Reactive arthritis.

Reactive arthritis is one of the spondyloarthropathy family of clinical syndromes. The clinical features are those shared by other members of the spondyloarthritis family, though it is distinguished by a clear relationship with a precipitating infection. Susceptibility to reactive arthritis is closely linked with the class 1 HLA allele B27; it is likely that all sub-types pre-dispose to this condition. The link between HLA B27 and infection is mirrored by the development of arthritis in HLA B27-transgenic rats. In this model, arthritis does not develop in animals maintained in a germ-free envi…

Psoriatic arthritis.

Psoriatic arthritis (PsA) is an inflammatory arthritis associated with psoriasis. It is classified with the human leukocyte antigen B27 seronegative spondyloarthropathies because it shares certain features with other conditions included in that group. Specific features of PsA, however, distinguish it from both rheumatoid arthritis and other spondyloarthropathies. Although the cause and pathogenesis of PsA are not known, genetic, immunologic, and environmental factors are thought to play a role, and provide a rationale from therapeutic modalities.

Psoriatic arthritis.

Recent population-based studies have examined the incidence, prevalence, and survival rates of patients with psoriatic arthritis (PsA). Although there are still no completely satisfactory diagnostic criteria for PsA, Moll and Wright's criteria--although not a true classification/diagnostic scheme--are the most used. The sensitivity of these criteria is low (61%), as are the sensitivities of the European Spondyloarthropathy Study Group and Amor classification criteria for the whole spectrum of spondyloarthropathy. In some patients, PsA can occur only with peripheral enthesitis, particularly Ach…

Gonococcal arthritis.

Disseminated gonococcal infection is the most common systemic complication of acute gonorrhea and occurs in 0.5% to 3.0% of patients with untreated mucosal infection. It is also the most common cause of septic arthritis in patients less than 30 years of age. Fortunately, the incidence of gonorrhea is decreasing dramatically in the United States and Western Europe, although it is still high in developing countries. Increasing resistance to antibiotics requires continuous surveillance of antimicrobial susceptibilities to determine the efficacy of current therapeutic measures.

Viral arthritis.

Viral infections are important in rheumatic disease not only because of the acute and subacute syndromes they cause but also because of their potential importance as etiologic factors in common chronic diseases such as rheumatoid arthritis. During 1996, the clinical spectrum of the acute polyarthritis caused by parvovirus B19 was further delineated and was shown to include carpal tunnel syndrome, hepatic dysfunction, and possibly angioedema. B19 infection can be studied using the salivary antibody response. No convincing additional data were reported regarding B19 in chronic syndromes such as …

Psoriatic arthritis.

Psoriatic arthritis and its associated enthesopathy appear to be another immune-mediated manifestation of the genetic predisposition to develop psoriasis. The occurrence of psoriatic arthritis in the setting of the selective immune deficiency of advanced HIV infection, especially in the association with HLA-B27, points to a role of the CD8 T cell in disease pathogenesis. The relationship of Reiter's syndrome to psoriatic arthritis is emphasized.

[Septic arthritis].

Septic arthritis (SA) is reviewed in the English literature with reference to registration of localisation of the infected joints, the bacteriological cause, predisposing factors, clinical information and laboratory data related to SA. Seventy-eight publications were available concerning 975 patients with 1086 infected joints caused by 1032 bacteria. Large joints such as the knee and hip were most frequently, however, infected, in recent years, SA in small axial joints has been reported with increased frequency. Gram-positive cocci and especially Staphylococcus aureus are most frequently the c…

[Reactive arthritis].

In most cases, reactive arthritis occurs after urogenital and gastrointestinal infections. Anamnesis, clinical picture, antigen HLA-B27 determination and serologic antibody ascertainment are of essential importance to the diagnosis. The therapeutic value of antibiotics is questionable, owing above all to the fact that in chlamydia-induced arthritis the causative agent was found in the intraarticular biopsy specimen. The prognosis of the disease is favourable although relapses are frequent. Especially Reiter's disease may have a chronic course.

Bacterial arthritis.

The 1991 literature on septic arthritis included a concise review of adult septic arthritis, examples of pseudoseptic arthritis, and two interesting animal studies. One animal study examined the induction of acute synovitis by the intra-articular injection of bacterial endotoxin and the cytokines tumor necrosis factor-alpha, and interleukin-1 beta; and the other studied the effects of early and delayed synovectomy in the management of septic arthritis. The predispositions to septic arthritis can be divided into local joint abnormalities, systemic factors, or both. Examples of the local joint a…

AIM: Description of a clinical case of enteropathic arthritis. DESIGN: Descriptive study. SITE. Health Centre. PATIENTS AND OTHER PARTICIPANTS: A clinical case is described, starting with back pain where X-rays perceive no irregularity. Later a diagnosis of Crohn's disease is made. Finally lesions characteristic of anchylo-poyethic spondylitis appear. INTERVENTIONS: Corticosteroid treatment. MEASUREMENTS AND MAIN FINDINGS: Clinical investigations and complementary diagnoses. CONCLUSIONS: Way in which spondylitis presents, with no clear X-ray indications. Importance of closely following th…

Sporotrichal arthritis.

Sporotrichal arthritis is a rare disease entity. Diagnosis is often difficult and delayed. Presentation may be either monoarticular or polyarticular. A case of polyarticular sporotrichal arthritis which exemplifies these problems is reported.

[Reactive arthritis].

The term reactive arthritis was introduced to describe an acute non-purulent arthritis complicating an infection elsewhere in the body. Reactive arthritis can also be classified into HLA-B27 associated and non-associated forms. Rheumatic fever is an example of the HLA-B27 non-associated forms with genetic factors other than HLA-B27 involved. HLA-B27 associated reactive arthritis includes enteric, urogenic and idiopathic arthritides. The bacteria known to trigger post-enteritic reactive arthritis are: Yersinia, Salmonella, Shigella, Campylobacter, Clostridium difficile and Brucella; those known…

[Lyme arthritis].

Infection by spirochete Borrelia burgdorferi is mainly observed in the United States where it has taken the name of Lyme's disease, and in Europe. Its evolution may be very extended, in several phases. A few weeks after a tick-bite, the primary lesion appears, which is a chronic migrating erythema. In the following weeks or months, in the secondary phase, nerve lesions may appear, quite similar in Europe and in the United States, and cardiac lesions, mostly seen in the United States. In the following weeks, months, or years, during the tertiary phase, articular lesions may appear, frequent in
