[Aiming for zero blindness].

Glaucoma is the leading cause of acquired blindness in Japan. One reason that it often leads to blindness is that it can continue to worsen even after effective medical reduction of intraocular pressure (IOP), the only evidence-based treatment. The limitations of current treatments make it critical to identify IOP-independent factors that can cause glaucoma and develop new drugs to target these factors. This is a challenging task, as the pathology of glaucoma is thought to be very complex, with different combinations of factors underlying its development and progression in different patients. …

Corneal blindness and xenotransplantation.

Approximately 39 million people are blind worldwide, with an estimated 285 million visually impaired. The developing world shoulders 90% of the world's blindness, with 80% of causative diseases being preventable or treatable. Blindness has a major detrimental impact on the patient, community, and healthcare spending. Corneal diseases are significant causes of blindness, affecting at least 4 million people worldwide. The prevalence of corneal disease varies between parts of the world. Trachoma, for instance, is the second leading cause of blindness in Africa, after cataracts, but is rarely foun…

[Blindness after prostate biopsy].

We report on a case of sepsis-associated irreversible blindness in a patient after transrectal rebiopsy of the prostate. The patient was on immunosuppressive and long-term antibiotic treatment. Such a severe complication after transrectal biopsy of the prostate is unusual. Peri-interventional antibiotic prophylaxis reduces the general risk for infections after needle biopsy of the prostate. To avoid severe complications, suitable antibiotic prophylaxis in high-risk patients is recommended.

[Congenital stationary night blindness].

Congenital stationary night blindness (CSNB) is a group of genetically heterogeneous retinopathy with characterized clinical and visual electrophysiological abnormality. Five candidate genes associated with the disease have been identified. Clinical, electrophysiological and molecular genetics about CSNB are reviewed in this paper.

Blindness following medial maxillectomy.

Medial maxillectomy via lateral rhinotomy approach is used in the treatment for tumours affecting the lateral nasal wall. The most frequent complications are crusting, epicanthal scarring and epiphora. The authors report a rare case of blindness secondary to indirect optic neuropathy following medial maxillectomy undertaken to treat a malignant melanoma arising in the lateral nasal wall. It is important to keep in mind, the possibility of this rare complication, while patients are being counselled preoperatively for medial maxillectomy.

Blindness from bad bones.

Progressive visual loss is the most common neurologic finding in osteopetrosis. Several mechanisms may explain this phenomenon, including compression of the optic nerves caused by bony overgrowth of the optic canals and retinal degeneration. We report a child with osteopetrosis and progressive visual loss, even though patent optic canals were demonstrated by computed tomography and digital holography. This patient's visual loss was caused by increased intracranial pressure secondary, to obstruction of cerebral venous outflow at the jugular foramen. This case points to the importance of a full

Childhood blindness in Uzbekistan.

PURPOSE: To elucidate the aetiology of childhood blindness in the Republic of Uzbekistan and to assess the needs for future provision of ophthalmic services for children. METHODS: Six hundred and seventy-one children in seven schools for the blind and visually impaired throughout Uzbekistan were examined using the WHO/PBL (World Health Organization Prevention Of Blindness) childhood blindness proforma. The locations were chosen to give a representation of the major areas of population within the country. RESULTS: Of the 671 children examined, 506 (75.4%) were blind or severely visually impai…

WORD MEANING BLINDNESS REVISITED.

Lambon Ralph, Sage, and Ellis (1996) described a patient, JO, who had impaired understanding of written words in the presence of normal comp … ection of the visual input lexicon from the semantic system and labelled the disorder "word meaning blindness". JO's word meaning blindness resulted from a progressive illness that provided us with an opportunity to investigate the pattern of deterioration in this apparently rare form of dyslexia. Over a period of one year we tested her on three occasions w …

Radiation-associated cerebral blindness.

PURPOSE: To report a case of blindness caused by a white-matter injury after whole brain irradiation for metastatic renal cell carcinoma. METHODS: Case report. We performed comprehensive serial neuro-ophthalmologic examinations. RESULTS: Four to 5 months after resection of renal cell metastasis in the left occipital cortex, interleukin-2 therapy, and whole brain irradiation with 3,000 cGy plus a 1,500-cGy boost to the posterior third of the brain, the patient developed a progressive decline in visual acuity in both eyes to hand motions. Magnetic resonance imaging disclosed signal abnormaliti…

[Congenital stationary night blindness].

The clinical and electrophysiological data in 48 cases of congenital stationary night blindness (CSNB) are analysed. The existence of the complete (with only the cone part of the dark adaptation curve) and incomplete (some rod activity also present) form of this anomaly has been confirmed. The Schubert-Bornschein's type of ERG responses corresponds to the complete CSNB. After 12 years of observation period in some patients the diminution of both the a and b potential of the ERG curves but not the extinction of the ERG potentials has been found. PERG curves and PVEP responses in CSNB are normal…

[Unilateral blindness in neurosyphilis].

A 55 year-old patient developed loss of vision in his right eye over a period of a few weeks with the clinical finding of uveitis and papillaedema. Subsequently, also the left eye was affected by a marked deterioration of vision. The neurological findings were normal. The laboratory findings showed a marked elevation of the ESR (> 100 mm within the first hour) and protein electrophoreses showed changes suggestive of chronic inflammation. Analysis of the cerebrospinal fluid revealed a lymphocytic meningitis with 180/3 cells; the positive serological and liquor findings for TPHA and FTA led to t

Childhood blindness in Peru.

A survey of childhood blindness in Peruvian children was done. Although most causes of blindness were due to congenital and hereditary conditions, measles accounted for almost 10% of blindness. With widespread measles immunization, this preventable cause of blindness in children can be eliminated or dramatically reduced.

Methyltestosterone-induced night blindness.

A 59-year-old man presented with a 3-month history of night blindness and a 9-month history of steatorrhea. Both symptoms had appeared after he had begun taking methyltestosterone. Investigations revealed low serum levels of carotene (0.1 mmol/L) and vitamin A (0.4 to 0.7 mmol/L), anomalous colour perception, elevation of the rod threshold by 3.5 log units in dark adaptometry, and decreased b-wave amplitudes in photopic and scotopic electroretinograms. No biochemical evidence of cholestasis was elicited. The symptoms and the biochemical and electrophysiologic abnormalities resolved within 9 mo…

Nutritional blindness in Africa.

Nutritional blindness is loss of useful vision resulting from vitamin deficiency. A malnutrition Xerophthalmia means all the ocular manifestations of inadequate metabolism of vitamin A, nutritional blindness being the end result of the most severe cases. The estimated overall prevalence of nutritional blindness in Africa is very low, below the WHO levels of significance, although isolated clusters of locally high prevalence exist, usually in arid, sparsely-populated regions. The peak age group affected is 2-year olds, with most nutritional blindness having its effect before age 6 years. Xeroph…
