Human heart preservation for transplantation commonly involves infusion of cold cardioplegic solutions and subsequent immersion in the same solution. The objectives of the present study were (1) to establish the temporal relationship between storage time (at 10 degrees C) and the postischemic recovery of function in the isolated rat heart, (2) to assess, by metabolic and functional measurements, whether storing the heart in fluid as opposed to moist air had any effect on the viability of the preparation, and (3) to ascertain the optimal storage temperature. Isolated rat hearts (at least 6 in e

Myocardial protection during cardiac surgery is a multifaceted process that is structured to limit injury and preserve function. Evolving techniques use solutions with varying constituents that enter the systemic circulation and alter intrinsic systemic concentrations. This study compared two distinct cardioplegia solutions on affecting intraoperative glucose levels. Data were abstracted from a multi-institutional perfusion registry, including a total of 1,188 propensity-matched cases performed from January through October 2016, at 17 cardiac surgical centers across the United States in which

Administration of catecholamines to newborn infants may potentiate reperfusion injury because of increased transsarcolemmal Ca2+ influx and the presence of less developed sarcoplasmic reticulum in the immature hearts. We investigated the effect of administration of epinephrine (1.5 micrograms/kg per minute for 120 minutes) before ischemia and modified serum ionized Ca2+ concentrations in the cardioplegic solution and perfusate on postischemic left ventricular systolic and diastolic function in 25 piglets (5 to 7 days old) undergoing 90 minutes of cold blood cardioplegic arrest. The piglets wer

This case report describes a pediatric cardiac surgery in which electrocardiographic (ECG) changes appeared immediately after the initiation of cardiopulmonary bypass (CPB), followed by severe bradycardia and ultimately flaccid cardiac arrest within minutes. Lab analysis revealed severe hyperkalemia (13.1 mEq/L) and metabolic acidosis, strongly suggesting contamination with cardioplegia solution in the CPB circuit system. CPB was continued, and the heartbeat resumed after potassium removal via hemodialysis, allowing the surgery to proceed as planned. Despite the safe design of CPB and cardiopl

The presence of cold agglutinins (CA) during cardiac surgery with cardiopulmonary bypass usually creates the need for an altered surgical plan. In this case, the CA were discovered after the initiation of bypass, limiting the time, and cardioplegia solutions that could be used in the new approach. The inability to cannulate the coronary sinus with a retrograde cardioplegia catheter excluded the standard approach to myocardial preservation with CA of using continuous warm blood. For this case, we used intermittent cold crystalloid delivered via the antegrade needle for the first half of the pro

We compared the efficacy of Belzer UW solution (UWS) with modified Collins solution (MCS) in a 24 h canine heterotopic heart-lung transplantation model. Nine pairs of mongrel dogs were divided into 2 groups: MCS group (n = 5) and UWS group (n = 4). The donor heart was arrested with cardioplegia. The heart and lung were flushed with MCS or UWS, excised en bloc and immersed in 4 degrees C MCS or UWS for 24 h. Graft function was adequate in 1 of 5 grafts in the MCS group and in all 4 grafts in the UWS group (P less than 0.05). In the UWS group, the prepreservation and posttransplantation cardiac

Cardiac catheterization in a 57-year-old man disclosed a left coronary artery-coronary sinus fistula with annuloaortic ectasia and severe aortic regurgitation. During surgery, the fistula vessel was exposed and clamped before infusing cardioplegic solution for cardiac arrest. The proximal orifice of the fistula was closed directly and the distal orifice doubly ligated. A modified Bentall's procedure was conducted without difficulty. The postoperative course was uneventful.

BACKGROUND: Because of the decreased tolerance to ischemia and increased reperfusion injury in hypertrophied myocardium, myocardial hypertrophy is a well known risk factor for cardiac surgery. We have previously demonstrated in a left ventricular hypertrophy (LVH) model that a highly buffered cardioplegic solution (HBS) that provided glucose as a substrate and promoted anaerobic glycolysis during ischemia afforded superior myocardial protection when compared to standard formulations. And we reported the superiority of this cardioplegia in human cardiac surgery.
METHODS: In this study, 16 pati

At present, intimal dissection, restenosis, or vessel closure occurs in approximately 5 to 10% of patients undergoing percutaneous transluminal coronary angioplasty. Coronary artery bypass grafting is usually required to remedy this complication and prevent substantial myocardial damage. The results of these revascularization procedures, however, are less satisfactory than those of elective coronary bypass grafting. Hemodynamic instability of the patients and the presence of ongoing myocardial ischemia usually necessitate that the operations be performed on an emergent basis. This report descr

Efforts to extend myocardial preservation for transplantation by crystalloid perfusion have been limited by edema and compromised function. We hypothesized that hypothermic perfusion preservation with a polyethylene glycol (PEG) conjugated hemoglobin solution may extend preservation times. The purpose of this study was to compare cardiac function after continuous perfusion by using a hypocalcemic, normokalemic crystalloid perfusate with and without the addition of PEG-hemoglobin (Hb). The hearts of 20 anesthetized and ventilated New Zealand White rabbits were harvested after cold cardioplegic

UNLABELLED: Impaired myocardial metabolism after cardioplegic arrest results in persistent anaerobic lactate production. Insulin may protect the heart from ischemia and reperfusion by enhancing myocardial metabolic recovery. However, the stimulation of glycolysis during ischemia may be detrimental because of an accumulation of metabolic end-products. We examined the effect of insulin on quiescent human ventricular cardiomyocytes subjected to simulated cardioplegic ischemia and reperfusion.
METHODS: Primary cardiomyocyte cultures were established from patients undergoing corrective repair of t

A best evidence topic in cardiac surgery was written according to a structured protocol. The question addressed was 'For the [cold static storage of donor hearts] does the [cardiac preservation solution] impact on [heart transplant survival]?'. Altogether 182 papers were found using the reported search, of which 9 represented the best evidence to answer the clinical question. The authors, journal, date and country of publication, patient group studied, study type, relevant outcomes and results of these papers are tabulated. A wide variety of both custom and standardized cardiac preservation so

BACKGROUND: The contemporary results of coronary artery bypass grafting using a variety of myocardial preservation techniques are excellent. In recent years, the number of "high-risk" patients referred for operation has increased, thus necessitating continued advances in surgical myocardial protection.
METHODS: In this article, we review recent advances in clinical myocardial protective techniques and emphasize studies conducted at The Toronto Hospital. Further, on the basis of promising current research, we speculate on future prospects for myocardial protection.
RESULTS: At The Toronto Hos

The effects of phosphoenolpyruvate (PEP) and ATP in cardioplegia and during early reperfusion were studied in pigs. Twenty pigs divided into three groups were placed on cardiopulmonary bypass and subjected to 2 h of hypothermic cardioplegic arrest. Group I (n = 7) served as a control. In group II (n = 7) PEP (14.4 mM) and ATP (0.067 mM) were added to the cardioplegic solution. Group III (n = 6) was given PEP (28.8 mM) and ATP (0.134 mM) with 500 ml of isotonic NaCl in the aortic root during early reperfusion. All animals in group III were weaned from bypass compared with 6 of 7 in group I and

Pediatric myocardium is unique from mature myocardium; thus, the use of adult cardioplegia for pediatric cardiac operations may provide suboptimal myocardial protection. It is found that children undergoing heart surgery show evidence of less myocardial damage when del Nido cardioplegia is used instead of a standard cardioplegic solution. Del Nido cardioplegia solution provides a depolarized hyperkalaemic arrest lasting up to 60 minutes, reduces spontaneous and inducible activity during arrest, and prevents hyper contraction during early reperfusion. In this single blind randomized trial, a to

The nucleoside adenosine is used clinically for the treatment of paroxysmal atrioventricular junctional tachycardia. Surgically oriented experimental studies indicate an improvement of myocardial protection when adenosine is administered as an adjunct to cardioplegic protection. This regards both postischemic functional recovery and the regeneration of myocardial energy-rich phosphates. However, as yet it remains to be determined which of the various effects of adenosine is truly protective. Likewise it is still unclear if the beneficial effects of "ischemic preconditioning" may be simulated b

A case of successful patch angioplasty for the right coronary artery (RCA) orifice dissection following selective coronary perfusion is reported. A 56-year-old woman who had mitral restenosis, aortic stenosis, and atrial fibrillation with bradycardia-tachycardia syndrome was referred to our hospital for operation. The operation, which contained aortic valve replacement and mitral commissurotomy, was performed with hypothermic cardiopulmonary bypass and crystalloid cardioplegic arrest. The ascending aorta was opened, and selective coronary perfusion was performed. The right coronary cannula was
