[Surgery of proctologic diseases in the polyclinic].

The incidence of proctological diseases is on a rising trend, and some of them require surgical treatment. Outpatient handling is indicated, following thorough diagnosis, and in common practice worldwide. The surgical specialist, in command of pain-killing methods, is in a position to apply surgical treatment on a outpatient basis. Careful attention has to be given, in this context, to the anal complex and anal sanitation. Perianal thrombosis, incarcerated prolapsed haemorrhoids, acute anal fissure, and, with some reservation, periproctic abscess are situations in which acute action is recomme

Chemoradiotherapy for squamous cell cancer of the anal canal: a systematic review.

Squamous cell cancer of the anal canal is a rare tumour for which there remains uncertainty regarding optimal therapy. A systematic review was conducted to summarise the evidence examining concurrent chemotherapy and radiotherapy or different chemotherapy regimens in combination with radiotherapy. MEDLINE, EMBASE and conference proceedings were searched for relevant randomised controlled trials. Outcomes of interest were colostomy rate, local failure, overall survival, disease-free survival, adverse effects and quality of life. Six randomised controlled trials were identified. Two trials repor

Anorectal Malignancies Presenting as a Perianal Abscess or Fistula.

BACKGROUND: Acute anorectal abscess and fistula are common conditions that usually presents as a painful lump close to the anal margin. Tumors in the distal rectum and in the perianal region may mimic the symptoms and signs of anorectal sepsis, thereby leading to a delay in diagnosis and management. The purpose of this study was to describe patients presenting with acute perianal abscess or fistula who were subsequently diagnosed with anorectal cancer. METHODS: We performed a retrospective, review of all cases presenting with acute perianal abscess or fistula who were subsequently found to ha

Perianal Mucinous Adenocarcinoma: A Case Report and a Systematic Review of the Literature.

BACKGROUND: Mucinous adenocarcinoma of anus is an uncommon neoplasm of the gastrointestinal tract. In most of the cases, early diagnosis of this disease is difficult since its symptoms frequently mimic benign inflammatory conditions. METHODS: A systematic PubMed and Scopus search was conducted, a propos of a case report. RESULTS: One hundred fifty patients from 93 case reports were included. The mean age of the patients was 60.5 years (range: 18-81). The majority of them were males (124 out of 150, 82.7%), while the main known risk factor was the history of chronic fistula (109 out of 150, 7

Clinical Inquiry: Does qHPV vaccine prevent anal intraepithelial neoplasia and condylomata in men?

Yes. Quadrivalent human papillomavirus (qHPV) vaccine reduces rates of anal intraepithelial neoplasia (AIN) by 50% to 54%, and persistent anal infection by 59%, associated with the 4 types of HPV in the vaccine (6, 11, 16, and 18) in young men who have sex with men (MSM); it also reduces external genital lesions by 66%, and persistent HPV infection associated with the same 4 HPV types by 48 to 59% in all young men, heterosexual men, and MSM.

Condyloma acuminatum with superficial spirochetosis simulating condyloma latum.

We report a case of a 12-year-old boy with a large perianal condyloma. The lesion was found to contain both spirochetes (Warthin-Starry stain) and human papillomavirus types 6 or 11 (in situ hybridization). Because of persistent negative serologies for syphilis and lack of other clinical stigmata of syphilis, the lesion was diagnosed as condyloma acuminatum with secondary involvement by spirochetes of probable intestinal origin. The case demonstrates the potential histologic similarities between condyloma latum and condyloma acuminatum, as well as the pitfalls of using the Warthin-Starry stain

Colorectal malignancies in HIV-positive patients.

Due to the development of more effective medications, those infected with HIV are living longer. Consequently, more tumors and infections have been added to the AIDS-defining criteria in the last decade. Our aim was to review the occurrence and clinical course of colorectal (CR) malignancies in HIV infected/AIDS patients from a single institution. A retrospective review of HIV/AIDS patients with colorectal malignant tumors was undertaken. We included adult patients, with ELISA and Western blot test positive for HIV, and primary malignant tumors located in the colon or rectum. Malignant neoplas

Tumours and inflammatory lesions of the anal canal and perianal skin revisited: an update and practical approach.

Tumours of the anal and perianal region are relatively rare, and clinically often interpreted as innocuous lesions, leading to frequent delays in diagnosis and adequate treatment. Although squamous cell neoplasia represents the most common entity encountered in this anatomically complex area, many conditions, both neoplastic and inflammatory, may occur. Adding to the challenge of correct diagnosis and patient management, recent years have seen major updates in the terminology of squamous cell neoplasia, created to reflect advances in our understanding of the role of human papilloma virus and u

Malignant neoplasia of the colon, rectum and anus at the King Faisal Specialist Hospital and Research Centre.

Malignant neoplasms of the colon, rectum, and anus are relatively uncommon in the Kingdom of Saudi Arabia. This paper documents the King Faisal Specialist Hospital and Research Centre tumor registry colorectal cancer data base. Between June 1975 and December 1989, 622 patients were registered with malignant colorectal and anal tumors. There were 383 males and 239 females. The average ages were 53.5 years and 47.8 years. The majority of patients were Saudi. The single most common site for large bowel malignancy was the rectum. There were 455 simple adenocarcinomata, 39 mucinoud adenocarcinomata

Imiquimod treatment of vulvar melanoma in situ invading the urethra.

The primary treatment of both in situ and invasive vulvar melanoma is wide local excision of the primary neoplasm. However, this can be a surgical challenge for size, multifocal presentation with proximity to urethra or anus and tendency for local recurrence. The data on adjuvant therapy for vulvar MIS is very limited. A 69-year-old patient with melanoma of the vulva underwent a simple vulvectomy with positive margins in peri-clitoral area, followed by modified radical vulvectomy and bilateral inguinofemoral sentinel lymph node dissection with negative margins. She was later diagnosed with MIS

Prepubertal vaginal bleeding caused by a vaginal foreign body: Diagnostic challenges and forensic implications.

INTRODUCTION: Vaginal bleeding in prepubertal girls merits thorough investigation, as it is always pathological and its etiology is broad and includes inflammatory conditions, endocrine disorders, local irritation from a foreign body, neoplasms, injuries, and sexual abuse. The diagnostic approach used should be immediate, systematic, and multidisciplinary. CASE: We present the case of a 7-year-old girl who was admitted to a tertiary pediatric hospital with a history of prolonged vaginal bleeding. During the clinical, pediatric, and forensic investigation, the presence of a foreign body within

Anal Adenocarcinoma: A Rare Malignancy in Need of Multidisciplinary Management.

Adenocarcinoma of the anal canal is an uncommon malignancy, making it challenging to perform randomized controlled clinical trials to define best practices in care. For patients with localized disease, there remains a lack of consensus regarding the optimal management, with some physicians advocating for trimodality therapy (similar to the locally advanced rectal adenocarcinoma paradigm) and others advocating for definitive radiation therapy with concurrent chemotherapy (similar to the management of anal squamous cell carcinoma). The objective of this clinical review is to describe the managem

Primary anorectal malignant melanoma: A clinical, radiology and pathology correlation.

INTRODUCTION: Primary gastrointestinal melanomas are mucosal malignancies that arise from melanocytes in the oropharynx, rectum, and anus. Anorectal malignant melanoma (ARMM) are exceedingly rare, accounting for less than 1% of all melanomas, 0.1% of all rectal malignancies and 4% of anal malignancies. Diagnosis is frequently delayed as these lesions are often mistaken for haemorrhoids. Histological evaluation with special immunohistochemical stains is often necessary for definitive diagnosis. Due to the aggressive nature, 61% of patients with ARMM would already have lymph node involvement or

Presentation of a Large and Debilitating Fungating Anogenital Lesion.

CASE REPORT: A 65-year-old Caucasian man presented with a debilitating anogenital lesion. DIFFERENTIAL DIAGNOSIS: The differential diagnosis of anogenital lesions includes infectious (syphilis, herpes simplex virus), noninfectious (hidradenitis suppuritiva, lymphedema), benign (condyloma acuminata), and malignant pathologies (squamous cell carcinoma, Kaposi sarcoma). DIAGNOSTIC ASSESSMENT, MANAGEMENT, AND OUTCOME: Biopsy of an anogenital lesion will determine any oncologic potential. Further imaging can better characterize the disease. Once in the operating room, oncologic principles should
