[High-pressure air-blast injury of the esophagus].

Blast injuries causing pneumatic damage to the oesophagus are very rare. Patients usually present with respiratory distress, subcutaneous and mediastinal emphysema, and may also have pneumothorax. Ruptures need early repair to avoid serious morbidity and potential lethal complications. We report about a successfully treated 47-year-old patient, who has been admitted to our department with compressed air injury of the oesophagus.

Unusual blast colonic injury due to a fall.

Civilian colonic injuries are usually due to penetrating injuries like gun shots, stab wounds and blunt trauma especially following road traffic accidents. Blast injuries are caused by bomb blasts, intracolonic explosion of gases after diathermy, over-enthusiastic bowel insufflation at sigmoidoscopy or by pressure hose applied to the anus. We report the case of a 28-year old man with an unusual blast injury of the colon following a fall from a colanut tree. The transverse colon was sheared off at its two ends while the descending colon was split open along its entire length. There was a delay

Auditory brainstem evoked potentials in blast injury.

Blast injury typically consists of a mixed conductive and sensorineural hearing loss. The sensorineural component includes temporary as well as permanent threshold elevations. Auditory brainstem evoked potentials (ABEP) are sensitive to functional changes in various levels along the auditory pathway. ABEP were recorded from 37 survivors of blasts and latency measures were correlated with clinical findings. Prolongation of peak latencies was correlated with the conductive component of blast-induced hearing loss, as well as with the TTS component of the sensorineural impairment. No central effec

Advance on Blast-Induced Traumatic Brain Injury.

Blast-induced traumatic brain injury (bTBI) is an injury to brain tissue caused by the blast shock wave of an explosive, which is the leading cause of death and disability among military personnel during wartime. By establishing animal models of bTBI that simulate different degrees and types of injuries, it is possible to clarify the neuropathological changes including molecular and cellular mechanisms. The aim of this review is to summarize the establishment of animal models of bTBI, the mechanism, diagnosis, treatment and brain-related diseases of bTBI, which will provide a basis for improvi

Periorbital emphysema in mechanical air blast injury.

A young adult male presented to the emergency department, due to an accidental compressed air injury by high-pressurised air hoses, complaining of left-sided facial pain, swelling and an inability to open his left eye. The left eye appeared oedematous on clinical examination, with swelling extending to the premaxillary region. Additional findings included conjunctival chemosis, palpable crepitus and tears in the inferotemporal bulbar conjunctiva accompanied by subconjunctival haemorrhage. CT of the brain, orbits and facial bones revealed periorbital emphysema around the left orbit, with air po

Prominent Injury Types in Vehicle Underbody Blast.

BACKGROUND: To fully understand the injury mechanisms during an underbody blast (UBB) event with military vehicles and develop new testing standards specific to military vehicles, one must understand the injuries sustained by the occupants. METHODS: Injury data from Service Members (SM) involved in UBB theater events that occurred from 2010 to 2014 were analyzed. Analysis included the investigation of prominent skeletal and visceral torso injuries. Results were categorized by killed-in-action (n = 132 SM) and wounded-in-action (n = 1,887 SM). RESULTS: Over 90% (553/606 SM) of casualties in U

The past and present of blast injury research in China.

With the increasing incidence of blast injury, the research on its mechanisms and protective measures draws more and more attention. Blast injury has many characteristics different from general war injuries or trauma. For example, soldiers often have various degrees of visceral injury without significant surface damage, combined injuries and arterial air embolism. Researchers in China began to investigate blast injury later than the United States and Sweden, but the development is so fast that lots of achievements have been gained, including the development of biological shock tube, the mechan

Blast injury face: An exemplified review of management.

Facial injuries are extremely common due to increased incidence of vehicular and industrial trauma and warfare injuries. But isolated injury to the face due to low voltage cells exploding is rare. In blast injury, the force can cause massive soft tissue injury, along with injury to facial fractures and damage to adnexa. Facial injury is not life threatening unless associated with other injuries of the skull and airway. The major risks to airway in facial trauma are due to anatomic alteration of patient's airway through bony and soft tissue disruption and increased chances of aspiration. The pa

Factors affecting blast traumatic brain injury.

The overlapping pathologies and functional outcomes of blast-induced TBI (bTBI) and stress-related neurobehavioral disorders like post-traumatic stress disorder (PTSD) are significant military health issues. Soldiers are exposed to multiple stressors with or without suffering bTBI, making diagnosis and treatment as well as experimental modeling of bTBI a challenge. In this study we compared anxiety levels of Naïve rats to ones that were exposed to each of the following conditions daily for 4 consecutive days: C I: transportation alone; C II: transportation and anesthesia; C III: transportation

Primary blast injury: update on diagnosis and treatment.

BACKGROUND: : Injuries from combat and terrorist explosions are increasing worldwide. As such, physicians can expect to treat more patients with complex and unique patterns of injury produced not only by fragments and blunt trauma, but also by high-pressure air expanding from the detonation center. DISCUSSION: : Tissue damage from the blast wave or primary blast injury can be an important cause of occult trauma to the ocular, aural, pulmonary, cardiovascular, musculoskeletal, and neurologic systems. Awareness of the extensive corporal effects of the blast wave is an essential prerequisite to

Blast injury ear in a low intensity conflict.

The Eardrum is the most sensitive organ involved i blast injury and can be ruptured at relatively low pressure differentials. This study presents 200 cases of traumatic perforation among service personnel involved in a low intensity conflict, treated in a forward zonal hospital. Most blast injuries of the tympanic membrane (TM) heal spontaneously with conservative treatment (83%).

Perforated tympanic membrane after blast injury.

The rate of spontaneous healing of tympanic membrane perforations in relation to time after blast injury, size, and location of perforations, was evaluated in 147 patients (210 ears). In 46 patients a paper patch was placed over the perforation. Spontaneous healing was seen in 155 ears (74%), 131 (62%) healed within the first 3 months and 145 ears (69%) healed within 10 months. Large and central kidney-shaped perforations had the least tendency to heal and usually required surgical intervention. Tympanoplasty was performed on 33 ears (15%), ossiculoplasty on 5 and mastoidectomy on 10. Interven

Characterization of an Advanced Blast Simulator for Investigation of Large Scale Blast Traumatic Brain Injury Studies.

Blast traumatic brain injury (bTBI) is a prominent military health concern. The pervasiveness and long-term impacts of this injury highlight the need for investigation of the physiological outcomes of bTBI. Preclinical models allow for the evaluation of behavioral and neuropathological sequelae associated with bTBI. Studies have implemented rodent models to investigate bTBI due to the relative small size and low cost; however, a large animal model with similar neuroanatomical structure to humans is essential for clinical translation. Small blast simulators are used to induce bTBI in rodents, b

Post-Concussion Syndrome Following Blast Injury: A Cross-Sectional Study of Beirut Blast Casualties.

INTRODUCTION: The massive 2020 blast in Beirut, Lebanon, caused by improperly stored ammonium nitrate, was one of the most powerful non-nuclear explosions in history, Following the blast, head injuries emerged as a predominant presentation to the emergency department (ED). Blast-induced head injuries can lead to mild traumatic brain injuries (mTBI) mediated via primary blast overpressure without direct head trauma. The recovery process from mTBIs can be prolonged and affected by several factors. If symptoms persist for more than three months, patients should be evaluated for post-concussion sy

Blast-induced biomechanical loading of the rat: an experimental and anatomically accurate computational blast injury model.

Blast waves generated by improvised explosive devices (IEDs) cause traumatic brain injury (TBI) in soldiers and civilians. In vivo animal models that use shock tubes are extensively used in laboratories to simulate field conditions, to identify mechanisms of injury, and to develop injury thresholds. In this article, we place rats in different locations along the length of the shock tube (i.e., inside, outside, and near the exit), to examine the role of animal placement location (APL) in the biomechanical load experienced by the animal. We found that the biomechanical load on the brain and inte
