Antifungals in systemic neonatal candidiasis.

Fungal infections are common in the newborn period, especially among premature neonates, and are responsible for considerable morbidity and mortality. Currently, three classes of antifungals are commonly used in the treatment of systemic fungal infections in neonates: the polyene macrolides (e.g. amphotericin B [deoxycholate and lipid preparations]); the azoles (e.g. fluconazole); and the fluorinated pyrimidines (e.g. flucytosine). The echinocandins (e.g. caspofungin and micafungin) are a newer class of antifungals which shows promise in this population. The available kinetic data on amphoteric

Invasive neonatal candidiasis: an overview.

Until the 1980s, recovery of Candida species from normally sterile body sites in high-risk infants was often dismissed as contamination. Such delay in diagnosis often resulted in death, multifocal disease, or significant morbidity, outcomes that still occur today. Problems establishing the diagnosis of invasive candidiasis persist, while the frequency of this infection in increasingly fragile and smaller premature infants appears to be increasing. Standard culture techniques have limited sensitivity and often take several days for recovery of Candida when positive. Heightened suspicion and imp…

Antifungal pharmacotherapy for neonatal candidiasis.

Candida is a leading cause of late onset infection (> 3 days of age) in the premature infant. Therefore, decisions about the diagnosis and management of infections caused by Candida are commonplace in the neonatal intensive care unit. Despite this fact, there are few comparative trials about treatment of neonatal Candida infections to guide the practitioner. New antifungals have been developed in the past decade and some clinical experience has been reported that can be used to guide the treatment of infants with serious Candida infections. This article reviews recent pertinent data with regar

Systemic candidiasis in four foals.

Four foals were admitted to the neonatal intensive care unit in the first 2 days of life with problems related to birth hypoxia (neonatal maladjustment syndrome, renal failure, necrotizing enterocolitis) and sepsis. Foals were hospitalized for an extended period (35 to 70 days) and received treatment with several broad spectrum antimicrobial agents. Invasive monitoring and treatment procedures included intravenous catheterization, urinary catheterization, and parenteral nutritional and ventilatory support. In each foal, infections of undetermined cause developed, and systemic candidiasis was d…

Oesophageal candidiasis after omeprazole therapy.

Oesophageal candidiasis was diagnosed incidentally at endoscopy in two patients receiving omeprazole therapy. There were no other predisposing factors for the development of candidiasis. The infection was resolved rapidly by anti-candidal therapy and by stopping omeprazole. These findings suggest that gastric acid secretion and physiological reflux of acid into the oesophagus may play a protective role in preventing candida infection.

Abdominal candidiasis in surgical patients.

Although abdominal candidiasis in critically ill surgical patients is becoming increasingly common, optimal management has not been defined. We treated 16 patients with abdominal candidiasis over a 36 month period. Violation of the gastrointestinal tract mucosa was the most common precipitating event (13 patients). Predisposing factors included: CVP catheters, broad spectrum antibiotics, and parenteral hyperalimentation in all patients, H2-blockers/antacids in 14 patients, as well as malnutrition (7 patients), DM (3 patients), alcoholism (3 patients), and steroids/chemotherapy (3 patients). Ca…

Experimental candidiasis in iron overload.

In an attempt to study the influence of iron overload on deep mycotic infection, experimental candidiasis was induced in mice. One group received intravenous injections of colloidal iron (60 mg/kg weight) for three consecutive days before intravenous inoculation of Candida albicans spores (1 X 10(7). The other received the same number of Candida spores without iron overload. The animals in both groups were observed for 28 days. The Candida lesions, regardless of iron administration, were located mainly in the kidney. There was a marked difference in mortality between the two groups, i.e., 40% …

Candidiasis and atrophic tongue lesions.

In an attempt to shed more light on the role of Candida albicans in the genesis of median rhomboid glossitis, a three-part study of normal, atrophic, and cadaver tongues was undertaken. Occasional fungal hyphae were found in 36 percent of the clinically normal tongues. Hyphae were far more numerous in cytologic smears from atrophic tongue lesions than in those from normal tongues. More than 40 percent of the cadaver tongues showed fungal hyphae. The possible role of C. albicans as a cause of median rhomboid glossitis is discussed in the light of these findings.

Aphthoid ulcerations in gastric candidiasis.

Tiny aphthoid erosions appear to represent the earliest detectable radiographic change in gastric candidiasis. In this report, the radiographic appearance of these lesions is correlated with gross and microscopic pathologic anatomy, and the progression of these ulcerations to deeper linear furrows is illustrated. It is hoped that the detection of the gastric candidiasis at this early stage will hasten therapy, and thus decrease the risk of fulminant infection in the immunocompromised host.

Neonatal Invasive Candidiasis: Current Concepts.

Neonatal invasive candidiasis (NIC) is an important cause of morbidity and mortality, especially in preterm and very low birthweight infants, with the incidence being inversely correlated with gestational age and birth weight. Epidemiological studies in high-income countries show Candida albicans and Candida parapsilosis being the commonest species causing NIC, while in low- and middle-income countries NIC is more often caused by non-albicans Candida species with higher rates of antifungal resistance. The increased incidence of fluconazole-resistant C. parapsilosis and multi-drug-resistant Can…

Pathophysiological microenvironments in oral candidiasis.

Oral candidiasis (OC), a prevalent opportunistic infection of the oral mucosa, presents a considerable health challenge, particularly in individuals with compromised immune responses, advanced age, and local predisposing conditions. A considerable part of the population carries Candida in the oral cavity, but only few develop OC. Therefore, the pathogenesis of OC may depend on factors other than the attributes of the fungus, such as host factors and other predisposing factors. Mucosal trauma and inflammation compromise epithelial integrity, fostering a conducive environment for fungal invasion…

Esophageal candidiasis and its diagnosis.

The concomitant use of two distinct sampling methods can enhance the diagnostic yield of esophageal candidiasis during upper endoscopy. Furthermore, when initial staining is inconclusive, the use of additional stains can help establish the diagnosis.

Autoimmune polyendocrinopathy-candidiasis-ectodermal dystrophy.

Autoimmune polyendocrinopathy-candidiasis-ectodermal dystrophy is a rare autoimmune disorder. The clinical spectrum of symptoms is diverse; the diagnosis relying on the presence of at least two out of the three main conditions defining the syndrome: chronic mucocutaneous candidiasis, hypoparathyroidism, and Addison's disease.
