International health and the limits of its global influence: bhutan and the worldwide smallpox eradication programme.

Histories of the global smallpox eradication programme have tended to concentrate on the larger national formations in Africa and Asia. This … ipheral or as being largely shorn of sustained campaigns against the disease. Using a case study of Bhutan, a small Himalayan kingdom sandwiched between India and China, an effort is made to reclaim the historical experiences in small national entities in the worldwide smallpox eradication programme. Dog bites in humans and estimating human rabies mortality in rabies endemic areas of Bhutan.

BACKGROUND: Dog bites in humans are a public health problem worldwide. The issues of increasing stray dog populations, rabies outbreaks, and the risk of dogs biting humans have been frequently reported by the media in Bhutan. This study aimed to estimate the bite incidence and identify the risk factors for dog bites in humans, and to estimate human deaths from rabies in rabies endemic south Bhutan. METHODS: A hospital-based questionnaire survey was conducted during 2009-2010 among dog bites victims who visited three hospitals in Bhutan for anti-rabies vaccine injection. Decision tree modeling…

The outbreak peaked in February 2006 and subsided by the end of April 2006 with sporadic cases reported until November 2007. Rabies affected 18 of the 40 sub-districts in the three eastern districts of Bhutan. There were reportedly one human and 256 domestic animal fatalities. The outbreak affected cattle (n = 141, 55%), dogs (n = 106, 41%), horses (n = 7, 3%) and cats (n = 2, 1%). Rabies was primarily diagnosed by clinical signs but 36 cases were confirme…

Extensive Endemic Transmission of Multidrug-Resistant Mycobacterium tuberculosis in Bhutan: A Retrospective Genomic-Epidemiological Study.

BACKGROUND: The proportion of multidrug-resistant tuberculosis (MDR-TB) cases is increasing in Bhutan. We conducted the first retrospective genomic-epidemiological study to provide insights into the population structure, resistance patterns, and recent transmission in Bhutan. METHODS: Whole genome sequencing was performed on randomly selected drug-resistant (DR-TB) and drug-sensitive TB (DS-TB) isolates from Bhutan, collected between 2018 and 2022 at the Microbiological Diagnostic Unit Public Health Laboratory in Melbourne, Australia. Bioinformatic analysis was performed to identify drug-resi…

Valuing health across groups: a cross-sectional population-based willingness-to-pay survey in Bhutan.

INTRODUCTION: Context-specific cost-effectiveness thresholds (CETs) informed by societal willingness to pay (WTP) are crucial for healthcare resource allocation in low- and middle-income countries. This study investigated WTP for health per quality-adjusted life year (QALY) in Bhutan. METHODS: A WTP survey was conducted alongside the 2023 National Health Survey in Bhutan, sampling 1869 households. Using contingent valuation, respondents assigned monetary values to three hypothetical scenarios: 1 year of cancer symptom-free life, 1 year of perfect health and 5 years of perfect health. We used …

Cervical cancer screening program in Thimphu, Bhutan: population coverage and characteristics associated with screening attendance.

BACKGROUND: Bhutan has been engaged in good-quality cytology-based cervical screening since 2000 and has vaccinated >90% girls against human papillomavirus (HPV) since 2010. We explored the characteristics associated with lack of previous screening and screening coverage in women age ≥25 years. METHODS: Women were invited at home or during their attendance at 2 outpatient clinics, in the capital, Thimphu, and nearby Lungthenphu. Age-adjusted odds ratios for lack of previous screening by selected characteristics were computed among 1,620 participating women. In Thimphu an invitation registry a…

Mortality among patients diagnosed with stroke presenting to remote Emergency Department in Bhutan - a retrospective study.

BACKGROUND: Stroke is a leading cause of disability and mortality globally, particularly in low- and middle-income countries (LMICs). Outcomes remain poor in remote, resource limited settings due to delayed presentation, limited health literacy, and constrained emergency care systems. Evidence on stroke outcomes from such settings is limited. This study aimed to determine stroke-related mortality and identify factors associated with mortality among patients presenting to a remote Emergency Department (ED) in Bhutan. METHODS: A retrospective cross-sectional study was conducted from August 1, 2…

Effective refractive error coverage and quality gaps in Bhutan: evidence from rapid assessment of refractive error.

BACKGROUND: Uncorrected refractive errors (URE) and presbyopia are leading causes of visual impairment (VI) globally. Bhutan currently lacks population-based data on adult refractive error, hindering efforts to meet WHO’s 2030 targets for effective refractive error coverage (eREC). AIM: To estimate the prevalence of URE, presbyopia, spectacle use, and eREC among individuals aged 18 to 49 years in Bhutan. METHODS: A population-based cross-sectional survey using the Rapid Assessment of Refractive Error (RARE) methodology was conducted in Bhutan across 61 clusters in 19 districts. Participants ag…

Effects of COVID-19 lockdown and unlock on health of Bhutan-India-Bangladesh trans-boundary rivers.

The COVID-19 pandemic significantly destructs the rhythm of global modern human civilization but worldwide lockdown radically recovers the health of the total environment. The Himalayan trans-boundary rivers provide huge provisional, regulatory and cultural ecosystem services to millions of people throughout the year but in the recent years the water quality is being deteriorated due to multiple reasons. In the last decade, India-Bangladesh political relationship has been slightly broken down due to water sharing and environmental flow of rivers. The COVID-19 lockdown offered a great scope to e

Breastfeeding Mothers' and Nurses' Perceptions of Breast Milk Sharing and Donation for Research in Bhutan: A Qualitative Study.

INTRODUCTION: Breast milk is globally recognized as the optimal source of infant nutrition due to its immunological and neurodevelopmental benefits. In low-and middle-income countries like Bhutan, limited availability of donor milk and absence of milk banking infrastructure hinder neonatal nutrition. While breast milk sharing for feeding has been explored elsewhere, little is known about maternal and nursing perspectives of milk donation for research use within Bhutanese sociocultural context. OBJECTIVE: To explore breastfeeding mothers' perceptions of breast milk sharing and donation for bot…
