An RCT to compare high flow nasal cannula with standard nasal cannula to prevent desaturation in subjects undergoing EBUS-TBNA: HIFLO-EBUS trial.

BACKGROUND AND OBJECTIVE: Oxygen desaturation events occur commonly during Endobronchial endobronchial ultrasound (EBUS) guided-transbronchial needle aspiration (TBNA) performed under conscious sedation. We hypothesized that high-flow nasal cannula (HFNC) would be superior to conventional nasal cannula (NC) in preventing these hypoxemic episodes. METHODS: We randomized consecutive subjects ≥18 years undergoing EBUS-TBNA to receive oxygen with HFNC or NC. The primary objective was to compare the proportion of subjects experiencing oxygen desaturation events (defined as SPO2 < 90% for at least …

Iatrogenic Fracture of a Peripheral Intravenous Cannula: A Case Report.

INTRODUCTION: Peripheral intravenous cannulation is a routine invasive procedure widely used in healthcare settings. While it is typically associated with minimal risk, uncommon complications such as cannula fracture and intravascular migration may occur. They can result in significant morbidity if diagnosis and intervention are delayed. CASE REPORT: We describe an adult patient who developed an iatrogenic fracture of a peripheral intravenous cannula, with proximal displacement of the fractured segment into the brachial vein. Ultrasonography was employed to identify and localize the intravasc…

The protekduo cannula for acute right ventricular support in thyrotoxicosis.

A 25-year-old female was presented with acute right heart failure (aRHF) and cardiogenic shock secondary to thyrotoxicosis with concomitant acute respiratory failure. A ProtekDuo cannula was placed to provide temporary percutaneous right ventricular assistance and extracorporeal membrane oxygenation (ECMO) in venopulmonary (V-P) configuration, which provided both decompression of the right ventricle (RV) and oxygenation. With treatment of the underlying thyrotoxicosis, the RV function improved and respiratory failure resolved. She was discharged home in good condition. This case details altern…

Weaning from neonatal and pediatric ECMO with stand-by cannula.

The precise moment for weaning a patient off extracorporeal membrane oxygenation (ECMO) is not always easy to establish. Also, mechanical causes may obligate to disconnect the patient from the circuit before the optimal weaning off. In these selected cases, the patient can be disconnected from the circuit and the cannula can be left in place (stand-by cannula) until the patient's stability without ECMO is assured. The aim was to describe our experience with the stand-by cannula. Single-institution, long-term retrospective study in a pediatric tertiary care hospital. Neonatal and pediatric pati…

Arterial Wall Penetration Forces in Needles versus Cannulas.

BACKGROUND: If safety is defined as the diminished ability to penetrate facial arteries, the goal of this study was to investigate whether different-sized cannulas are safer than correspondingly sized needles for the application of facial soft-tissue fillers. METHODS: Two hundred ninety-four penetration procedures of the facial and superficial temporal arteries were performed in four fresh frozen cephalic specimens using both needles (20-, 22-, 25-, and 27-gauge) and cannulas (22-, 25-, and 27-gauge). Continuously increasing force was applied and measured until intraarterial penetration occur…

How to perform a tracheostomy dressing and inner cannula change.

RATIONALE AND KEY POINTS: Proactive tracheostomy management increases patient safety and reduces adverse events. ▶ A cleaning regimen performed every four hours reduces the risk of a blocked tracheostomy cannula, complete tube occlusion and respiratory arrest. ▶ Sterile tracheostomy dressings allow secretions from the stoma to be absorbed and prevent pressure damage from the tracheostomy tube. ▶ Regular dressing changes and skin inspection permit timely identification of inflammatory processes and skin excoriation, enabling prompt treatment to be instigated. REFLECTIVE ACTIVITY: Clinical skill…

The evidence for high flow nasal cannula devices in infants.

High flow nasal cannula (HFNC) devices deliver an adjustable mixture of heated and humidified oxygen and air at a variable flow rate. Over recent years HFNC devices have become a frequently used method of non-invasive respiratory support in infants and preterm neonates that is generally popular amongst clinicians and nursing staff due to ease of use and being well tolerated by patients. Despite this rapid adoption relatively little is known about the exact mechanisms of action of HFNC however and only recently have data from randomised controlled trials started to become available. We describe…

Origin of the cannula for tracheotomy during the middle ages and Renaissance.

The purpose of this article was to trace the historical origin of the inserted cannula during tracheotomy. Tracheotomy is mentioned in most ancient medical texts, but the origin of cannula insertion into the windpipe is unclear. We reviewed the incunabula and Renaissance texts reporting the utilization of surgical cannulas and tracheotomy. The incunabula disclosed extended use of surgical cannulas during the middle ages and Renaissance. Although tracheotomy was advocated in acutely suffocating patients for a disease of the throat termed squinantia or angina, the first report of the procedure w…

Suprachoroidal hemorrhage caused by breakage of a 25-gauge cannula.

A 72-year-old woman had vitreous surgery for epiretinal membrane using the 25-gauge vitrectomy system. During the removal of the cannula at the end of the surgery, half of its tip was noted to be missing. The following day, a severe choroidal detachment associated with a hypotony was found. A second surgery was performed, including drainage of suprachoroidal hemorrhage and choroidal fluid and the removal of the tip of the 25-gauge cannula stuck inside the sclerotomy. The retained cannula tip might have acted as a channel allowing vitreous fluid into the suprachoroidal space, resulting in choro…

The double balloon cannula in the treatment of ruptured abdominal aneurysms.

Two cases of ruptured abdominal aneurysm were successfully treated, using the double balloon cannula, which was devised in our surgical department. After cross-clamping of the descending thoracic aorta and both common iliac arteries, a small incision was made in the aneurysm and the cannula was inserted through this incision into the lumen of the abdominal aorta and positioned at the proximal end of the aneurysm. Then the double balloon was inflated, blocking the blood flow just above the aneurysm. The aneurysm was incised, and grafting was performed, in the usual manner, in a bloodless surgic…

Comparative evaluation of efficacy of oxygenation using high flow nasal cannula vs. conventional nasal cannula during procedural sedation for endoscopic ultrasound: A pilot study.

BACKGROUND AND AIMS: High flow nasal cannula (HFNC) has numerous advantages against conventional oxygen therapy delivery systems. However, there is limited evidence supporting the use of HFNC in endoscopic ultrasound (EUS) under procedural sedation. The aims of this study is to evaluate the efficacy of two different oxygen delivery devices, that is,HFNCand conventional nasal cannula on the oxygenation status of patients during procedural sedation for EUS. MATERIAL AND METHODS: Sixty adult patients undergoing EUS for various ailments were randomized to two groups group HFNC (n=30) and group na…

The Impact of High-Flow Nasal Cannula on Olfactory Function.

BACKGROUND: Oxygen therapy provided via high-flow nasal cannula (HFNC) improves gas exchange lung compliance and results in increased lung expiratory volumes. Previous data indicate that hyperbaric and humid states improve the olfactory thresholds compared to hypobaric and dry conditions. This prospective, observational study aimed to determine the impact of oxygen delivery through HFNC on olfactory function in subjects admitted to the ICU for acute respiratory failure (ARF). METHODS: 30 subjects who were admitted to the ICU for ARF underwent an olfactory sniff test before and after oxygen th…

New lacrimal cannula for probing and irrigation in the tearing patient.

Katena cannula K7-3016 (Katena Products, Inc, Denville, NJ) is a 23-gauge cannula that features an end-opening port for direct irrigation and a tapered tip for ease of entry into an undilated punctum. The 13-mm length makes it ideal to probe as well as irrigate the proximal lacrimal system. Katena cannula K7-3016 eliminates the need for punctal dilation and placement of Bowman probes to dilate the punctum and measure canalicular obstruction, respectively. The end-opening, direct irrigation design prevents misdiagnosis in the setting of canalicular stenosis.
