It was found that clinical strains of Ps. aeruginosa produced inducable beta-lactamase in the presence of penicillins, such as benzylpenicillin, ampicillin, carbenicillin. The amount of beta-lactamase depended on the strain and the inductor type and increased with the inductor concentration increase. From November 1988 to March 1989, stools from 804 Malagasy children were examined, thirty-seven strains of Shigella were isolated, from which 5 S. dysenteriae serotype 1 (= Shiga bacillus) strains were found to be resistant to ampicillin, carbenicillin, streptomycin, chloramphenicol, tetracycline, sulphonamide and even to trimethoprim. The beta-lactam antibiotic susceptibility and beta-lactamase activity of 22 Yersinia kristensenii strains were determined in order to compare these properties with those reported for Y. enterocolitica. The in vitro activities of Ro 17-2301 and aztreonam against 191 Pseudomonas aeruginosa isolates were compared with those of five other beta-lactam antibiotics. Both compounds showed activities comparable to that of ceftazidime and were bactericidal. They were as effective against gentamicin-or carbenicillin-resistant isolates as against susceptible ones.

The activity of enoxacin, a new quinolone carboxylic acid, was evaluated against 3014 clinical isolates of Enterobacteriaceae, Pseudomonas and other non-fermenters and Staphylococcus aureus. Comparison was made with gentamicin, tobramycin, amikacin, netilmicin, ampicillin, piperacillin, carbenicillin, ticarcillin, ticarcillin plus clavulanic acid, trimethoprim, cotrimoxazole and erythromycin. In general enoxacin was the most active compound and resistance was only rarely encountered.

The in vitro sensitivity of 300 nosocomial strains of Pseudomonas aeruginosa to ceftazidime was compared with their sensitivities to eleven other beta-lactam antibiotics. Concentrations of 2 mg/l of ceftazidime were sufficient to inhibit all of the strains tested including the carbenicillin-resistant ones. Ceftazidime shows considerably greater activity against Ps. aeruginosa than the other beta-lactam antibiotics.

We have compared the in-vitro bacteriostatic and bactericidal activity of ceftazidime against cephalothin-resistant Gram-negative bacteria, Haemophilus influenzae and Gram-positive bacteria. Comparison was made with cefaperazone, cefuroxime, moxalactam, cefotaxime, cefamandole, cefoxitin, cephalothin, cefazolin, azlocillin, carbenicillin, ticarcillin, ampicillin and oxacillin. In general ceftazidime was one of the most active compounds with low MIC and MBC values.

The neurological sequelae of malignant external otitis (MEO) form a characteristic syndrome. Following Pseudomonas external otitis, usually in an elderly, diabetic patient, either isolated facial nerve paralysis or multiple cranial nerve palsies develop. Once extensive neurological signs have developed, recovery rarely occurs. We saw a patient with MEO and multiple cranial nerve palsies who recovered following an extended course of gentamicin sulfate and carbenicillin disodium therapy.

Cefsulodin was the most active of the cephalosporins and exhibited 4-16 times more activity than carbenicillin or ticarcillin against 50 clinical isolates of Pseudomonas aeruginosa. Azlocillin and piperacillin showed good activity, while tobramycin was the most effective aminoglycoside. The activity of cefsulodin was unaltered by increases in inocula, but resistance was induced easily. When combined with gentamicin, no synergistic or antagonistic activity was observed against multiply resistant isolates.
