[Intrauterine treatment of fetal tachycardia causing circulatory failure].

The authors report a case of fetal supraventricular tachycardia with the heart rate of 400/min diagnosed in the 34th week of pregnancy. Congestive heart failure forming non immun hydrops fetalis was occurred by this rhythm disturbance. With digitalis and beta receptor blocker propranolol therapy administered to the mother the rhythm disturbance has stopped, ascites and hydrothorax secondary to fetal congestive heart failure have been abolished. Congenital heart disease and cardiomyopathy were excluded by fetal echocardiography. In the 40th week a healthy newborn was delivered by cesarean secti

[Medicinal rhythm control in atrial fibrillation].

Medicinal antiarrhythmic therapy is either used in the acute setting to convert atrial fibrillation to sinus rhythm or as chronic medication to preserve sinus rhythm if a rhythm control strategy is followed. The choice of the antiarrhythmic agent is based on the presence or absence of structural heart disease. In addition, oral anticoagulation should be established according to current guidelines. In the acute setting the armamentarium comprises flecainide, propafenone, vernakalant and amiodarone. Usually, combination therapy with an atrioventricular (AV) node slowing drug (a beta blocker or v

Ventricular tachycardia and accelerated ventricular rhythm presenting in the first month of life.

Fourteen infants aged less than 1 month presented to our institution during the last 22 years with ventricular tachycardia (VT) or accelerated ventricular rhythm and a structurally normal heart. In 2, VT was associated with long QT syndrome. Both are alive on beta-blocker therapy, 1 with an implanted pacemaker. Twelve infants had accelerated ventricular rhythm, and 2 of these died in the first 2 months of life of unrelated conditions. The other 10 are alive at a median age of 4 years (range 2 months to 11 years), and none were lost to follow-up. Hemodynamic compromise did not occur with accele

Factors predicting success rate and recurrence of atrial fibrillation after first electrical cardioversion in patients with persistent atrial fibrillation.

BACKGROUND: The recurrence rate of atrial fibrillation (AF) after elective cardioversion is high. HYPOTHESIS: The study aimed to identify clinical predictors for successful electrical cardioversion and maintenance of sinus rhythm after a first electrical cardioversion in patients with persistent AF without concomitant antiarrhythmic drugs of class I and III. METHODS: Consecutive outpatients (n = 166) with persistent AF for > 1 month, scheduled for elective cardioversion, were prospectively included in the study. A clinical investigation, echocardiographic assay, and Holter electrocardiogram

Circadian rhythm disorder in a rare disease: Smith-Magenis syndrome.

Smith-Magenis syndrome (SMS) is a clinically recognizable contiguous gene syndrome, caused by interstitial deletion of chromosome 17p11.2. The SMS phenotype include distinctive facial features, developmental delay and neurobehavioral abnormalities. The patients present major sleep disturbances ascribed to a phase shift of their circadian rhythm of melatonin with a paradoxical diurnal secretion of the hormone. Treatment with morning beta-blockers and evening melatonin reinstated a normally timed melatonin circadian rhythm, improved daytime behavior and restored normal sleep habits, resulting in

Serum from Myalgic encephalomyelitis/chronic fatigue syndrome patients causes loss of coherence in cellular circadian rhythms.

Myalgic Encephalomyelitis/Chronic Fatigue Syndrome (ME/CFS) is a disabling disorder characterized by disrupted daily patterns of activity, sleep, and physiology. Past studies in ME/CFS patients have examined circadian rhythms, suggested that desynchronization between central and peripheral rhythms may be an important pathological feature, and identified associated changes in post-inflammatory cytokines such as transforming growth factor beta (TGFB). However, no previous studies have examined circadian rhythms in ME/CFS using cellular models or studied the role of cytokines on circadian rhythms

Management of atrial fibrillation in the emergency department and following acute myocardial infarction.

Atrial fibrillation (AF) is the most common arrhythmia managed by emergency physicians and there is increasing evidence that selected patients with acute AF can be safely managed in the emergency department without the need for hospital admission. The principles of management are identification and treatment of precipitating or underlying causes, hemodynamic stabilization/rate control, reduction of thromboembolism risk and the conversion/maintenance of sinus rhythm. A strategy of rate or rhythm control should be chosen based on the patient's clinical status, the duration of AF, the experience

Electrical cardioversion for persistent or chronic atrial fibrillation: outcome and clinical factors predicting short and long term success rate.

AIMS: To assess the effectiveness and to identify predictors for successful electrical cardioversion (ECV) and maintenance of sinus rhythm, in long term follow up of patients with persistent (PAF) and chronic atrial fibrillation (CAF). METHODS AND RESULTS: Retrospective analysis of medical records of 68 patients with PAF or CAF, who underwent 91 cardioversions. ECV was successful in 86 attempts (94.5%). In obese (body mass index>30) and hypertensive patients (blood pressure >140/90 mm Hg), ECV was less successful in restoring sinus rhythm (p<0.05, p<0.021, respectively). Sinus rhythm was main

Carvedilol overdose with quantitative confirmation.

Carvedilol is a non-selective beta-adrenoreceptor antagonist that is also an antagonist at the alpha(1)-adrenoreceptor. This unique pharmacological effect may produce a different toxicodynamic profile compared to other beta-adrenoreceptor antagonists. Only one previous case of carvedilol overdose has been reported. Here, we report massive carvedilol ingestion confirmed by quantitative analysis. The case report deals with an 84-year-old man who chewed a total of 60 (6.25 mg) tablets and rapidly developed symptoms. Vital signs on presentation were systolic blood pressure 70 mmHg; heart rate 45 b

The changes of 272 EEG parameters in 252 women and 97 men during the period of life of 16 to 45 years in increments of one month were investigated. Correlation analysis and approximations were used. Ontogenetic regression of amplitude and power amplitude and power of alpha, theta, delta, but not beta rhythms predominantly in the right posterior cortex leads in women was more regressive than in men was shown. Overall ontogenetic tendency to fall to the nonlinear amplitude and power of EEG rhythms with age is reproduced in both sexes. Frequency of rhythms in the course of ontogeny, as a rule, wa…

The clinical usefulness of ictal surface EEG in neocortical epilepsy.

PURPOSE: Localizable scalp EEGs, during ictal episodes, appear to be rare in neocortical epileptic syndromes. However, studies based on large numbers of patients are also rare. This study aims to identify the characteristic patterns of variable neocortical epilepsies and to evaluate their clinical usefulness in the localization of epileptogenic focuses. METHODS: We retrospectively assessed 394 noninvasive ictal recordings from 86 patients who subsequently underwent invasive study and resective surgery. Ictal EEGs were recorded using a video-EEG monitoring system with electrodes placed accordi

[The effect of short peptides isolated from the brain of a hibernating suslik on the rat EEG and behavior].

The intracerebroventricular injection of the peptides kyotorphin (KT), newkyotorphin (NKT), and Aps-Tyr (BS) changed rat behaviour. There was an increase in the rate of exploratory reactions, rearing, grooming, stereotypic scratching movements, yawning, stretching, hiccupping, sneezing. Similar effects had been earlier observed in rats after administration of the fraction (F, 1-10 kD) from the brain of hibernating ground squirrels in which the studied peptides were found. The EEG effects depended on a peptide dose. As in the case of F injection, KT in the dose of 4 mcg suppressed the theta- an

Cortical sources of resting-state EEG rhythms in "experienced" HIV subjects under antiretroviral therapy.

OBJECTIVE: Treatment-naïve patients with human immunodeficiency virus (HIV) are characterized by diffuse abnormalities of resting-state cortical electroencephalographic (EEG) rhythms (Babiloni et al., 2012a). Here, we tested the hypothesis that these EEG rhythms vary as a function of the systemic immune activity and antiretroviral therapy (ART) in HIV patients. METHODS: Resting-state eyes-closed EEG data were recorded in 68 ART-HIV patients (mini mental state evaluation (MMSE) of 27.5 ± 0.3 SEM), in 60 treatment-naïve HIV subjects (MMSE of 27.5 ± 0.4 SEM) and in 75 age-matched cognitively nor

Electroencephalographic quantification by time domain analysis in normal 7--15-year-old children.

Automatic EEG analysis was performed on 239 apparently healthy school age children. The children were classed in 3 age groups: (1) average a … g their means and standard deviations, for each age group under each recording condition. Delta and beta rhythm values were not calculated, because of their rarity in this apparently normal sample. The precision obtained by automatic analysis could not have been obtained by means of classical visual analysis. The present findings suggest that the statistical …

Frontal theta and beta oscillations during lower-limb movement in Parkinson's disease.

OBJECTIVES: Patients with Parkinson's disease (PD) have deficits in lower-limb functions such as gait, which involves both cognitive and motor dysfunction. In PD, theta and beta brain rhythms are associated with cognitive and motor functions, respectively. We tested the hypothesis that PD patients with lower-limb abnormalities would exhibit abnormal theta and beta rhythms in the mid-frontal cortical region during lower-limb action. METHODS: This study included thirty-nine participants; 13 PD patients with FOG (PDFOG+), 13 without FOG (PDFOG-), and 13 demographically-matched controls. We recor…

Spontaneous high-frequency (10-80 Hz) oscillations during up states in the cerebral cortex in vitro.

High-frequency oscillations in cortical networks have been linked to a variety of cognitive and perceptual processes. They have also been recorded in small cortical slices in vitro, indicating that neuronal synchronization at these frequencies is generated in the local cortical circuit. However, in vitro experiments have hitherto necessitated exogenous pharmacological or electrical stimulation to generate robust synchronized activity in the beta/gamma range. Here, we demonstrate that the isolated cortical microcircuitry generates beta and gamma oscillations spontaneously in the absence of exte

[The antiarrhythmic effect of digitalis in acute myocardial infarction with cardiac failure (author's transl)].

In 59 digitalized and 3 non digitalized patients the effect of digitalis during the 1st to 4th days after transmural myocardial infarction was controlled. Rhythm disturbances in acute myocardial infarction may arise secondary to a complicating cardiac failure and may be influenced by digitalis. In 9 of 17 cases (53 p.c.) with ventricular or supraventricular extrasystoles daily doses of 0,4 mg beta-Methyldigoxin or 0,4 mg Digoxin i.v. resulted in undisturbed sinus rhythm. In two cases supraventricular tachycardia and extrasystoles with rapid ventricular rate were abolished by 1,2 mg beta-Methyl

Cellular- and systems-level profiling of amyloid-beta effects on circadian timing.

Disruption of the circadian timing system has been reported in the preclinical phase of Alzheimer's disease (AD) and is a well-characterized component of mid- and late-stage AD. Given the distributed nature of the circadian timing system, with a central pacemaker in the suprachiasmatic nucleus (SCN) and peripheral clocks throughout the brain, understanding how AD affects this system has been challenging. To investigate how AD may disrupt circadian physiology, we focused on the amyloid-beta peptide, a key contributor to familial early-onset AD. Using the 5xFAD mouse model and ex vivo single-cel
