Limb salvage surgery for primary bone sarcoma of the lower extremities: long-term consequences of endoprosthetic reconstructions.

BACKGROUND: Adjuvant chemotherapy and endoprosthetic replacement for bone sarcomas of the lower extremity is well established. The specific long-term consequences of these endoprosthetic reconstructions for the patient's affected limb are unknown. METHOD: The oncologic results and the survival of the endoprostheses were reviewed in 32 patients with primary bone sarcoma of the femur or proximal tibia. There were 26 high-grade sarcomas, and 6 low-grade sarcomas. A proximal femoral endoprosthesis was used for reconstruction in 4 patients, a total or push-through femoral endoprosthesis in 11 pati

Complex Open Fractures of the Lower Extremity: What is the Optimal Time from Bone Fixation to Flap Coverage?

Shorter delays from presentation to soft tissue coverage in patients with lower extremity open fractures are associated with fewer infections. Orthoplastic teams should know how long flaps can be safely delayed after internal fixation (IF), rather than presentation, because concurrent life-threatening injuries delay limb salvation. We compared infection rates (IRs) of flap coverage delays within 24, 24 to 72, and over 72 hours of IF.This is a retrospective cohort study of adult patients in a Level I trauma center from 2011 to 2021. Patients sustained Gustilo III lower extremity fractures and r

Outcomes of simultaneous versus staged intramedullary nailing fixation of multiple long bone lower extremity fractures.

INTRODUCTION: Repair of multiple lower extremity long bone fractures with intramedullary nail (IMN) fixation is associated with significant cardiopulmonary burden and may result in mortality. These patients are at an increased risk for fat embolism syndrome, pulmonary embolism, Acute Respiratory Distress Syndrome (ARDS), and pneumonia. No standardized guidelines exist to guide treatment of these patients. Further, there is a paucity of data regarding the risk of simultaneous versus staged fixation of multiple long bone fractures that includes both tibial and femoral injuries, as patients with

Long-term follow-up of children treated with the Repiphysis expandable prosthesis for lower extremity bone sarcoma.

Expandable endoprostheses provide a limb salvage option for skeletally immature patients with bone sarcoma of the lower extremities. Initial reports of the Repiphysis prosthesis were encouraging; however, medium-term follow-up revealed high complication rates. We report on the long-term follow-up of a cohort of patients treated with the Repiphysis prosthesis. Eleven patients were included in the study. Data collected included sex, age at surgery, duration of follow-up, site of disease, histologic diagnosis, number of lengthening sessions, amount lengthened, postoperative complications, endopro

3-D lower extremity bone morphology in ambulant children with cerebral palsy and its relation to gait.

Changes in lower-extremity bone morphology are potential mid- to long-term secondary consequences of cerebral palsy (CP), affecting activity. Little is known about the 3-D morphology of lower-extremity bones in children with CP and the association with gait deviations. The main aim of this study was to describe and compare 3-D lower-extremity bone morphology in ambulant children with unilateral or bilateral CP. Secondary aims were to determine whether certain bone parameters were related to the unilateral or bilateral CP and to quantify the association between bone parameters and gait deviatio

Outcome of oscillating saw open osteotomy in two-stage lower extremity bone transport with monolateral frame.

INTRODUCTION: Bone transport techniques have been widely used to solve massive bone defects due to trauma, osteomyelitis or bone tumors. The technique of bone interruption to achieve better new bone formation is a subject of debate. Low-energy osteotomy (LEO) techniques have been proposed as the gold standard. Some authors reject open osteotomy with an oscillating saw (OOS osteotomy), based on the danger of bone tissue thermal necrosis and periosteal damage. To date, however, there is no strong clinical evidence to discourage this high-energy (HEO) bone interruption technique. METHODS: The ai

Modified classification and single-stage microsurgical repair of posttraumatic infected massive bone defects in lower extremities.

Posttraumatic infected massive bone defects in lower extremities are difficult to repair because they frequently exhibit massive bone and/or soft tissue defects, serious bone infection, and excessive scar proliferation. This study aimed to determine whether these defects could be classified and repaired at a single stage. A total of 51 cases of posttraumatic infected massive bone defect in lower extremity were included in this study. They were classified into four types on the basis of the conditions of the bone defects, soft tissue defects, and injured limb length, including Type A (without s

Effects of cast-mediated immobilization on bone mineral mass at various sites in adolescents with lower-extremity fracture.

BACKGROUND: Leg or ankle fractures occur commonly in the pediatric population and are primarily treated with closed reduction and cast immobilization. The most predictable consequences of immobilization and subsequent weight-bearing restriction are loss of bone mineral mass, substantial muscle atrophy, and functional limitations. The purposes of this study were to determine if lower-limb fractures in adolescents are associated with abnormal bone mineral density or content at the time of fracture, and to quantify bone mineral loss at various sites due to cast-mediated immobilization and limited

Bone mineral loss after lower extremity trauma. 62 cases followed for 15-38 years.

62 former patients, who had been treated at our department for tibial shaft fracture (n 38) or knee ligament injury (n 24) 15-38 years earlier, were re-evaluated for post-traumatic osteopenia. 62 age- and sex-matched subjects without fracture served as controls. By means of a Lunar DEXA apparatus we measured the bone mineral density (BMD) in the total body, the hips and special regions of interest (ROI) in the lower extremities. We found a difference in the BMD between the injured and uninjured legs, most obvious in the femur condyle. Measurements of bone mineral loss early after the injury di

Is liquid nitrogen recycled bone and vascular fibula combination the biological reconstruction of choice in lower extremity long bone tumor-related defects?

INTRODUCTION: Combination techniques, which encompass the combined use of vascularized bone grafts with massive allografts or autografts (recycled bone grafts), are especially important in the biological reconstruction of tumor-related lower extremity long bone defects. Liquid nitrogen recycled bone (frozen autograft) and free vascular fibula graft (FVFG) combination, which was coined as the "frozen hotdog (FH)" method by the authors, has not been as widely used nor its outcomes reported for significantly sized patient groups. This study aims to provide an answer to whether FH is a safe and ef

[The cytochemical indicators in patients with II stage hypertension disease and isolated traumas of long tubular bones of lower extremities].

The cytochemical indicators of neutrophils of peripheral blood were analyzed in 30 patients with hypertension disease and isolated traumas of long tubular bones of lower extremities. It is established that in such patients the variations of microbicidal neutrophils systems manifest in the increase of content of cationic proteins and non-significant decrease of myeloperoxidase activity. This is one of mechanisms of immunity support during first 48 hours after trauma.
