Efficacy of apraclonidine 0.5% in the diagnosis of Horner syndrome in pediatric patients under low or high illumination.

PURPOSE: To evaluate the efficacy and safety of apraclonidine 0.5% in the diagnosis of Horner syndrome in pediatric patients. DESIGN: Prospective, interventional case series. METHODS: Ten pediatric patients with a diagnosis of Horner syndrome and 10 age-matched controls with physiologic anisocoria underwent pharmacological testing with apraclonidine. The difference between the pupil diameters of both eyes under low (room light off) and high (room light on) ambient illumination before and one hour after apraclonidine was instilled was recorded. Any adverse effects during the examination or re…

Reversible Mydriasis of Sudden Onset: Think Before You Act.

BACKGROUND: The presence of abrupt anisocoria in clinical examination usually leads to the performance of urgent neuroimaging studies to exclude intracranial hemorrhage, although unilateral mydriasis might be the result of other benign etiologies. CASE REPORT: In this work, we report an illustrative case of a patient presenting with sudden-onset anisocoria while receiving ipratropium bromide nebulization in the emergency department to treat acute asthma. No other abnormalities were found on neurological examination and the computed tomography scan was normal. As a muscarinic antagonist, iprat…

Hemiplegia resulting from acute carbon monoxide poisoning.

Carbon monoxide (CO) poisoning can cause neurological complications such as movement disorders and cognitive impairment through hypoxic brain damage. Although peripheral neuropathy of the lower extremities is a known complication of CO poisoning, hemiplegia is very rare. In our case, a patient who developed left hemiplegia due to acute CO poisoning received early hyperbaric oxygen treatment (HBOT). The patient had left hemiplegia and anisocoria at the beginning of HBOT. Her Glasgow coma score was 8. A total of five sessions of HBOT at 243.2 kPa for 120 minutes were provided. At the end of the …

Improving diagnostic acumen in pupillary evaluation: a review for the primary care physician.

Many primary care physicians perform only a superficial pupillary evaluation as part of the physical examination. The authors review several maneuvers that require no special equipment and help to improve diagnostic yield. The "swinging flashlight" test aids in detecting a relative afferent pupillary defect. When a patient has anisocoria, the initial diagnostic challenge is to determine which pupil is abnormal. Comparing the extent of pupillary inequality under both dim and bright illumination almost always identifies the dysfunctioning pupil(s) and narrows the differential diagnosis. The auth…

Implications of systemic hypotension for the neurological examination in patients with severe head injury.

Thirty-six patients admitted with severe head injury and various degrees of systemic hypotension were studied to determine the effect of hyp … score on the Glascow coma scale was 3 (range 3-8). Although 4 of the 10 cardiac arrest patients had anisocoria, only one (10%) had an intracranial hematoma. Among the seven patients with severe hypotension, only two had anisocoria and neither had and intracranial hematoma; one patient in this group (14%) had a hematoma that was diagnosed at autopsy. Pupillary Responses and Vital Signs in Hypoglycemic Patients with Impaired Consciousness During Prehospital Care: A Retrospective Observational Study.

Background/Objectives: Impaired consciousness has various causes. One such cause includes hypoglycemia, which may be symptomatic or asymptom … . Miosis and abnormal pupillary light reflexes were observed in 68% and 84% of cases, respectively. Anisocoria occurred in 7.6% of the patients. After glucose administration, both abnormal reflexes and anisocoria significantly decreased (both p < 0.0001). Although vital signs did not consistently reflect autonomic responses, changes in pupillary findings wer …

Pupillary abnormalities in non-selected critically ill patients: an observational study.

BACKGROUND: Repeated pupillary examination is a key element of neurologic surveillance in intensive care units (ICU). However, in non-select … patients with at least one pupillary examination were included. PA was defined as areflexia and/or anisocoria present at the time of ICU admission or occurring during the ICU stay. RESULTS: During the study period, we included 297 patients who had 6±9 pupillary examinations per day (totaling 11,360 pupillary assessments). The majority of patients (n=161, 5 …

Unilateral mydriasis in a child with a ventriculoperitoneal shunt for obstructive hydrocephalus: a diagnostic dilemma.

There are several causes for sudden onset unilateral mydriasis, however impending transtentorial uncal herniation needs to be ruled out. Thi … rgent CT scan of the brain did not show any new intracranial abnormality. A case of pharmacological anisocoria was diagnosed that resolved completely within 24 hours of discontinuation of ipratropium bromide. Although ipratropium-induced anisocoria has been reported in children, but to our knowledge none in a child with VP shunt for hydrocephalus. This empha …

Pupillary dilation lag is intermittently present in patients with a stable oculosympathetic defect (Horner syndrome).

PURPOSE: To examine the repeatability of detecting pupillary dilation lag in patients with Horner syndrome. DESIGN: Retrospective interventional study. SETTING: Single referral institution. PATIENT POPULATION: Fifteen patients with unilateral Horner syndrome and 16 subjects with physiologic anisocoria. Intervention procedure: Each subject underwent four pupillometric recordings in darkness. The asymmetry of pupillodilation between the two eyes was calculated as the change in anisocoria between five seconds and 15 seconds in darkness. Pupillary dilation lag was considered present if the asym…
