New perspectives for the treatment of thoracic aortic aneurysm with self-expanding endoprostheses. Preliminary experience.

We present our preliminary experience with the application of covered aortic stents to treat aneurysms and dissections of the thoracic artery, a technique that was developed in 1996. Seven selected patients were treated with World Medical Talent bare spring tip endoprostheses and followed up for a total of 67 months. All prostheses were implanted at the Parma General Hospital Cardiovascular Department. Seven patients, average age 57.8 years, range 44-73 years, were treated; a total of 11 prosthetic segments were implanted. Aortic pathologies included: 2 isthmic atherosclerotic aneurysms, 2 chr

Novel insights into thoracic endografts technology for prevention of distal stent-graft induced new entry (dSINE) following endovascular repair of type B aortic dissections: from bench to bedside.

INTRODUCTION: Endovascular treatment of type B aortic dissections (TBAD) has currently acquired a primary therapeutic role when anatomically feasible. The main issue with thoracic endovascular aortic repair (TEVAR) for aortic dissection is the actual nature of the aortic wall, which is structurally compromised and more fragile. Indeed, a wealth of data have shown that TEVAR for TBAD will lead, in a substantial proportion of cases, to a device-related adverse event named distal stent-graft induced new entry (dSINE). AREAS COVERED: Currently available aortic stent-grafts have not been specifica

HYBRID AORTIC ARCH SURGERY TO CREATE A LANDING ZONE IN THE ASCENDING AORTA.

Thoracic Endovascular Aortic Repair (TEVAR) has enabled the treatment of aortic pathology that previously required open surgery, with higher morbidity and mortality. The need for a favorable landing zone (Lz), without compromising the patency of the supra-aortic vessels meant that Ishimaru Lz 2 was the most proximal technically feasible Lz. We developed a hybrid technique for the creation of a more proximal Lz in high risk patients - in the first stage, debranching/ rerouting of the supra-aortic vessels, with or without ascending aorta replacement, was performed; a few weeks later, a TEVAR wit

Adverse complications of frozen elephant trunk, do we have enough quality data?

BACKGROUND: Frozen elephant trunk has in recent times become a mainstay for total arch replacement in aortovascular surgery and is indicated to treat a spectrum of complex aortic pathologies. However, despite associated excellent postoperative results it is incredibly important to recognize potential adverse complications such as negative aortic remodeling, endoleak, and distal stent-graft induced new entry so that outcomes can be further improved. AIM OF THE STUDY: Below we provide commentary on a recent article in the Journal of Cardiac Surgery discussing the topic. CONCLUSIONS: Despite th

Frozen Elephant Trunk Completion: Endovascular Extension in The Distal Thoracic Aorta.

INTRODUCTION: Complex pathologies involving the aortic arch can be treated using the frozen elephant trunk (FET) technique, which is versatile and continues to be improved with different innovations to further reduce, for example, circulatory arrest time and the need for hypothermia. FET may or may not be a definitive repair, however. Distal extension or completion-especially endovascular-is common but not well described in the literature. This review describes the considerations that are necessary during FET planning and preparation, how pathology specifics and sizing decisions will affect th

Effects of Stents or Prostanoids on Critical Limb Ischemia.

Peripheral arterial disease (PAD) affects up to 10% of the global population and is often underdiagnosed and inadequately treated until it progresses to critical limb ischemia (CLI) with rest pain, gangrene, and/or ulceration. At this late stage in PAD management, outcomes are typically measured in terms of amputation or mortality. The 5-year mortality rate has been reported to exceed the likelihood of that due to prostate and breast cancer combined. Structured exercise and/or calf muscle stimulation increased collateral circulation, improving pain, quality of life, and mobility outcomes for p

[Surgical policy in treatment of patients with infected aortofemoral bifurcation allografts].

Described in the article are the results of treating a total of 44 patients who endured repeat operations for infection of aortofemoral bifurcation allografts over the period between 2001 and 2018. These patients had previously undergone reconstructive operations using various synthetic grafts for lower limb ischaemia on the background of lower-limb artery atherosclerosis and infrarenal aortic aneurysms. Infection of synthetic vascular allografts in these patients was observed both in the immediate and remote postoperative periods. The dominating infection in 70% of patients was methicillin-re

A systematic review and meta-analysis of endovascular juxtarenal aortic aneurysm repair demonstrates lower perioperative mortality compared with open repair.

OBJECTIVE: The objectives of the present study were to summarize and pool the available data from studies that had directly compared endovascular and open repair of juxtarenal aortic aneurysms. METHODS: OVID Medline and Embase were searched for studies from January 2000 to December 2018 that had compared endovascular vs open repair of juxtarenal aortic aneurysms. Studies that had included patients with pararenal and suprarenal aneurysms were also included. The endovascular interventions included short-neck standard endovascular aneurysm repair (EVAR), parallel grafts, and fenestrated/branched

Electrospinning of biomimetic scaffolds for tissue-engineered vascular grafts: threading the path.

Tissue-engineered vascular grafts (TEVGs) offer an alternative to synthetic grafts for the surgical treatment of atherosclerosis and congenital heart defects, and may improve graft patency and patient outcomes after implantation. Electrospinning is a versatile manufacturing process for the production of fibrous scaffolds. This review aims to investigate novel approaches undertaken to improve the design of electrospun scaffolds for TEVG development. The review describes how electrospinning can be adapted to produce aligned nanofibrous scaffolds used in vascular tissue engineering, while novel p

The role of stent-grafts in the management of aortic trauma.

Stent graft has resulted in major advances in the treatment of trauma patients with blunt traumatic aortic injury (TAI) and has become the preferred method of treatment at many trauma centers. In this review, we provide an overview of the place of stent grafts for the management of this disease. As a whole, TEVAR repair of TAIs offers a survival advantage and reduction in major morbidity, including paraplegia, compared with open surgery. However, endovascular procedures in trauma require a sophisticated multidisciplinary and experienced team approach. More research and development of TAI-speci

Rupture of a normal-sized, non-dissected distal aortic arch in a Marfan patient.

We successfully repaired a rupture of a normal-sized, non-dissected distal aortic arch in a patient with Marfan syndrome. Six years previously she had undergone repair of the thoraco-abdominal aortic aneurysm with a 24-mm knitted Dacron graft for type B chronic aortic dissection. The rupture site was located at the back of the native distal aortic arch just 10 mm above the proximal anastomosis, and just below the left subclavian artery. This unexpected situation might be related to dilatation of the knitted Dacron graft up to 34 mm (142%), thus stretching out the fragile native aorta in this M

Comparison of Biomechanical Properties of the iCover and VBX Stent Grafts When Used as Bridge Stents in Fenestrated Endografts: An In Vitro Study.

OBJECTIVE: The objective of this study was to compare in vitro the different biomechanical properties of 2 balloon-expandable covered stents, VBX (Viabahn VBX, W.L. Gore & Associates, Flagstaff, AZ) and iCover (iVascular, Barcelona, Spain), as bridging stents in fenestrated aortic endoprostheses. METHODS: Three biomechanical tests were performed to evaluate a total of 12 stents (6 VBX 6 × 59 mm and 6 iCover 6 × 57m). First, a 3-point bending test was performed with a calibrated dynamometer to evaluate the bending strength of the 40% and 150% deformation of the system in its crimped state and

Effect of different expansion strategies on coronary stent deployment in a tapered artery.

BACKGROUND: Vascular stenting has been widely used to treat vessel stenosis. However, long-term successes of the procedure are often compromised by late stent thrombosis (ST) and in-stent restenosis (ISR), especially in tapered arteries. OBJECTIVE: The aim of this study was to choose a reasonable expansion strategy for tapered arteries. METHODS: A balloon-expandable coronary stent deployment in a tapered vessel was numerically studied fol-lowing three strategies: (i) selecting the proximal diameter of the tapered vessel as the reference diameter to expand the stent, (ii) selecting the middle

Promoting Angiogenesis Using Immune Cells for Tissue-Engineered Vascular Grafts.

Implantable tissue-engineered vascular grafts (TEVGs) usually trigger the host reaction which is inextricably linked with the immune system, including blood-material interaction, protein absorption, inflammation, foreign body reaction, and so on. With remarkable progress, the immune response is no longer considered to be entirely harmful to TEVGs, but its therapeutic and impaired effects on angiogenesis and tissue regeneration are parallel. Although the implicated immune mechanisms remain elusive, it is certainly worthwhile to gain detailed knowledge about the function of the individual immune

[Devising a Technique for Distal Anastomosis During Total Arch Replacement Using the Frozen Elephant Trunk Technique:Delayed Deployment Technique].

Frozen elephant trunk( FET) technique is an effective procedure used to repair aortic arch aneurysm and aortic dissection. We modified FET technique in order to reduce bleeding, which we named "delayed deployment technique." This procedure is performed in the following manner: 1. A tube graft is anastomosed to the proximal descending aorta using open distal method. 2. A FET is deployed within the tube graft and the descending aorta. 3. Proximal end of the inner FET and the outer tube graft is trimmed at the same position. 4. A four-branched graft is anastomosed to doubly layered distal grafts.

Intraoperative Open Aortoscopy-A New Emerging Technique in Hybrid Aortic Arch Surgery.

Intraoperative open aortoscopy is a new emerging technique in hybrid aortic arch surgery using hybrid open stent grafts for frozen elephant trunk operation. It is performed on a detumescent open aortic arch during the period of deep hypothermic circulatory arrest. It aids in operative planning and choice of open stent graft and confirms proper deployment and distal seal prior to resumption of circulation.
