A spontaneous external biliary fistula is most commonly a cholecystocutaneous fistula secondary to acute cholecystitis. A fistula arising from an intrahepatic duct is extremely rare. An 87-year-old man presented with swelling of the epigastric region and right upper quadrant abdomen. He had a history of cholecystectomy and endoscopic sphincterotomy. After antibiotic treatment and surgical opening of both lesions, abdominal computed tomography demonstrated a soft tissue mass cephalad to the umbilicus. We excised the mass, and found it to be associated with a fistula through the linea alba. Fist…

We demonstrate a patient with a fistula between the gallbladder and the small intestine combined with a gallstone ileus of the small bowel, diagnosed by ultrasound. Plain abdominal X-ray only revealed small bowel obstruction. All the typical diagnostic criteria of a biliodigestive fistula could be detected by ultrasound. The patient was operated and discharged a few days later. We demonstrate that biliodigestive fistulas can be diagnosed by ultrasound.

An exceptional case is presented of double biliary fistula, internal (to duodenum) and external (to the abdominal wall) in a patient with cholelithiasis of long evolution. The diagnosis of suspicion was made by CAT and was confirmed in the surgical act. Treatment was cholecystectomy, closure of the duodenal communication and external drainage of the parietal abscess. The postoperative evolution coursed without complications.

BACKGROUND: A 38-year-old woman with a 32-year history of recurrent biliary stones (cholecystectomy at age 6 and open choledochotomy with 6-month T-tube indwelling at age 16) presented with 2 h of postprandial right upper quadrant pain. Laboratory tests showed mild hyperbilirubinemia (total bilirubin 24.4 μmol/L, direct bilirubin 8.4 μmol/L) and elevated alanine transaminase of 287 U/L. Abdominal CT scan revealed common bile duct (CBD) stones without pneumobilia. Her symptoms resolved spontaneously before scheduled endoscopic retrograde cholangiopancreatography (ERCP). ERCP showed compensated

Bronchobiliary fistulas are defined as an abnormal communication between the biliary system and the bronchial tree. They are extremely rare complications of radiofrequency or microwave ablation. A 39-year-old woman with a history of neuroendocrine pancreatic carcinoma suffering from liver metastasis was treated with microwave ablation (MWA). In this case report, we present a case of intractable biliptysis from a bronchobiliary fistula secondary to an MWA. The patient was diagnosed by endoscopic retrograde cholangiopancreatograph and hepatobiliary scintigraphy. Treatment involved a right hemihe…

We report the occurrence of common bile duct obstruction and biliary-colonic fistula after open cholecystectomy. Although it is a very unusual complication after cholecystectomy, biliary-colonic fistula should be part of the differential diagnosis for patients presenting with sepsis after open or laparoscopic cholecystectomy. After confirmation and characterization of the injury by endoscopic retrograde cholangiopancreatography and cholangiogram, assessment for undrained collections by computed tomography scan, control of sepsis and coagulopathy, and nutritional support, surgical repair was un
