[Triggering bacterial infections in reactive arthritis].

108 patients, 75 men and 33 women, mean age 35 years, with reactive arthritis were studied by microbiological and serological methods for identification of a probable triggering infection. In 32 of the patients a Chlamydia infection, in 30 patients--Yersinia infection, in 7 patients--Staphylococcus infection and in 4 patients--Shigella infection were found. In 27 patients no infectious agent was found but in 4 of them a positive CBR for chlamydia in the sex partner was found. The course of the arthritis is acute or subacute, oligo- or polyarthritis, involving mainly the large joints of the leg

Clinical picture of reactive salmonella arthritis.

Twenty-three patients with reactive salmonella arthritis were treated at the Departments of Medicine (in 1970-1984) or at the Outpatient Department (in 1980-1986) of Meilahti Hospital. Nine different salmonella species were implicated as triggering factors. Salmonella was detected in 74% of the patients by stool cultures. Widal test was positive (greater than or equal to 1:160) in the remainder. The acute clinical picture was that of oligo or polyarthritis. Other musculoskeletal and inflammatory symptoms were frequent. The mean duration of the acute arthritis was 4.7 months (range 1.5-6.0). In

[Intestinal bacteria incriminated in reactive arthritis].

Amongst the numerous microbial species which constitute the intestinal flora, three bacteria belonging to the family of Enterobacteria have been incriminated in cases of reactive arthritis: the Salmonellae, the Shigellae and the Klebsiellae. These bacteria are easy to isolate and identify, but they are difficult to classify because of the multiplicity of serotypes and species. The participation of these bacteria in reactive arthritis is far from being clear: their isolation does not always have pathological significance, as these bacteria are usually opportunistic pathogens. The serological re

Reactive arthritis associated with Campylobacter enteritis.

Campylobacter jejuni has recently become recognized as a leading cause of diarrhea. To date reactive arthritis has rarely been diagnosed in association with Campylobacter enteritis. Clinical manifestations, diagnostic studies and management of 21 patients with Campylobacter-reactive arthritis are reviewed. The symptom-free interval between diarrhea and arthritis lasts from a few days to several weeks. In two-thirds of the patients sterile joint involvement is multiple, is migratory and includes large joints. Fever and leukocytosis are usually absent, whereas the erythrocyte sedimentation rate

Reactive arthritis associated with campylobacter enteritis.

Campylobacter enteritis is a known aetiological factor in reactive arthritis. We surveyed patients in the Harrow Health District known to have had campylobacter enteritis for manifestations of arthritis. Acute attacks of arthritis occurred in 8 of 33 adults admitted with enteritis. None were identified in patients under 16 or in those diagnosed in the community.

Reactive arthritis mimicking inflammatory bowel disease arthritis: a challenging diagnosis.

Reactive arthritis comprises a subgroup of infection-associated arthritis which occurs after genitourinary or gastrointestinal tract infection in genetically susceptible hosts. Studies have proposed Salmonella, Shigella or Yersinia infection as the microorganisms responsible for the post-dysenteric form. The human leukocyte antigen (HLA)-B27 is a well recognised best-known predisposing factor. We report a case of HLA-B27-associated reactive arthritis after Salmonella goldcoast enteritis, mimicking inflammatory bowel disease arthritis.

Reactive arthritis: the role of bacterial antigens in inflammatory arthritis.

For more than 100 years it has been suspected that bacteria or products derived from them are deposited in joints and cause arthritis without suppuration. Over this time a vast amount of evidence, much of which is still unchallenged, has accumulated to demonstrate that whole bacteria and subcellular bacterial elements do pass, under certain circumstances, from sites of mucosal colonization or infection into the circulation and thence into joints. Similarly, experimental studies have demonstrated that the deposition of both inert material and bacterial components within synovium is sometimes, b

Reactive arthritis: is it a useful concept?

Reactive arthritis (ReA) is one of the most common arthritides affecting young adults. In most cases it follows urogenital or enteric bacterial infection, but its pathogenesis is not fully understood. It is generally considered a sterile arthritis which appears to involve immune response to bacterial organisms and genetic host susceptibility associated with the presence of HLA-B27 antigen. New findings suggest that in some ReA cases, viable bacteria are present inside the joints, and these organisms may cause the disease and trigger the inflammatory response. ReA manifests clinically as a rheu

Mycobacterium-Induced Reactive Arthritis in an Older Adult With Rheumatoid Arthritis.

The use of immunosuppressive medications to treat rheumatoid arthritis may trigger the activation of latent mycobacteria, leading to infection. These infections can lead to reactive arthritis. Conversely, both reactive and rheumatoid arthritis may be encountered in the geriatric population. When such complications arise, the treatment process becomes more complicated, necessitating careful consideration of elaborate therapeutic approaches. An 83-year-old man presented to our hospital with subacute back pain and arthralgia of the extremities. The patient was diagnosed with rheumatoid arthritis

[The comparative clinico-laboratory characteristics of Lyme arthritis and reactive arthritis].

The paper presents comparative clinical and laboratory characteristics of 17 cases of Lyme-arthritis (LA) and 28 cases of reactive arthritis (RA). LA and RA patients differed by age and sex. LA arose after the tick bite followed by erythema migrans or infection symptoms. In RA patients the above symptoms were absent. In LA patients arthritis was associated with typical symptoms of neuroborreliosis. The articular syndrome manifested similarly in LA and RA: arthritis was preceded by arthralgias, low limb joints and periarticular tissues were involved most frequently. The differences were: monoar

Poststreptococcal reactive arthritis mimicking acute septic arthritis: a hospital-based study.

Poststreptococcal reactive arthritis (PSRA) is an inflammatory arthritis that follows group A streptococcal pharyngitis. The clinical presentation of PSRA can mimic acute septic arthritis. We present 12 children with PSRA seen between December 1991 and September 1993. Most children had only mild pharyngitis by history. The pattern of arthritis was variable and included migratory polyarthritis, additive polyarthritis, and monoarthritis, and was accompanied by fever in seven children. In four patients, the presentation of fever and acute onset of painful monoarthritis mimicked septic arthritis,

Systemic Mastocytosis: A Mimicker of Reactive Arthritis.

OBJECTIVES: Illustration of a case of systemic mastocytosis mimicking reactive arthritis in the absence of an infectious etiology. METHODS: Review of the patient's medical records. RESULTS: We report a case of systemic mastocytosis relapse, presenting with pancytopenia accompanied by knee monoarthritis, cystitis, and bilateral conjunctivitis occurring simultaneously at the same time interval within 2-4 days, mimicking reactive arthritis in the absence of an infectious etiology. CONCLUSION: Our case demonstrated reactive arthritis features (triad of urethritis, conjunctivitis, and arthritis)

Reactive arthritis after SARS-CoV-2 infection.

SARS-CoV-2 has been recognised as a potential trigger of inflammatory arthritis in individuals with inflammatory rheumatic diseases as well as in previously unaffected individuals. However, new-onset arthritis after COVID-19 is a heterogeneous phenomenon that complicates differential diagnosis. For example, acute arthritis with features of viral arthritis has been reported after COVID-19, as has crystal-induced arthritis. Arthritides mimicking reactive arthritis (ReA) have also been described, but these patients often do not fulfil the typical features of ReA: several reports describe cases of
