Initial psychometric evaluation of the Arm Activity Measure (ArmA): a measure of activity in the hemiparetic arm.

OBJECTIVE: To evaluate the psychometric properties of the Arm Activity Measure (ArmA), a patient-reported measure of active and passive function in the paretic upper limb. DESIGN: Psychometric evaluation study. SETTING: Two specialist rehabilitation and spasticity management services. METHOD: Patients (n = 92) with upper limb paresis were recruited from two specialist neurorehabilitation centres. Mean age 44.5 (SD 16.7). Diagnostic distribution: stroke 48 (52%); other brain injury 28 (31%); or other neurological condition 16 (17%). Evaluation of convergent and divergent validity; unidimensi…

[Arm Swelling with a Camouflaged Cause].

Arm Swelling with a Camouflaged Cause Abstract. We report on a 72-year-old patient who presented to the emergency department due to a proximal multiple-fragment humeral fracture on the left. Despite correct therapeutic approaches, there was no improvement in the left arm swelling. Further investigations showed a provoked arm vein thrombosis on the left with a post-humeral fracture on the left. After therapeutic anticoagulation, the swelling regressed immediately, but with persistent lymphedema of the left hand.

Kimura's disease of the left arm.

Kimura's disease is a rare, benign entity that causes subcutaneous angioblastic lymphoid hyperplasia with eosinophilia. It usually presents with subcutaneous lymphoid swellings with regional lymphadenopathy and salivary gland masses. Kimura's disease is frequently associated with renal involvement, which includes proteinuria and nephrotic syndrome as the most common presentations. This report presents a case of a man in his early 20s with a swelling in the medial aspect of the distal left arm with two previous episodes of nephrotic syndrome. Multiple enlarged axillary lymph nodes and epitrochl…

Long-Term Outcomes of Arm Replantation.

BACKGROUND: Advances in microsurgery have improved the survival rate of arm replantation. However, the functional outcome of the replanted arms remains unpredictable. The authors performed a systematic review to evaluate the long-term outcome of arm replantation and the secondary reconstructive procedures after replantation. METHODS: All available English literature in the PubMed, Embase, and CENTRAL databases was searched for articles reporting functional outcome of successful arm replantation with follow-up period more than 1 yr. Clinical data were extracted including study characteristics,…

Hand, Wrist, Forearm, and Arm Replantation.

Level of injury plays a significant role in decisions to perform replantation, with improved function, nerve regeneration, and decreased rate of reperfusion injury for injuries at the distal forearm or wrist compared with proximal injuries. The principles of a functional and sensate outcome dictate replantable parts, whereas patient comorbidity, expectations, and safety dictate patient candidacy. Vascular grafts are an expected part of the operation, and the contralateral arm or a lower extremity should be prepped into the surgical field. Despite diminished function, patient satisfaction and i…

New developments in prosthetic arm systems.

Absence of an upper limb leads to severe impairments in everyday life, which can further influence the social and mental state. For these reasons, early developments in cosmetic and body-driven prostheses date some centuries ago, and they have been evolving ever since. Following the end of the Second World War, rapid developments in technology resulted in powered myoelectric hand prosthetics. In the years to come, these devices were common on the market, though they still suffered high user abandonment rates. The reasons for rejection were trifold - insufficient functionality of the hardware,

Mini C-arm: faster, cheaper, safer?

BACKGROUND: Mini C-arm image intensifiers (IIs) are promoted to permit lower radiation dose than traditional IIs with a lower purchase price and without the need for a radiographer, saving time. In real-world usage, radiation dose is not always lower. METHODS: A retrospective review of prospectively collected data for 620 children undergoing forearm fracture reduction in theatre was undertaken. Imaging was performed with the Fluoroscan mini C-arm or a comparison traditional II. Radiation dose and theatre time were recorded. RESULTS: There was no significant difference in radiation dose as me…

Intramedullary angioma with bilateral arm hypothermia.

Spinal cavernous malformations are collections of abnormal blood vessels in the spinal cord. They are rare and frequently accompany cranial cavernous angiomas. They exhibit clinical features representing the region of the spine affected by the cavernous malformation. We present a 12-year-old boy with bilateral hypothermia predominantly in the left arm and motor weakness of the upper extremities, and lesser involvement of the lower extremities The case had normal cranial magnetic resonance imaging, but MRI of cervical region revealed an intramedullary cavernous haemangioma confirmed with histop…

[Arm injuries due to sport climbing].

There are 26 large climbing centres and 44 smaller indoor or outdoor climbing walls in The Netherlands at this time. Hand and finger injuries are the most common types of sport climbing injuries. Most injuries are caused by overstraining. Injuries include avulsion fractures, arthrosis, pulley rupture, damage to joint capsule and collateral ligaments, 'gamekeeper's thumb', 'climber's finger', lumbrical shift syndrome, 'climber's elbow', shoulder injuries and nerve compression syndromes. Treatment is usually conservative. Depending on the extent of damage surgical intervention may be indicated.

Hand-arm vibration exposure of dentists.

OBJECTIVES: The use of dental handpieces exposes the dental personnel to high-frequency vibration. Dentists have been shown to have a high frequency of finger-related and other upper limb symptoms and a high prevalence of osteoarthrosis in the distal interphalangeal joints. METHODS: The vibration of 22 dental handpieces was measured during normal work with accelerometer. The weighted vibration according to the standard ISO 5349-1 and the total acceleration of high frequency vibration ("ultravibration") in the frequency range of 1.6-10 kHz were analyzed. In order to compare non-contact vibrati

Associated anomalies with anorectal malformation (ARM).

OBJECTIVE: To find the prevalence of associated anomalies in children with anorectal malformation (ARM). METHODS: One hundred and forty patients (80 males and 60 females) with expand were studied to detect associated anomalies and to find their prevalence. High and low type of ARM was seen in 52.14% and 47.86% of patients respectively. Associated anomalies were more common with high type of ARM (78.08%) than in patients with low type of ARM (37.31%). 58.57% patients had associated anomalies which included those of urinary system (37.14%), vertebral system (34.28%), skeletal system other than …

Ambulatory blood pressure monitoring: which arm?

To determine the effects of routinely selecting the nondominant arm for ambulatory blood pressure monitoring (ABPM) on estimates of patients' blood pressure (BP) and to evaluate the practise of using manual BP from one arm and ambulatory BP from the other on the estimation of white coat effect (WCE), an observational study was conducted in 10 volunteers, exhibiting an interarm resting clinic systolic BP (SBP) difference > or =10 mm Hg. The main outcome measures were: (i) average ambulatory SBP measured on right and left arm simultaneously during 24 h, and (ii) estimate of WCE derived, by curre…

Above-elbow arm replantation during pregnancy.

We report a successful replantation of an above-elbow amputated left arm in a 26-year-old female with a good, functional, long-term result. The patient was at 18 weeks estimated gestational age at the time of surgery. Major upper extremity replantation during pregnancy has not been previously described in the literature. Current operative and perioperative aspects, including considerations that favored microsurgical replantation in this unusual case, are discussed.
