Is automated electronic surveillance for healthcare-associated infections accurate in the burn unit?

As monitoring requirements for healthcare-acquired infection increase, an efficient and accurate method for surveillance has been sought. The authors evaluated the accuracy of electronic surveillance in multiple intensive care unit settings. Data from 500 intensive care unit patients were reviewed to determine the presence of central line-associated blood stream infection (CLABSI) and catheter-associated urinary tract infection (CAUTI). An electronic surveillance report was obtained to determine whether patients had a blood-line nosocomial infection marker or a urine nosocomial infection marke

Clonality of Providencia stuartii isolates involved in outbreak that occurred in a burn unit.

In order to investigate an outbreak of multidrug-resistant Providencia stuartii that occurred in a burn unit, we analyzed by pulsed-field gel electrophoresis (PFGE) all isolates of P. stuartii collected during 4 months of 2005 from patients and from a tracheal aspirator. Seventeen clinical isolates of P. stuartii, extended-spectrum beta-lactamase (ESBL) producing, were collected from 17 patients. All these isolates were nosocomially acquired. Three other isolates were collected from the aspirator probe, the aspirator reservoir and from the aspirator tube. Three different antibiotypes were iden

Prevalence of toxin producing strains of Staphylococcus aureus in a pediatric burns unit.

The aims of the study were to determine the sites in a pediatric burns unit that were contaminated with Staphylococcus aureus. Samples from the environment in bedrooms and the common room were taken monthly for 6 months using blood agar for total counts and Baird-Parker agar for S. aureus. The air was sampled using an air-sampling device and settle plates. Hard and soft surfaces including bed, blanket, sofa, chair, taps, bathtub, soft toys, locker and cupboard in the same rooms were sampled using contact plates. Swabs were taken from staff monthly for 3 months. S. aureus isolates were tested f

Spinal cord injury after electrical trauma treated in a burn unit.

OBJECTIVE: To analyse the incidence, diagnosis and outcome of spinal cord injury in patients with electrical injuries. PATIENTS AND METHODS: Retrospective analysis of patients with electrical injuries admitted to our Intensive Care Burn Unit over a 5 year period. Among 435 admissions, 57 (13.1% of all admissions) were electrical injuries, due to either electrical flash (n = 34) or high voltage (n = 23). Two cases (8.6% of high voltage injuries) presented signs of spinal cord injury. Both cases presented an acute transverse myelopathy, involving the pyramidal tract, the posterior cords and the

Preliminary experience with cultured epidermal autograft in a community hospital burn unit.

Initial experience with cultured epidermal autograft (CEA) in a community hospital burn unit is described. Five applications of CEA to three patients (mean burn size, 59% total body surface area) were made. Final graft "take" of CEA ranged from 10% to 80%. Healed CEA is cosmetically superior to meshed autograft and appears to form less hypertrophic scar tissue. CEA is more sensitive to infection than meshed autograft. A review of the literature concerning topical antibiotic use with CEA is included. This experience with CEA demonstrates that large burns can be successfully managed with this mo

Protective isolation in a burns unit: the use of plastic isolators and air curtains.

The use of plastic isolators and of an ;air curtain' isolator for protection of patients against infection was studied in a burns unit.Preliminary bacteriological tests showed that very few airborne bacteria gained access to a plastic ventilated isolator; even when the filter and pre-filter were removed from the air inflow, settle-plate counts inside the isolator were much lower than those in the open ward, but the difference was smaller in tests made with an Anderson air sampler, which showed also that fewer large bacteria-carrying particles appeared inside the isolator than outside it. An op

Functional Outcomes in Patients with Hand Burns Receiving Long-Term Occupational Hand Therapy: A Burn Unit's 10-Year Experience.

The hand is the most commonly burned region and often leads to unique functional limitations. Here, we review the associations of long-term outpatient hand therapy (HT) on hand joint range of motion (ROM), activities of daily living (ADL) status, pain, and ability to return to work after burn injury. Adult patients with hand burns admitted to a single American Burn Association-verified burn center from January 2015 to June 2024, with properly documented outpatient HT follow-up, were included. 86 patients were identified who consistently presented for outpatient OT. 75.6% (n = 65) demonstrated

Exploring infection risk factors and multi-drug-resistant organisms (MDROs) in burn intensive care units: a multi-centre case-control study.

BACKGROUND: Despite the widespread incidence of multi-drug-resistant organisms (MDROs) within burn centres, there is a lack of research investigating the risk of infection with MDROs in critically ill burn patients. This study aimed to identify the risk factors associated with the acquisition of MDROs among critically ill burn patients admitted to burn intensive care units (BICUs), and to determine the prevalence of MDROs in this population. METHODS: This multi-centre observational case-control study included adult patients (age ≥18 years) admitted to BICUs between January 2015 and December 2

Toxic epidermal necrolysis: five years of treatment experience from a burn unit.

BACKGROUND: Toxic epidermal necrolysis (TEN) is a serious drug eruption that results in death in approximately 25% to 50% of patients. There is controversy over whether SCORTEN accurately predicts mortality or if treatment interventions such as intravenous immunoglobulin (IVIg) can alter mortality. OBJECTIVES: We sought to determine whether SCORTEN accurately predicts mortality in this cohort, whether IVIg improved survival, and which drugs and medical comorbidities impacted mortality. METHODS: We summarize our experience prospectively over 5 years and 82 patients. Patients either received s

Prevalence and risk factors for development of delirium in burn intensive care unit patients.

Delirium affects 60 to 80% of ventilated patients and is associated with worse clinical outcomes including death. Unfortunately, there are limited data regarding the prevalence and risk factors of delirium in critically ill burn patients. The objectives of this study were to evaluate the prevalence of delirium in ventilated burn patients, using validated instruments, and to identify its risk factors. Adult ventilated burn patients at two tertiary centers were prospectively evaluated for delirium using the Confusion Assessment Method in the Intensive Care Unit (CAM-ICU) for 30 days or until int

Dispersal of methicillin resistant Staphylococcus aureus (MRSA) in a burn intensive care unit.

Methicillin resistant Staphylococcus aureus (MRSA) is a pathogen of special concern in intensive care units (ICUs). The burn units are a very susceptible habitat to colonization and infection events by this organism. In this paper isolation of MRSA from a sepsis case and from samples of the care unit air is described, along with simultaneous circulation of two clones of MRSA. Some peculiar epidemiological features of MRSA in burn intensive care wards are confirmed.

Persistence of a multidrug-resistant Pseudomonas aeruginosa clone in an intensive care burn unit.

Long-term colonization of various body sites with a multidrug-resistant Pseudomonas aeruginosa clone (resistant to piperacillin, cefoperazone, ceftazidime, aztreonam, imipenem, cefepime, cefpirome, ofloxacin, ciprofloxacin, minocycline, and aminoglycosides) with subsequent severe infections in burn patients has not been reported previously. Thirty-nine isolates of multidrug-resistant P. aeruginosa (resistant to ceftazidime and at least three of the agents listed above) recovered from various clinical samples from three patients in an intensive care burn unit from April 1997 to May 1997 and sev

The Changing Landscape of Pediatric Burns in the United Kingdom: A 20-Year Epidemiological Study.

Burns are a leading cause of childhood injury. Analysis of pediatric burn epidemiology is essential for targeting educational and public health strategies, but there is limited available UK data, particularly traversing the COVID-19 pandemic. We hypothesize that the last 20 years have seen significant changes in injury patterns and service provision, with a marked effect from national lockdown measures. This study aims to describe the epidemiology of pediatric burns referrals and admissions within a pediatric burns unit in the United Kingdom over 20 years (2005-2024). A retrospective analysis

Ambient Room Temperatures in a Burn Intensive Care Unit-A Quality Improvement Project.

Introduction: Patients with major burn injuries are particularly susceptible to hypothermia. The ability to maintain and rapidly increase ambient temperatures may reduce the impact of hypothermia and the hypermetabolic response. The purpose of this study was to determine ambient patient room temperatures in a burn intensive care unit (ICU) and to evaluate our ability to adjust these temperatures. Methods: The ambient temperatures of 9 burn ICU patient rooms were recorded hourly over a 6-month period in an American Burn Association-verified burn centre. Temperatures were recorded using wall-mou

The outcome of fungal infections in a burn intensive care unit: a study of 172 patients.

Burned patients are an especially fragile population in which infections are a leading cause of death and morbidity. Fungal infections have become increasingly prevalent in Intensive Care Units (ICUs) and burn ICUs. Management of fungal colonization and infection still constitutes a challenge for clinicians. The aim of this retrospective cross-sectional study was to characterize the population of burn patients with fungal infections admitted to our Burn ICU between January 2013 and December 2015. Patients were characterized according to age, gender, date of admission and exit, type of burn, ty

A Phytophototoxic Injury at a Burns Unit: The Ungraceful After-Effects of the "Common Rue" Plant.

Phytophototoxic burn injuries refer to dermal loss after contact with light-sensitizing plants and subsequent exposure to sunlight, which can at their most severe result in full-thickness burns. Phytophototoxic injuries secondary to the Herb of Grace, otherwise known as Ruta Graveolens or the "Common rue" plant, are very rare. The authors report the first case to have been referred directly and solely managed in a specialist burns unit along with a review of the literature. The authors report a case of a 50-year-old gentleman who suffered a phytophototoxic burn injury to his hand 2 days after
