Macrophages preserve endothelial cell specialization in the adrenal gland to modulate aldosterone secretion and blood pressure.

Macrophages play crucial roles in organ-specific functions and homeostasis. In the adrenal gland, macrophages closely associate with sinusoidal capillaries in the aldosterone-producing zona glomerulosa. We demonstrate that macrophages preserve capillary specialization and modulate aldosterone secretion. Using macrophage-specific deletion of VEGF-A, single-cell transcriptomics, and functional phenotyping, we found that the loss of VEGF-A depletes PLVAP+ fenestrated endothelial cells in the zona glomerulosa, leading to increased basement membrane collagen IV deposition and subendothelial fibrosi

Treatment of Unicoronal Craniosynostosis by Rotational Flap Technique Preserves Blood Supply to the Frontal Bone Flap.

BACKGROUND: Surgical correction of unicoronal craniosynostosis (UCS) remains a challenging problem. Long-term results are often unsatisfactory secondary to recurrence of the original deformity, requiring secondary operations such as fat grafting or complete revision of the calvarial remodeling. Distraction osteogenesis (DO) has recently emerged as a new modality for treatment of UCS, with promising results and decreased rates of reoperation compared with open cranial vault remodeling. A theoretical benefit of DO is preservation of blood supply to the frontal bone following osteotomy, as the fr

Blood pressure visit-to-visit variability and outcomes in patients with heart failure with preserved ejection fraction.

AIMS: Previous studies report that blood pressure (BP) variability is associated with increased risk of adverse outcomes in patients diagnosed with cardiovascular disease. However, studies have not fully explored this association in patients with heart failure with preserved ejection fraction (HFpEF). This study sought to explore the association between visit-to-visit variability (VVV) of BP and clinical outcomes in patients with HFpEF. METHODS AND RESULTS: A total of 1988 patients (mean age of 67.73 ± 9.22, 51.7% female) from the Treatment of Preserved Cardiac Function Heart Failure with an

Low-dose dopamine preserves renal blood flow in endotoxin shocked dogs treated with ibuprofen.

Drugs that inhibit prostaglandin (PG) biosynthesis improve hemodynamics and survival in experimental endotoxic and septic shock. The therapeutic utility of these agents in the management of septic patients may be limited, however, by their tendency to decrease renal blood flow (RBF) in animals and humans stressed by experimental manipulations or disease states that promote renal vasoconstriction. In the present study, we addressed this question: can low-dose intravenous (iv) dopamine (4 micrograms/kg/min), a known renal vasodilator, improve renal perfusion in endotoxin-shocked dogs treated wit

Effect of normal saline and half saline preservation serums on sodium levels, blood pressure, and acidosis in PICU patients.

BACKGROUND: This study aimed to evaluate the differential effects of normal saline (N/S) (0.9% sodium chloride) and half saline (H/S) (0.45% sodium chloride) preservation solutions on sodium levels, blood pressure, and acid-base balance in critically ill pediatric patients. METHODS: A double-blind interventional clinical trial involving 88 pediatric patients was conducted in the pediatric intensive care unit. Patients were randomly assigned to receive either N/S (Group A) or H/S (Group B). Sodium, potassium, chloride, bicarbonate, pH, and blood pressure were measured at baseline and 6, 24, 48

Transition from room temperature to cold-stored platelets for the preservation of blood inventories during the COVID-19 pandemic.

BackgroundThe COVID-19 pandemic has placed great strain on blood resources. In an effort to extend platelet (PLT) shelf life and minimize waste, our institution transitioned room temperature to cold-stored PLTs for administration to bleeding patients.Study design and methodsWe describe the administrative and technical processes involved in transitioning room temperature PLTs to cold storage in April 2020. Additionally, we describe the clinical utilization of cold-stored PLTs in the first month of this practice change, with a focus on changes in PLT counts after transfusion, hemostasis, and saf

Diurnal Rhythm in Blood Pressure Is Preserved in Hypertensive Mice Despite Therapy With Mineralocorticoid Receptor Antagonists.

Human arterial blood pressure follows a circadian rhythm, where pressure is highest during the day and lowest during the night. Patients with hypertension can have either a preserved rhythm or a dampened rhythm with a lower day-to-night difference, called "dippers" and "nondippers," respectively. Spironolactone, eplerenone, and the newer finerenone are mineralocorticoid receptor antagonists (MRAs) used for patients with resistant hypertension. In this study, we describe the 24h diurnal cycle in blood pressure in mice with and without hypertension to test whether high blood pressure induced a n

Orthostatic blood pressure reduction is associated with renal damage in functionally preserved elderly hypertensive patients.

BACKGROUND: Blood pressure (BP) exhibits specific characteristics in elderly patients. We evaluated the association between positional BP changes during orthostatism and the presence of hypertension-mediated organ damage (HMOD) in functionally preserved elderly hypertensive patients. METHODS: This was a multicenter, cross-sectional study of 156 hypertensive patients with a mean age of 75 ± 5.8 years (49% women). Central and peripheral office and 24-h BP were measured using an oscillometric device. Orthostatic BP changes were recorded according to the conventional definition of orthostatic hyp

Intensive blood pressure control in heart failure with preserved ejection fraction: a symptom-modifying strategy.

Patients with heart failure with preserved ejection fraction (HFpEF) account for over half of heart failure (HF) cases globally and the presence of HFpEF is closely linked with systemic hypertension, which contributes to the structural, hemodynamic, and symptomatic burden of the disease. HFpEF is a heterogeneous syndrome in which concentric remodeling/hypertrophy and diastolic dysfunction commonly arise from chronic pressure overload (often hypertension) and/or age-related arterial stiffening, metabolic comorbidity, and other myocardial substrates. This review critically examines the rationale

Resting beat-to-beat blood pressure variability is preserved in young adults with major depressive disorder.

Major depressive disorder (MDD) often first emerges during young adulthood and is associated with an increased risk of future hypertension, but our understanding of blood pressure (BP) regulation in young adults with MDD who are otherwise clinically healthy remains limited. We tested the hypothesis that beat-to-beat BP variability (BPV) would be greater in young unmedicated adults with MDD compared with nondepressed healthy adults (HA). Because the arterial baroreflex is essential for beat-to-beat BP regulation, we also hypothesized that 1) sympathetic baroreflex sensitivity would be reduced i

On-treatment systolic blood pressure and preserved kidney function in hypertensive patients with proteinuria. The VALUE Trial.

AIMS: We investigated on-treatment systolic BP (SBP) <130, 130-139 and ≥140 mmHg related to nephroprotection in 3065 patients with proteinuria and 10,738 patients without proteinuria in the VALUE Trial. METHOD AND RESULTS: Worsened kidney function (WKF) was ≥50% increase in serum creatinine, and end-stage kidney disease (ESKD) was dialysis/transplantation. Cox proportional hazards models were adjusted for covariates in the on-treatment SBP groups. Lower SBP was significantly related to less WKF (p < .001) in patients with proteinuria, both at <130 mmHg (n = 14/529, 2.6%) and 130-139 mmHg (n =

Magnetic Nanotrap Particles Preserve the Stability of Venezuelan Equine Encephalitis Virus in Blood for Laboratory Detection.

Most of the modern techniques used for identification of viral-induced disease are based on identification of viral antigens and/or nucleic acids in patient's blood. Diagnosis in the field or in remote locations can be challenging and alternatively samples are shipped to diagnostic labs for testing. Shipments must occur under controlled temperature conditions to prevent loss of sample integrity. We have tested the ability of magnetic Nanotrap® (NT) particles to improve stability and detection of Venezuelan equine encephalitis virus (VEEV), viral capsid protein, and viral genomic RNA in whole h

Blood volume measurements in patients with heart failure and a preserved ejection fraction: implications for diagnosing anemia.

Racial differences in the prevalence of anemia in patients with heart failure have been noted. The diagnosis of anemia in heart failure patients can be confounded by many factors. Plasma volume expansion is one of the most prominent confounders. The authors investigated the difference of anemia prevalence using two different diagnostic techniques: peripheral hemoglobin recommended by the World Health Organization criteria and blood volume (BV) analysis. Racial disparities in the prevalence of anemia using both measures were compared. Sixty patients with heart failure and preserved ejection fra

[Clinical features of cases whose cerebral blood flow was preserved only in the basal ganglia region].

In the series of our studies of positron emission tomography (PET), we had some cases whose cerebral blood flow was reduced in the cerebrum, cerebellum and brain stem, and was preserved only in the basal ganglia region. We studied their clinical features and electrophysiological findings of these cases. These 5 cases included neuronal ceroid lipofuscinosis, Krabbe disease, Tay-Sachs disease, progressive myoclonus epilepsy and subacute sclerosing panencephalitis. Clinically they showed symptoms associated with diffuse cerebral and brain stem involvements. Electrophysiological studies also revea
