Patellofemoral pain in athletes.

Patellofemoral pain (PFP) is a frequent cause of anterior knee pain in athletes, which affects patients with and without structural patellofemoral joint (PFJ) damage. Most younger patients do not have any structural changes to the PFJ, such as an increased Q angle and a cartilage damage. This clinical entity is known as patellofemoral pain syndrome (PFPS). Older patients usually present with signs of patellofemoral osteoarthritis (PFOA). A key factor in PFPS development is dynamic valgus of the lower extremity, which leads to lateral patellar maltracking. Causes of dynamic valgus include weak …

Pars Injuries in Athletes.

Pars injuries are common causes of low back pain in adolescent athletes. Workup traditionally has included lumbar radiographs with oblique views and single-photon emission computed tomography (SPECT). However, recent literature has demonstrated the accuracy of MRI as a diagnostic modality. Acute injuries may be amenable to bracing with the goal of a healed lesion. Most cases of spondylolysis will result in asymptomatic non-union, though pars repair is an option for symptomatic pars defects without spondylolisthesis.

Lyme disease in athletes.

Lyme disease, a bacterial infection transmitted by ticks, is the most common vector-borne disease in the northern hemisphere. Athletes who train or compete in wooded environments in endemic regions are at increased risk of contracting Lyme disease. Variability in clinical presentation, masquerading symptoms, and limitations in testing may lead to misdiagnosis. Early diagnosis and treatment result in full recovery for most patients with Lyme disease; however symptoms may persist for months to years, especially when diagnosis is delayed. This article reviews the epidemiology, clinical manifestat…

[Atrial fibrillation in athletes].

Atrial fibrillation (AF) is the most common cardiac arrhythmia affecting up to 1-1.5% of the population. Regular physical activity reduces the risk of cardiovascular diseases, however several studies have shown paradoxically increased incidence of AF in people practicing sport, especially in elite athletes. The results of studies suggest a U-shape relationship between sport and risk of arrhythmia. Minor regular exertion protects from arrhythmia through reduction in AF risk factors, while intense physical activity increases the risk of arrhythmia. The etiopathogenesis of arrhythmia in athletes …

[Patella dislocation in athletes].

Most dislocations of the patella occur during sports activities. The entities instability, maltracking and loss of tracking should be define … l knee baseline or special forms of torsional deformities. Although patella dislocations in trained athletes are seldom due to the stabilizing muscular status, the treatment strategy is similar to that of normal persons. Additional cartilage injuries, type of sports and time for rehabilitation have to be considered for optimal treatment.

Vaccination in elite athletes.

Public health vaccination guidelines cannot be easily transferred to elite athletes. An enhanced benefit from preventing even mild diseases is obvious but stronger interference from otherwise minor side effects has to be considered as well. Thus, special vaccination guidelines for adult elite athletes are required. In most of them, protection should be strived for against tetanus, diphtheria, pertussis, influenza, hepatitis A, hepatitis B, measles, mumps and varicella. When living or traveling to endemic areas, the athletes should be immune against tick-borne encephalitis, yellow fever, Japane…

Atrial fibrillation in athletes.

Many studies have shown a relationship between long-term endurance sport practice and atrial fibrillation. Inflammation, anatomic remodelling, alterations in the autonomic system and neurohormonal activation are all possible explanations for the increased prevalence of this arrhythmia in athletes. Atrial fibrillation may determine disabling symptoms like palpitations and impaired physical performance, compromising eligibility for competitive activities, but exclusion from sport is not necessary for all athletes. Limited data are available on drug therapy and recently ablation resulted to be a …

Infectious disease in athletes.

While orthopedic injuries most commonly are associated with sports, infectious diseases cause significant morbidity in athletes. Exercise improves immunity at moderate intensity but impairs immune function at extremes of duration and intensity. Respiratory infections are the most common, but skin, blood borne, sexually transmitted, and even cardiac infections occur. Infectious disease outbreaks are a constant concern. Treatment of such infections resembles those used in the general population. Return to play issues and prevention of infection are especially important in athletes.

Back pain in athletes.

The athlete with back pain presents a clinical challenge. Self-limited symptoms must be distinguished from persistent or recurrent symptoms associated with identifiable pathology. Athletes involved in impact sports appear to have risk factors for specific spinal pathologies that correlate with the loading and repetition demands of specific activities. For example, elite athletes who participate in longer and more intense training have higher incidence rates of degenerative disk disease and spondylolysis than athletes who do not. However, data suggest that the recreational athlete may be protec…

Neurovascular trauma in athletes.

Injuries to neurovascular structures are not the most common injuries seen in athletes and for this reason may often be overlooked. Additionally, diagnosis and management may be more difficult because of inexperience with these injuries. The majority of acute sports-related neurovascular injuries are associated with contact sports such as rugby, wrestling, ice hockey, and especially football. These injuries most commonly occur about the shoulder girdle and brachial plexus, with "burners" syndrome being the most common. Less common injuries include thoracic outlet syndrome, effort-induced throm…

Scapholunate instability in athletes.

Sports medicine practitioners must be sensitive to even the smallest injuries that affect athletes. Often, less severe ligament injuries go undetected. Scapholunate dissociation is often overlooked in this manner. The authors provide an overview of presentation, diagnosis, and nonoperative and operative treatments of this condition.

Hip arthroscopy in athletes.

The limited data (n = 42) and diverse pathology within this study make statistical analysis difficult, although the observations are still meaningful. Diagnostic arthroscopy has defined elusive causes of disabling hip pain in an athletic population including occult labral and chondral damage and rupture of the ligamentum teres. Operative arthroscopy has been effective in reducing the symptoms associated with many of these forms of pathology. For more evident causes of hip pain, such as loose bodies or impinging osteophytes, arthroscopy offers an excellent alternative to traditional open techni

Elbow injuries in athletes.

Most elbow injuries carry an excellent prognosis for return to a given sport. Early identification of overuse injuries and joint contractures can deter bone deformities in later life. Early evaluation can deter severe complications such as that of neurovascular injury. Rehabilitation should not be considered complete until there has been maximal restoration and correction of technique and accuracy in addition to the issue of soft-tissue healing.

Nerve Injuries in Athletes.

In brief: Nerve injuries in athletes may be serious and may delay or prevent an athlete's return to his or her sport. Over a two-year period, the authors evaluated the condition of 65 patients who had entrapments of a nerve or nerve root, documented with electromyography. They describe four case histories: Two patients had radial nerve entrapments, one caused by baseball pitching and the other by kayaking; one football player had combined suprascapular neuropathy and upper trunk brachial plexopathy; and one patient had carpal tunnel syndrome of a median nerve secondary to rowing. Sports-relate…
