Intrathecal baclofen is a useful therapy in patients with spasticity. We describe a patient who underwent an intrathecal pump implant, complicated by epidural lipomatosis that ultimately required a single level laminectomy and fat debulking before successful implantation.

Five patients with generalized dystonia who were refractory to oral medications were treated by continuous intrathecal baclofen infusion. Dystonia was related to cerebral palsy in three patients and to Hallervorden-Spatz disease in two. Spasticity is a common problem encountered by those who have suffered a neurological injury either cerebral or spinal in origin. Multiple interventions are utilized to manage spasticity to decrease the pain and stiffness. These interventions can include an implanted device that delivers medication directly to the spinal cord. This clinical consultation reviews a patient case and the important information regarding the care of this individual with an intrathecal baclofen pump and important key points of education that all rehabilitation nurses need to know.

Spasticity of cerebral or spinal origin severely impairs an individual's functional ability and quality of life. Intrathecal baclofen (ITB) therapy via an implantable pump is indicated for use in patients unresponsive to oral antispasmodics. Baclofen has been increasingly used in the treatment of alcohol withdrawal syndrome (AWS). We present a patient with AWS and psychiatric comorbidity who ingested 700 mg of baclofen. ICU admission was necessary for ventilatory support and symptomatic treatment. The patient was dismissed without sequelae.

We report the case of a patient who presented with coma in the context of baclofen and quetiapine overdose. While the clinical picture was largely in keeping with previous descriptions of baclofen and quetiapine toxicity, the patient went on to exhibit clinical and biochemical features of cranial diabetes insipidus, a finding which has not previously been described in this context.

OBJECTIVE: Baclofen is a GABA-B agonist that may be useful in the treatment of substance use disorders, and also reduces 'binge-like' eating in rodents. We hypothesized that baclofen might be effective in reducing binge eating episodes in binge eating disorder (BED) and bulimia nervosa (BN).

Dose stabilization of intrathecal baclofen (ITB) is usually achieved within 6 to 18 months. In some patients, an increase in ITB dose may result in paradoxical unresponsiveness to baclofen. We present two cases demonstrating increased tone and functional decline with increasing ITB doses after a period of clinical stability. In both cases, reduction of ITB dose was associated with marked clinical improvement.

Recurrent priapism may lead to erectile dysfunction; its treatment is aimed at the prevention of new episodes. We report a case of idiopathic recurrent priapism in a 15-year-old patient. He was treated with oral baclofen, starting with a daily dose of 10mg that was increased to 30 mg to achieve control of symptoms. After a 6-month follow up, he reported no recurrence or side effects.

The aim of the study was to evaluate some clinical aspects of acute intoxication with baclofen.
