminoalkyl aryl ethers are reported. The anesthetic effects of these compounds were evaluated by the guinea pig intradermal wheal test. Lidocaine was used as the standard and normal saline as the control. These were, generally, better than or as good as the standard, in terms of potency, onset of action, duration of effect, time of full recovery from complete anesthesia and lack of both apparent local and systemic side effects. The duration of action of their local anesthesia has shown a strong dependence on the substitution

[Cardiotoxicity of local anesthetics: dynamic electrocardiogram study].

Twenty-two cardiopathic patients (mean age 54.4 +/- 9.9 years) underwent dynamic electrocardiography during tooth extraction under local anesthesia (mepivacaine hydrochloride 2% plus adrenalin 1:200.000). Heart rate (p less than 0.001), systolic and diastolic blood pressure (p less than 0.001) significantly increased during tooth extraction returning to values near to basal level in the recovery period. No patients showed significant changes in ST segment. One patient with chronic atrial fibrillation presented an increase in ventricular rate (180 beats/min) and appearance of premature ventricu

[Central toxicity of local anesthetics. New data].

The central nervous toxicity of local anaesthetics has long been thought to be limited to the generalized tonicoclonic convulsions which follow the appearance of such symptoms as sleepiness, tingling of the lips, slurred speech, numbness, etc. Also the central nervous system was considered to be more sensitive than the cardiovascular system. However, recent experimental studies would seem to indicate that at least the more potent local anaesthetics, such as bupivacaine, have deleterious effects on parts of the brain other than the cerebral hemispheres. This may point to an involvement of the c

The postoperative adverse effects of inhalational anesthetics.

Inhalational general anesthetics can contribute to postoperative morbidity (Table II). Postoperative effects of inhalational anesthetics on the central nervous system are speculative. The "toxic" effects of these agents during the postoperative period are most often an extension of their pharmacologic and physiochemical properties. Inhalational anesthetics may produce a number of varied changes in mental status after surgery such as headache, emergence excitement, and delirium. It is very important for health professionals to be aware of the risk of perioperative myocardial infarction in patie…

Densities of liquid anesthetics at 37 C.

Liquid densities of chloroform, diethyl ether, enflurane, halothane, methoxyflurane, and trichloroethylene were measured at 37.0 C. The maximum error in the density values was estimated to be +/-0.01 per cent.

Discussion of sensitivity to preservatives in anesthetics.

A 23-year-old woman with reported sensitivities to several drugs, including anesthetics, requested dental care. Because of the lack of evidence of allergic reactions to lidocaine, it was used in a dental emergency procedure. The patient had an allergic response. In an evaluation by an allergist, an allergy to methyl paraben was detected. The dental practitioner should be aware of possible allergies to preservative agents when a patient has a history of sensitivity to anesthetics. The practitioner's choice of anesthetics and OTC drugs for this type of patient is crucial.

[Anaphylactic reactions observed with anesthetics and adjuvants].

The author reports an inquiry concerning 66 patients who all had an anaphylactoid reaction. The hour of onset, the type of reaction, (skin, bronchial or systemic), their association, the drugs responsible, and the diagnostic difficulties are reviewed. In this respect, in relation to the results of the above inquiry and the data in the literature, the skin and bronchial reactions are described, and the signs of anaphylactic shock recalled. The substances possible are then reviewed with the possible correlation between the type of accident and the drug responsible. Finally the role of macromolec
