Syringomyelia and arachnoid cysts associated with spinal arachnoiditis following subarachnoid hemorrhage.

A 66-year-old woman with primary Sjogren syndrome developed syringomyelia following two episodes of subarachnoid hemorrhage (SAH) due to the … inished size of the syrinxes. We should recognize syringomyelia and arachnoid cysts due to adhesive arachnoiditis as a late complication of SAH. Microlysis of the adhesions focusing on the lesion thought to be the cause of the symptoms is one of the choices to treat massive syringomyelia and arachnoid cysts associated with arachnoiditis following SAH.

Hydrocephalus and arachnoid cysts.

AIM: The management of arachnoid cysts (AC) remains controversial. An additional problem derives from the management of hydrocephalus associated with an AC. In this work, we discuss existing procedures proposed in the current literature for their treatment. METHODS: We reviewed selected reports on intracranial ACs placing special interest in those about the association of hydrocephalus and ACs. We also briefly surveyed data of our patients with this association. RESULTS AND DISCUSSION: Hydrocephalus is often found in midline and posterior fossa ACs. Interhemispheric lesions may also evolve w

Intrasellar arachnoid cysts.

OBJECTIVE: To evaluate the clinical, endocrinological, and radiological presentation of nine cases of surgically verified intrasellar arachnoid cysts and to discuss the physiopathological mechanisms of formation of these cysts. METHODS: Among 1540 patients presenting with pituitary lesions, nine presented with an intrasellar arachnoid cyst. Their charts were retrospectively reviewed. RESULTS: Presenting symptoms included headache (n = 2), visual symptoms (n = 3), menstrual irregularities (n = 2), rapid weight gain (n = 1), vertigo (n = 1), and/or confusion (n = 1). Two cysts were discovered

Retroclival arachnoid cyst.

Retroclival arachnoid cyst is a rare mass lesion, with only seven cases previously reported in literature. MR is the imaging modality of choice in its evaluation and in its differentiation from epidermoid cyst. The case reported here was surgically proved and had an uncharacteristic MR signal intensity that was higher than that of cerebrospinal fluid due to previous hemorrhage.

The Challenge of Treating Syringomyelia Associated with Extensive Arachnoiditis.

Background/Objectives: Extensive arachnoiditis associated with syringomyelia is a complex and multifactorial disease. This narrative review aims to synthesize current evidence on the terminology, pathogenesis, diagnostic evaluation, conservative and surgical management, and experimental approaches to arachnoiditis-related syringomyelia, with particular emphasis on the factors that limit durable treatment in extensive disease. Methods: PubMed was searched for English-language publications from January 2016 through August 2026 using the primary search terms "syringomyelia AND arachnoiditis," sup…

Adhesive arachnoiditis: a rare cause of postoperative urinary retention.

Postoperative urinary retention is a common consequence of pelvic surgeries. It is often related both to the nature of surgery and the medication used in the perioperative period. Adhesive arachnoiditis (AA) is a rare condition, which has various clinical presentations ranging from back pain to disabling neurological deficits. Numerous aetiologies for AA have been described in the literature, one such noted is spinal anaesthesia. We report a patient who presented with isolated bladder dysfunction in the form of urinary retention after spinal anaesthesia. She was evaluated and diagnosed with AA…

CNS cryptococcosis presenting with cerebellar stroke and spinal arachnoiditis.

A 67-year-old man presented with a low-grade fever for 2 months, weakness of all four limbs for five days and altered sensorium for two days. He was recently diagnosed with AIDS and was treatment-naive. Investigations revealed a CD4 count of 27cells/mm3 MRI brain and spine exhibited bilateral cerebellar lesions with diffusion restriction, and severe arachnoiditis at the level of the lumbar spine. High suspicion of central nervous system tuberculosis in an endemic country like ours, led us to start antitubercular therapy and steroids. Repeated lumbar punctures resulted in a dry tap leading to a…

Fibromyalgia and arachnoiditis presented as an acute spinal disorder.

BACKGROUND: Adhesive arachnoiditis is a chronic, insidious condition that causes debilitating intractable pain and a range of other neurological problems. Its pathophysiology is not well understood. This manuscript discusses its presentations, which can mimic an acute spinal disorder, its hypothetical pathophysiology, treatment, and its relationship with fibromyalgia. CASE DESCRIPTION: The authors present a case of a 47-year-old female who presented with clinical features mimicking an acute spinal disorder but later found to have an adhesive arachnoiditis. She was admitted following a trauma …

Delayed Optochiasmal Arachnoiditis following Intervention for a Subarachnoid Haemorrhage.

Visual morbidity following a subarachnoid haemorrhage is common among survivors and it is usually the result of vascular ischaemia at the time of the initial insult. In this report, we present an unusual case of delayed, progressive visual loss following intervention for a ruptured anterior communicating artery aneurysm. We discuss the possible etiological factors and highlight potential diagnostic pitfalls.

[Arachnoiditis and tuberculuos cortical encephalitis in an HIV+ patient].

HIV infection is a major risk factor for tuberculosis. We describe the case of a 30-year-old male presenting with headache, compromised mental status, seizures, neck stiffness and fever that was subsequently diagnosed with HlV and neuroinfection. Clinical data, cerebrospinal fluid and brain imaging supported a diagnosis of neurotuberculosis. Cranial magnetic resonance imaging showed diffuse arachnoidal enhancement, mainly at the basal cisterns and cortical encephalitis. Such imaging findings play a key role in the diagnosis of central nervous system tuberculosis.

Spinal arachnoiditis ossificans. Observations on its investigation and treatment.

The histories of two patients with arachnoiditis ossificans leading to severe neurological impairment are presented. Analysis of these cases highlights the importance of three-dimensional (3 D) computed tomography (CT) scanning for the establishment of the diagnosis and in postoperative assessment of this rare disease, whereas magnetic resonance imaging (MRI) is less useful. The neurological function in both cases improved following re-exploration operations in which the whole length of the dural sac covering the intradural ossific lesions was freed totally from external obstructions imposed b…

Arachnoid cysts in paediatrics.

PURPOSE OF REVIEW: Arachnoid cysts are incidentally found on neuroimaging at an increasing rate in the paediatric population. Hence, learning their clinical implications to better inform families is important. The purpose of this review is to outline the current understanding of epidemiology, disease and clinical features based on location, imaging and management of arachnoid cysts in paediatrics. RECENT FINDINGS: The prevalence of arachnoid cysts is 2.6% in paediatrics, with greater than 90% being asymptomatic. When they do cause symptoms, it is based primarily on their location and size. Th
