The principal psychosocial approach to the treatment of bulimia nervosa has been cognitive-behavioral therapy, the effectiveness of which has been extensively documented in controlled trials, with full recovery (cessation of binge eating and purging) in some 50% to 60% of patients. More recently, interpersonal therapy, first introduced for the treatment of depression, has been shown in preliminary studies to be as effective as cognitive-behavioral therapy, particularly at follow-up. The evidence for effectiveness of psychosocial therapies in bulimia nervosa is first reviewed, and consideration…

Much recent research on bulimia nervosa has focused on the development of pharmacologic treatments for this disorder. Of the various classes of compounds which have been tested to date, only the antidepressants have displayed consistent and reproducible antibulimic efficacy. Double-blind, placebo-controlled trials have demonstrated that virtually all medications with antidepressant properties--tricyclic antidepressants, monoamine oxidase inhibitors, and atypical antidepressants, such as fluoxetine--significantly reduce the frequency of bingeing and purging in both depressed and nondepressed bu…

Anorexia nervosa and bulimia nervosa are at the present moment, two well defined clinical entities among the group of the eating disorders. The psychopathological differentiation of both syndromes has a great importance for diagnosis and therapy. The authors make a phenomenological description, based on case histories of patients with diagnostics of anorexia and bulimia nervosa, and try to establish an approach to the essential symptomatology of those disorders. The presence of affective symptomatology--depressive, but not exclusively--in the eating behaviour disorders in general and particula…

We characterized the naturalistic feeding patterns of 54 women with bulimia nervosa and 11 matched controls over a continuous 24-hr period in a feeding laboratory. Overall, bulimic women consumed more calories in 24 hr (4446 +/- 584 kcal) than did controls (1845 +/- 649 kcal). Bulimic women consumed a wide range of caloric intake, with 44% overeating and 19% undereating in comparison to the range of controls. In addition, bulimics showed a disruption of circadian feeding patterns. For overeating bulimic women, the majority of meals were of normal size and frequency. Increased caloric intake in…

The relationship between childhood sexual experiences and bulimia was examined. Sixty-three females completed the Bulimia Test (BULIT) and the Sexual Events Questionnaire. BULIT scores were positively correlated with greater numbers of sexual experiences, experiences with adults and relatives, and negative responses of fear or shock at the time of the sexual experience. A multiple regression analysis indicated that fear/shock was a significant predictor of BULIT scores, accounting for 44% of the variance. Additional analyses indicated that bulimics reported a greater number of fear reactions f…

Resting metabolic rate was measured in a group of 22 women of normal body weight with bulimia nervosa and in 19 age-, sex-, and weight-matched control subjects. Mean resting metabolic rate of patients was significantly lower than that of controls (5162 +/- 928 vs 5636 +/- 449 kJ/24 h [1229 +/- 221 vs 1342 +/- 107 kcal/24 h]), as was mean fasting blood glucose level (4.0 +/- 0.6 vs 4.6 +/- 0.6 mmol/L). Mean basal thyrotropin level was significantly lower in patients than controls, but other thyroid indexes did not differ. There were no group differences in body fat mass, fat cell size, or lipop…

Patients who have bulimia nervosa and are overweight have received little attention in the medical literature. The authors identified 25 patients who weighed greater than or equal to 130% of their ideal body weight out of a series of 591 patients with bulimia nervosa. This subgroup was contrasted with a sample of 25 patients with bulimia nervosa who were 90% to 110% of their ideal body weight. Members of the overweight bulimia nervosa group were binge-eating and vomiting less frequently than the comparison group but were more likely to be abusing laxatives, and to report a history of self-inju…

Heart rate and vagal tone were assessed during sleep and bed rest conditions in nine patients with bulimia nervosa (BN), six patients with concomitant anorexia nervosa and bulimia nervosa (AN + BN) and five control subjects. During bed rest conditions AN + BN patients had significantly lower heart rates than BN or control subjects. During sleep, the heart rate differed significantly for all three groups. The AN + BN group had a significantly lower heart rate than either the BN or control groups and the BN group had a significantly lower heart rate than controls. On the other hand, measures of …

Anorexia and bulimia nervosa are aetiologically distinct diseases with different treatments. Though it was described more recently, bulimia nervosa occurs more commonly. Accurate diagnosis and early treatment should reduce the mortality of both conditions.

Bulimia is an episodic compulsive urge to overeat often followed by recurrent attempts to lose weight by self-induced vomiting. Seven young women with this eating disorder and associated benign bilateral painless parotid enlargement are described. The glandular swelling was generally intermittent, with parotid enlargement usually developing 2 to 6 days after a binge overeating episode had stopped. Several had hypokalemic alkalosis and a moderate elevation in serum amylase levels. None had clinical evidence of pancreatitis, and a parotid gland biopsy in one patient was normal. The clinician sho…

Persons who contacted the Anorexia/Bulimia Association of Norway for information and stated that they had an eating disorder were asked to participate in this questionnaire study. The answers from the 32 women who fulfilled the DSM-III-R criteria for bulimia nervosa are presented. Usually the women's eating problems had started in the teens after a period of voluntary dieting. The mean duration of bulimia nervosa was six years. 31% had a history of anorexia nervosa. At the time of the study almost all had normal body weight, but nevertheless felt overweight. 78% practised self-induced vomiting…

Bulimia nervosa, a mental illness 4 times more common than anorexia nervosa, is characterized by binge-eating followed by compensatory purging behaviors, which include self-induced vomiting, diuretic abuse, laxative abuse, and misuse of insulin. Patients with bulimia nervosa are at risk of developing medical complications that affect all body systems, especially the renal and electrolyte systems. Behavior cessation can reverse some, but not all, medical complications.
