Advances in the Treatment of Anorexia Nervosa and Bulimia Nervosa.

During the last decade, much investigation into possible pharmacotherapy for eating disorders has been undertaken, especially for bulimia nervosa.Intensive hospital treatment compromising a combination of individual, group and family therapies with or without adjunctive pharmacological treatments are usually offered in anorexia nervosa. Osteoporosis and delayed gastric emptying are 2 medical complications that should be addressed as early as possible in the course of the disorder. Although not yet confirmed in controlled clinical trials, there is preliminary support for the use of hormone repl…

Melatonin disturbances in anorexia nervosa and bulimia nervosa.

The pineal gland releases melatonin into the blood stream in response to sympathetic noradrenergic stimulation of pinealocytes. This process … levels of melatonin also occur in depression. In this review, studies of melatonin in patients with anorexia nervosa and bulimia nervosa are considered in relation to potential abnormalities of noradrenergic function and circadian rhythm. The influence of weight loss, binging and purging, and depression on melatonin is discussed. Other studies involving the assess …

Psychopharmacology in the treatment of anorexia nervosa and bulimia nervosa.

Anorexia nervosa and bulimia nervosa remain enigmatic disorders with poorly understood etiologies. Clinicians continue to find these disorders very challenging to treat. As their underlying pathophysiology is clarified, it is hoped that specific pharmacologic treatments will be developed to alleviate the pain and disability these disorders produce.

Anesthesia considerations for anorexia nervosa and bulimia nervosa.

Anorexia nervosa and bulimia are appearing with increasing frequency in preadolescents and adolescents. Eating disorders lead to significant physical changes related to starvation and/or purging. These patients should be specifically questioned about taking laxatives, diuretics, or amphetamines to lose weight. Subclinical abnormalities, coupled with the patient's denial, emphasize the need for careful physical examination and laboratory assessment. Preexisting electrolyte, cardiovascular, and thermoregulatory alterations must be recognized so that an appropriate anesthesia plan can follow.

Bulimia nervosa with and without a history of anorexia nervosa.

This study compared two subgroups of women of normal weight with bulimia nervosa: those with a history of anorexia nervosa and those without such a history. Those with a history of anorexia nervosa indicated a desired body weight significantly lower than those without the history. Those with a history of anorexia nervosa were more likely to abuse laxatives and less likely to self-induce vomiting. Both groups reported feeling fat and worrying a great deal about weight and shape issues.

Personality disorders in anorexia nervosa and bulimia nervosa.

Two self-report questionnaires (MCMI and BSI) designed to measure personality disorder (PD) according to DSM-III (R) criteria were administered to patients with a diagnosis of anorexia nervosa (AN) (n = 19), bulimia nervosa (BN) (n = 16), or both diagnoses (AN + BN) (n = 9), both before and after treatment for the eating disorder. The main finding was that self-reported Personality Disorder (PD) diagnoses are not stable enduring characteristics among this group of eating disorder patients. A high rate of PD diagnoses occurred in all patient groups at admission (93%) and at discharge (79%). Bot…

Bulimia nervosa: an ominous variant of anorexia nervosa.

Thirty patients were selected for a prospective study according to two criteria: (i) an irresistible urge to overeat (bulimia nervosa), followed by self-induced vomiting or purging; (ii) a morbid fear of becoming fat. The majority of the patients had a previous history of true or cryptic anorexia nervosa. Self-induced vomiting and purging are secondary devices used by the patients to counteract the effects of overeating and prevent a gain in weight. These devices are dangerous for they are habit-forming and lead to potassium loss and other physical complications. In common with true anorexia n…

Considering Sufficient Weight Loss as a Prerequisite for Development of Anorexia Nervosa and Atypical Anorexia Nervosa.

BACKGROUND: Because premorbid BMI strongly predicts the amount of weight loss in anorexia nervosa (AN)/atypical AN, we hypothesize that weight loss triggers both disorders by inducing the endocrine adaptation to starvation. METHOD: We propose research to capture the onset of AN/atypical AN following intentional, otherwise behaviorally motivated, or unintentional weight loss in relationship to premorbid weight. RESULTS: We propose retrospective and prospective studies to examine the temporal development of symptoms in AN/atypical AN. Given a greater frequency of weight loss behaviors in indiv…

Food choice and neural reward systems in adolescents with anorexia nervosa and atypical anorexia nervosa.

BACKGROUND: Adolescence is a critical developmental period for the study of anorexia nervosa (AN), an illness characterized by extreme restriction of food intake. The maturation of the reward system during adolescence combined with recent neurobiological models of AN led to the hypothesis that early on in illness, restrictive food choices would be associated with activity in nucleus accumbens reward regions, rather than caudate regions identified among adults with AN. METHODS: Healthy adolescents (HC, n = 41) and adolescents with AN or atypical AN (atypAN, n = 76) completed a Food Choice Task…

The role of body weight in college students' perceptions of anorexia nervosa and atypical anorexia nervosa.

Objective: The present experimental study assessed college students' ability to recognize an eating disorder across various body weights. Participants: Undergraduate students (N = 428) from a midsized public university. Methods: Participants completed an online questionnaire and were randomized to read one of four vignettes describing a woman with anorexia nervosa symptoms who was underweight, average weight, overweight, or obese. Participants completed a survey to assess body image, eating concerns, and ability to recognize an eating disorder. Results: Results showed participants in the under…

Relational attitudes in adolescent girls with and without a diagnosis of anorexia nervosa or atypical anorexia nervosa.

BACKGROUND: People with eating disorders experience interpersonal difficulties, but little research explores relational attitudes in this population. We examined sense of relational entitlement towards parents, pathological concern, and psychological distress in adolescent girls with and without anorexia nervosa (AN) or atypical anorexia nervosa (AAN). METHODS: Questionnaires assessing sense of entitlement towards parents, pathological concern, and symptoms of depression and anxiety were completed by 85 girls with and 100 girls without AN/AAN (mean age 15.06 ± 1.41). The AN/AAN group also com…

[Compulsive exercise in anorexia nervosa].

Anorexia nervosa is the most deadly mental disorder. Early detection is crucial. Most patients engage in compulsive exercise, often in an early stage. Compulsive exercise has a deleterious impact on the course of the disease. It can manifest in extreme forms with rigid rules and an inevitable urge. Three different subtypes exist: intensive sports, increased movement in everyday life, and motor restlessness. Besides the underlining motives, neurobiological mechanisms play a role. Moreover, cold ambient temperature increases compulsive exercise, while many patients intentionally expose themselve…

[Anorexia nervosa, microbiota and brain].

Anorexia nervosa (AN) is a psychiatric disease with a high prevalence and comorbidities, characterized by a low response rate to treatment. It is considered as a multifactorial disease. In recent years, the focus has been placed on the presence of intestinal dysbiosis and its possible involvement as a causal factor as well as an alternative treatment. The objective of this work has been to review the current state of knowledge of alterations in the intestinal microbiota identified in patients with AN and the possibility of using probiotics as a therapeutic alternative. Significant changes in t…

Interoceptive awareness in anorexia nervosa.

Interoceptive awareness - the sense and awareness of the internal state of our bodies - has been of increasing interest in anorexia nervosa (AN) given the observation that people with AN do not respond appropriately to hunger cues. Despite the interest in the area, very little research has been undertaken to specifically assess interoceptive awareness in AN. The aim of this study was to explore levels of interoceptive awareness in individuals at different stages of AN, as well as first-degree relatives. Eighty participants were compared on self-reported interoceptive awareness using the Multid…

[Body care in anorexia nervosa].

Therapeutic touch body care in the management of eating disorders, particularly anorexia nervosa, aims to deepen the relationship of trust and the therapeutic alliance. Nursing research and the development of clinical nursing care, with nursing diagnoses as a support, have made it possible to provide new responses adapted to the patient's needs. This reassuring helping relationship favors the work around the patient's psychic and physical perceptions. The notion of the gender of the caregiver in the body approach care is a dimension to be taken into consideration.

Characterizing Treatment-Resistant Anorexia Nervosa.

Background: The issue of treatment resistance in eating disorder care is controversial. Prior research has identified multiple failed treatment attempts as a common criterion for severe and enduring anorexia nervosa, but little is known about patients who have multiple failed treatment attempts. This study was designed to compare the clinical and demographic characteristics of eating disorder patients with multiple, incomplete inpatient admissions to those with good outcomes. Understanding if these patient populations differ at initial admissions has implications for the prediction and charact…
