Beyond the Heart: The Significance of Depression in Cardiac Surgery.

OBJECTIVES: Preoperative depression is common among patients with cardiovascular disease and a significant risk factor for worse outcomes after cardiac surgery. This review summarizes the current evidence on depression as a risk factor and possible treatment options in patients undergoing cardiac surgery, and highlights future perspectives for clinical research. METHODS: This narrative review was based on a selection of key papers, identified through collegial expert discussions rather than a systematic literature search. RESULTS: Depression is often underdiagnosed and undertreated in patien

Understanding Electrocardiography in Adult Patients With Congenital Heart Disease: A Review.

IMPORTANCE: Congenital heart disease in adults is still a relatively new concept for many cardiologists, and the complexity as well as diversity of cardiac phenotypes encountered necessitate that systematic, practical information be available for the nonspecialist. The analysis of the 12-lead electrocardiogram is an invaluable cornerstone in the clinical appraisal of these patients. OBSERVATIONS: Consideration of the main anatomic and pathophysiological aspects of the various congenital heart conditions can shed light on their distinctive electrocardiogram patterns, which are an electrical re

[Different surgical approaches to Scimitar syndrome].

Scimitar syndrome is a rare congenital heart defect characterized by the combination of vascular, bronchial, and parenchymal malformations. This syndrome includes anomalous right pulmonary venous drainage to the inferior caval vein, hypoplastic right pulmonary artery, right lung hypoplasia and the presence of aortopulmonary collaterals to the right lung. In this study, we evaluate the different surgical approaches of 3 cases with Scimitar syndrome who was corrected successfully.

Radiographic and Clinical Brain Infarcts in Cardiac and Diagnostic Procedures: A Systematic Review and Meta-Analysis.

BACKGROUND AND PURPOSE: The incidence of periprocedural brain infarcts varies among cardiovascular procedures. In a systematic review, we compared the ratio of radiographic brain infarcts (RBI) to strokes and transient ischemic attacks across cardiac and vascular procedures. METHODS: We searched MEDLINE and 5 other databases for brain infarcts in aortic valve replacement, coronary artery bypass grafting, cardiac catheterization, and cerebral angiogram through September 2015. We followed the PRISMA (preferred reporting items for systematic reviews and meta-analyses) recommendations. We defined

Role of Computational Modelling in Planning and Executing Interventional Procedures for Congenital Heart Disease.

Increasingly, computational modelling and numerical simulations are used to help plan complex surgical and interventional cardiovascular procedures in children and young adults with congenital heart disease. From its origins more than 30 years ago, surgical planning with analysis of flow hemodynamics and energy loss/efficiency has helped design and implement many modifications to existing techniques. On the basis of patient-specific medical imaging, surgical planning allows accurate model production that can then be manipulated in a virtual surgical environment, with the proposed solutions fin

Intracardiac echocardiography: evolving use in interventional cardiology.

Intracardiac echocardiography (ICE) uses a catheter-based steerable ultrasound probe that is passed into the right heart chambers to image intracardiac structures. The transducer can be variably positioned for optimal imaging: in the inferior vena cava to visualize the abdominal aorta; in the right atrium for the interatrial septum, aortic, mitral, and tricuspid valves, and pulmonary veins; or in the right ventricle for the left ventricular function, outflow tract, or pulmonary artery. Intracardiac echocardiography is primarily used for imaging during an invasive cardiac procedure using consci

An interrupted suture technique for ascending aortic replacement concomitant to aortic valve repair.

While running suture anastomoses represent the mainstay of thoracic aortic surgery, advantages may exist for interrupted techniques in selected cases, especially with ascending aortic replacement and concomitant aortic valve repair. Interrupted suture anastomoses precisely reconstruct the sinotubular geometry in dissections, and allow the proximal suture line to be placed close to the coronary arteries and aortic valve posts. With aneurysm extension into the aortic arch, interrupted suture techniques allow hemi-arch replacement without the need for aortic clamp removal and circulatory arrest.

Technical modification of the neoaortic reconstruction in Norwood procedure: sparing of anterior wall of the main pulmonary artery.

The growth potential of the neoaorta after the stage I Norwood procedure could be limited when the patient has a small ascending aorta. Patients with hypoplastic left heart syndrome usually have a large main pulmonary artery (MPA) and from its posterior wall both pulmonary arteries arise. We describe a modified technique that spares a large anterior wall of the MPA in the neoaortic reconstruction of Norwood procedure to enhance the growth potential of the neoaorta.

Scimitar syndrome.

Scimitar syndrome is a rare congenital anomaly consisting in part of right pulmonary venous return to the inferior vena cava. There is a clear bimodal presentation of this syndrome with either an infantile manifestation or a pediatric/adult form. The infantile variant is marked by a higher incidence and severity of associated defects, heart failure, pulmonary hypertension, and significant mortality. The patient with the pediatric/adult form is less severely affected and may be asymptomatic on diagnosis. In this article, we review the historical aspects, presentation, and pathophysiology of Sci

[[Aortic reconstruction in neonates with hypoplastic left heart syndrome] ].

The surgical technique and tactics of the Norwood operation in neonates with hypoplastic left heart syndrome represent a surgical challenge. The Norwood operation was performed from the midline sternotomy approach, in extracorporeal circulation and deep hypothermic circulatory arrest was used for reconstruction of the hypoplastic aorta. Operation consisted of reconstruction of the "neoaorta" and the aortic arch from the original hypoplastic ascendent aorta, pulmonary trunk and a patch cut from a pulmonary homograft or pericardium, excision of the atrial septum and an arterial shunt from a Gore

Aortic valve disease in Marfan syndrome.

The Marfan syndrome patient undergoes care by many different physicians for the treatment of the varied systems affected by this connective tissue disorder. The most frequent visits are to a cardiologist, with referral to a cardiovascular surgeon who attends to the problems of dilatation and dissection of the ascending aorta. Follow-up is lifelong. Although currently some surgeons prefer to resuspend rather than replace the aortic valve, composite valve graft replacement for aortic root dilatation and aortic valve insufficiency has steadily improved patient outcome. At the same time, the almos

Cardiac Surgery: Increasing Access in Low- and Middle-Income Countries.

PURPOSE OF THE REVIEW: Low- and middle-income countries (LMICs) have long-battled communicable diseases, and now, a rise in non-communicable diseases (NCD) is conferring tremendous burden in these areas. Cardiovascular disease (CVD) remains the number one cause of death among NCDs across the globe. The current review provides insight regarding this disease burden and highlights challenges as well as strategies for establishing functional cardiac surgery centers and sustainable access to comprehensive cardiovascular care within LMICs. RECENT FINDINGS: Without effective prevention and treatment

Graft preservation solutions in cardiovascular surgery.

Vein grafts are still the most commonly used graft material in cardiovascular surgery and much effort has been spent in recent years on investigating the optimal harvesting technique. One other related topic of similar importance remained more or less an incidental one. The storage solutions of vein grafts following procurement and prior to implantation are, despite their assumed impact, a relatively neglected theme. There is no doubt that the endothelium plays a key role in long-term patency of vein grafts, but the effects of the different storage solutions on the endothelium remain unclear :
