Salivary secretion of immunoglobulin A by submandibular glands in response to autonomimetic infusions in anaesthetised rats.

Salivary secretion of immunoglobulin A (lgA) by submandibular glands is increased by stimuli from autonomic nerves. Since it is unclear which specific autonomic receptors transduce such stimuli, we have infused autonomimetics intravenously and compared secretion of fluid, IgA and stored proteins (peroxidase and total protein) with secretory responses during electrical stimulation of the parasympathetic nerve supply in anaesthetized rats. The greatest secretion of IgA was evoked by the alpha-adrenoceptor agonist phenylephrine and this was reduced by the beta-adrenoceptor blocking drug propranol

A Bridge Through the (Ventricular Tachycardia) Storm: The Role of Continuous Stellate Ganglion Blockade-A Case Report.

The stellate ganglion block (SGB) is a valuable adjunct to treat ventricular arrhythmias (VAs), particularly in cases of electrical storm refractory to conventional management. While single-shot SGB has been studied, the role of continuous SGB remains largely unexplored. An adult patient presented with stroke and acute aortic dissection requiring emergent surgery. Postoperatively, the patient developed recurrent polymorphic ventricular tachycardia (VT) refractory to medical management and multiple cardioversions. A continuous left SGB was performed, and VT storm resolved. This case highlights

Novel ultrasound-guided supraclavicular stellate ganglion block.

INTRODUCTION: Stellate ganglion block (SGB) provides diagnostic and therapeutic benefits in pain syndromes in the head, neck, and upper extremity, including complex regional pain syndrome Types I and II, Raynaud's disease, hyperhidrosis, arterial embolism in the region of the arm. METHODS: We present a novel ultrasound-guided supraclavicular stellate ganglion block. Considering the existing anatomical structures of the targeted area. RESULTS AND CONCLUSIONS: We hope that we can provide fewer complications and additional benefits with this new approach.

Use of thoracoscopy for thoracic sympathetic nerve block in primary hyperhidrosis.

Thoracic sympathetic nerve block (TSNB) has been widely used in the treatment of neuropathic pain. To reduce block failure rates, TSNB is assisted with several modalities including fluoroscopy, computed tomography, and ultrasonography. The present study describes our experience assessing the usefulness of thoracoscopy in TSNB for predicting compensatory hyperhidrosis before sympathectomy in primary hyperhidrosis. From September 2013 to October 2021, TSNB was performed under local anesthesia using a 2-mm thoracoscope in 302 patients with severe primary hyperhidrosis. Among the 302 patients, 294

[Ultrasound-guided low-volume continuous cervical sympathetic nerve block for treatment of an electrical storm].

In addition to the treatment for complex regional pain syndrome (CRPS), the stellate ganglion block is a treatment option for refractory intermittent ventricular tachycardia (VT). Despite the use of imaging techniques, such as fluoroscopy and ultrasound, numerous side effects and complications have been reported. These are a result of the complex anatomical site and the volume of injected local anesthetics. This article reports on the catheter placement for continuous block of the cervical sympathetic trunk with high-resolution ultrasound imaging (HRUI) in a patient with intermittent VT. The t

Ultrasound-guided femoral nerve block combined with lateral femoral cutaneous nerve block in a patient with congenital insensitivity to pain and anhidrosis: a case report.

Congenital insensitivity to pain with anhidrosis (CIPA), also known as hereditary sensory and autonomic neuropathies (HSAN I-V), is an exceptionally rare autosomal recessive disorder. The pathogenesis of CIPA remains not fully elucidated. Clinical manifestations primarily include the absence of pain perception, painless injuries to the extremities, oral mutilation, anhidrosis accompanied by hyperthermia, and delayed intellectual development. Given the autonomic dysfunction in CIPA patients, previous reports on anesthetic management for this rare disorder have predominantly depended on general

T1 Paravertebral Block Without Ventral Pleural Shift Resembles Intertransverse Process Block With Reduced Sympathetic Blockade: A Case Series.

Although T1-level paravertebral block (T1PVB) may provide both stellate ganglion and partial brachial plexus blockade, its deep anatomical location makes the procedure technically demanding. We retrospectively analyzed 54 of 65 ultrasound-guided T1PVB procedures performed in 11 patients, in which ventral shift of the pleura (VSP) could be evaluated. Procedures with VSP showed more frequent Horner's sign and greater ipsilateral upper-limb temperature increases than those without VSP. These findings suggest that T1PVB without VSP may functionally resemble intertransverse process block (ITPB), wh

Comparison of injectate spread and nerve coverage between single-injection intertransverse process block and paravertebral block at the T2 level: a cadaveric study.

BACKGROUND: We compared the spread of an injectate into the thoracic sympathetic chain resulting from a single-injection intertransverse process (ITP) block with that of a single-injection PV block at the T2 level. METHODS: Sixteen soft-embalmed cadavers were used. The right hemithorax was randomly allocated to receive either an ultrasound-guided single-injection ITP block or ultrasound-guided single-injection PV block at the T2 vertebral level, with the opposite block administered to the other side. Approximately 10 mL of latex dye solution was injected into each hemithorax using a random al

The hemodynamic and pain impact of peripheral nerve block versus spinal anesthesia in diabetic patients undergoing diabetic foot surgery.

PURPOSE: Comparison of hemodynamic profiles and pain scores in diabetic patients undergoing diabetic foot surgery receiving peripheral nerve block (PNB) or spinal anesthesia [subarachnoid block (SAB)]. METHODS: This was a prospective, randomised controlled trial. We recruited diabetic patients aged > 18 years, American Society of Anesthesiologists class II-III, who were scheduled for unilateral diabetic foot surgery below the knee. All patients were assessed for autonomic dysfunction using the Survey of Autonomic Symptoms score. Participants were randomly assigned to receive either PNB or SAB

beta-Bungarotoxin and parasympathetic nerve blocking action.

Autonomic effects of beta-bungarotoxin (beta-BuTX), a presynaptic blocking agent at the neuromuscular junction, were examined with parasympathetic (vagus), sympathetic nerve-atrial preparations isolated from guinea pigs. beta-BuTX in concentrations ranging from 0.01 to 10 micrograms/ml abolished the negative inotropic and chronotropic responses to electrical stimulation of the preganglionic trunks of the vagus nerve. This effect was time- and dose-related and was not restored by repeated washings. Such blocking action of beta-BuTX was also obtained when post-ganglionic stimulation was applied.

Pharmacological characterization of voltage-sensitive Ca2+ channels in autonomic nerves.

Hololena curta venom a potent inhibitor of voltage sensitive Ca2+ channels and neurotransmitter release in mammalian brain, and synthetic funnel web spider toxin an inhibitor of P channels, were examined for their activity on autonomic nerves. Hololena curta (0.5 to 5.0 micrograms venom protein/ml) potently inhibited motor responses of the cholinergic guinea pig ileum myenteric plexus and the adrenergic rat anococcygeus muscle. Synthetic funnel web spider toxin was inactive at concentrations up to 100 microM. Hololena curta inhibited K+, and electrically evoked release of tritium from labeled

[Macroglossia after neck clipping of a VA-PICA (vertebral artery-posterior inferior cerebellar artery) aneurysm].

A case of macroglossia following neck clipping of VA-PICA aneurysm is described. The etiology of this rare complication has been thought to be venous obstruction related to extreme flexion of the head in the sitting position. On the other hand, Moore et al recently proposed that the primary mechanism of macroglossia after posterior fossa exploration might be neurogenic origin. We consider that our case supports the latter hypothesis. In our case, the lingual swelling was aggravated for four consecutive days following the operation and then began to improve rapidly. The swelling resolved comple
